The Influence of the Menstrual Cycle on Electrical Thresholds for Sensory and Pain Perception: Implications for Exercise and Rehabilitation in Women With and Without Primary Dysmenorrhea-A Pilot Study.

Background: Hormonal fluctuations during the menstrual cycle (MC) influence pain perception, potentially affecting exercise performance and rehabilitation in women. This effect may be more pronounced in individuals with primary dysmenorrhea (PD), requiring tailored physiotherapeutic and exercise interventions. Objective: To analyze the influence of MC phases on sensory electrical threshold (SET) and pain electrical threshold (PET) in eumenorrheic women with and without PD, considering the potential implications for physical activity and rehabilitation. Methods: An observational longitudinal st

Convergent validity of a person-dependent definition of a low back pain flare.

Exacerbations of existing low back pain (LBP) or new LBP episodes are colloquially referred to as "flares". Although the experience of flares is common to many people with LBP, few validated measures enable people to self-report if they are experiencing a flare. This study examined the convergent validity of a person-dependent definition of flares ("a worsening of your low back pain that lasts from hours to weeks") as compared to (1) LBP intensity, (2) LBP-related pain interference, and (3) analgesic use, in a large, prospective research study of Veterans with LBP. Veterans seeking care for LB

Factors predicting orofacial pain patient satisfaction with improvement.

AIMS: To determine psychosocial predictors of patients' ratings of satisfaction with improvement and subjective pain relief. This study also examined the underlying components of patient satisfaction with improvement, as assessed at follow-up. METHODS: The sample consisted of 107 chronic orofacial pain patients evaluated at a university-based orofacial pain clinic and referred for treatment with individualized treatment plans. Pain and psychosocial functioning were assessed with standard, reliable, validated self-report instruments administered at the initial evaluation. Follow-up data were c

Structured Differential Diagnosis of Orofacial Pain Associated with an Enamel Crack Using ICOP and ICHD-3: A Case Report.

Background: Cracked teeth may present with variable and atypical symptoms, sometimes mimicking non-odontogenic orofacial pain conditions, ma … n of Headache Disorders (3rd edition). Diagnostic local anaesthesia eliminated both biting pain and referred pain, supporting an odontogenic source. Collectively, the findings suggested that the enamel crack was the most likely source of odontogenic pain, although a definitive causal relationship could not be established. Because pulp sensibility remained norma …

Developing biopsychosocial competence in musculoskeletal pain care: Australian physiotherapy educators' perspectives.

IMPORTANCE: A biopsychosocial approach serves as an important paradigm underpinning musculoskeletal pain education training. Understanding how educators teach this approach in their training may provide insights on how to improve the clinical translation and adoption of a biopsychosocial model in musculoskeletal pain care. OBJECTIVE: This primary qualitative study explored physiotherapy educators' perspectives on translating the biopsychosocial approach to pain care from knowing ('what') to doing (the 'how' of clinical adoption). METHODS: Semi-structured interviews were conducted with 18 pur

Acute Abdominal Pain in Children: Evaluation and Management.

Acute abdominal pain in children is a common presentation in the clinic and emergency department settings and accounts for up to 10% of childhood emergency department visits. Determining the appropriate disposition of abdominal pain in children can be challenging. The differential diagnosis of acute abdominal pain, including gastroenteritis, constipation, urinary tract infection, acute appendicitis, tubo-ovarian abscess, testicular torsion, and volvulus, and the diagnostic approach vary by age. Most causes of acute abdominal pain in children are self-limited. Symptoms and signs that indicate r

'I worked until my body was broken': an ethnomedical model of chronic pain among North Korean refugee women.

OBJECTIVES: While increasing numbers of North Korean refugees (NKRs) are migrating to South Korea, different understandings of physical pain between NKR patients and the South Korean healthcare providers may create miscommunication and less satisfaction with their care management. To identify strategies to improve care, this study elicits and presents an ethnomedical model of chronic pain among NKR women. DESIGN: Twenty semi-structured individual interviews were conducted with NKR women with chronic pain living in South Korea. Inductive and deductive thematic analysis was performed on three t

Predicting the evolution of neck pain episodes in routine clinical practice.

