Cyclic Vomiting Syndrome: A Functional Disorder.

Cyclic vomiting syndrome (CVS) is a functional disorder characterized by stereotypical episodes of intense vomiting separated by weeks to months. Although it can occur at any age, the most common age at presentation is 3-7 years. There is no gender predominance. The precise pathophysiology of CVS is not known but a strong association with migraine headaches, in the patient as well as the mother indicates that it may represent a mitochondriopathy. Studies have also suggested the role of an underlying autonomic neuropathy involving the sympathetic nervous system in its pathogenesis. CVS has know

Effectiveness of dexmedetomidine for emergence agitation in infants undergoing palatoplasty: a randomized controlled trial.

OBJECTIVES: In infants, there is a high incidence of emergence agitation (EA) after sevoflurane (Sev) anesthesia. This study aimed to test the hypothesis that dexmedetomidine (Dex) administration would reduce the incidence and severity of EA after Sev-based anesthesia in infants undergoing palatoplasty. METHODS: A prospective randomized clinical trial was conducted with 70 patients undergoing palatoplasty, aged 10-14 months. Infants were randomly allocated into two groups: Dex (n=35) and saline (n=35). In the Dex group, Dex (6 μg/kg/h) was administered approximately 10 min before the end of t

Multiple non-branching dissecting aneurysms of the mid-basilar trunk presenting with sequential subarachnoid hemorrhages.

OBJECTIVE: We describe a rare case of a patient with subarachnoid hemorrhage (SAH) due to a ventral dissecting mid-basilar aneurysm that was treated surgically. One week after surgery, the patient experienced sudden deterioration due to a new SAH caused by the development of a new aneurysm of the basilar trunk distinct from the previously clipped aneurysm. CASE DESCRIPTION: A 54-year-old woman with acute subarachnoid hemorrhage was found to have a small, broad-based aneurysm arising from the ventral aspect of the mid-basilar artery. This complicated lesion was treated with a microsurgical cli

Allodynia corresponding to the levels of cervical cord injury treated by surgical decompression: a report of 3 cases.

BACKGROUND: The role and timing of surgical decompression for SCI remains controversial, when the surgical outcomes are evaluated only by neurologic recovery. Other than neurologic deficits, severe pain after SCI is a significant problem, deteriorating the patient's activity of daily living. In the present report, allodynia of patients' upper limbs caused by cervical SCI was treated successfully by surgical decompression. CASE DESCRIPTIONS: Three male patients received cervical SCI through minor accidents. They complained of allodynia and motor dysfunction at the spinal level compressed by pr

Blood flow volume changes in the maturing arteriovenous access for hemodialysis.

In our center, we start hemodialysis using arteriovenous accesses empirically 1 mo after surgery in nearly all patients, when the vein diameter reaches 5 mm and blood flow is assumed to be adequate. We measured blood flow and vessel diameter in the maturing autogenous and prosthetic access to determine if this approach can be justified by quantitative physiological parameters. Of 66 consecutive autogenous and prosthetic arteriovenous accesses created over 3 mo in 2004, 62 were prospectively examined by duplex ultrasonography preoperatively, immediately after surgery in the recovery room, at 10…

Incidence and significance of pneumomediastinum after laparoscopic esophageal surgery.

BACKGROUND: Pneumomediastinum can be a sign of esophageal perforation. During laparoscopic esophageal surgery, the mediastinum is exposed to carbon dioxide gas under pressure that can cause pneumomediastinum. METHODS: Forty-five patients undergoing laparoscopic esophageal procedures had erect, inspiratory, single-view chest radiographs (CXR) performed in the recovery room (RR). Patients with extraabdominal gas underwent daily erect, inspiratory, single-view CXR until resorption of the gas or discharge from the hospital. Insufflation time and pressure were recorded, and morbidity was evaluated…