BACKGROUND: The objective of this study was to develop models for predicting the evolution of a neck pain (NP) episode. METHODS: Three thousand two hundred twenty-five acute and chronic patients seeking care for NP, were recruited consecutively in 47 health care centers. Data on 37 variables were gathered, including gender, age, employment status, duration of pain, intensity of NP and pain referred down to the arm (AP), disability, history of neck surgery, diagnostic procedures undertaken, imaging findings, clinical diagnosis, and treatments used. Three separate multivariable logistic regress

Viscosupplementation with hylan G-F 20 (Synvisc): pain and mobility observations from 74 consecutive patients.

Seventy-four patients (120 knees, 360 injections) referred to an orthopedic surgery clinic by their primary care physician for osteoarthritis knee pain received 3 weekly intra-articular injections of hylan G-F 20 (Synvisc; Genzyme Biosurgery, Ridgefield, NJ). Osteoarthritic knee pain, the ability to perform activities, and the amount of pain medication, physiotherapy, and assistive device use were prospectively evaluated 1 month and 1 year after treatment. The majority of patients reported improved or much improved pain at 1 month (99%) and 1 year (91%) after hylan G-F 20 therapy. Patient acti

Factors influencing outcome in consultations for chronic pelvic pain.

We aimed to document the demographic and clinical characteristics of women referred by primary care physicians for investigation of chronic pelvic pain to a university hospital gynecology outpatient clinic and to test the hypothesis that specific patient features and the quality of doctor/patient communication at the initial consultation would influence pain outcomes. A clinical questionnaire, visual analog scales for pain, and instruments for hostility and the experience of the consultation were administered at the initial clinic attendance to 105 consecutive women. Follow-up pain scores were

Case Report: 18 F-FDG PET/CT for FUO Due to Whipple Disease Infection.

We present the case of a 60-year-old man admitted to the hospital presenting with high FUO (fever of unknown origin), strong headache, face erythematous-desquamative cutaneous lesions, long history of abdominal pain, and diffuse myalgia. He was also previously treated with immunosuppressants and currently managed with corticosteroids for a seronegative rheumatic disease. Given the immunocompromised state, an infective etiology was suspected. The patient was referred, as part of standard clinical practice, to perform an 18 F-FDG PET/CT to identify potential causes of FUO.

Thoracic aortic dissection presenting with bilateral haemothoraces: a potentially missed diagnosis.

Thoracic aortic dissection is a life-threatening diagnosis, which should not be missed. We present a case in which a patient who presented to the emergency department with chest pain was assessed and referred for admission for treatment of pneumonia, due to misinterpretation of a chest radiograph finding. The patient was re-reviewed and subsequently underwent further investigation, which confirmed aortic dissection. She underwent emergency thoracic endovascular aortic graft repair with stent graft insertion. This case demonstrates haemothorax as an uncommon complication of aortic dissection.

Penile Ulcer due to Leukemia Cutis-A Rare Diagnosis Not to Be Missed.

We present a rare case of leukemia cutis presenting with an ulcer on the ventral penis. A 60-year-old Chinese man was referred for a penile ulcer of 10 days. He was diagnosed with acute myelomonocytic leukemia 6 months earlier and had received 2 cycles of venetoclax with azacitidine. He developed a spontaneous penile blister. It ulcerated after the patient pricked it with a needle. This enlarged after traditional medicated ointment was applied. He denied pain, itch, urethral discharge, systemic symptoms, or rashes elsewhere.

[Gastroduodenal trichobezoar: about a case].

Gastroduodenal trichobezoar ulcer is rare. Its diagnosis is easy in the presence of an evocative context. We report the case of a 21-year old patient followed up for schizophrenia, admitted with acute abdominal pain, vomiting and epigastric mass. Abdominal CT scan suggested the diagnosis of bezoar on the basis of heterogeneous, non-enhanced lesions occupying the entire stomach and appearing to be separated from the gastric wall. Surgical removal of trichobezoar by gastrotomy was performed without complications. The patient was referred to the Department of Psychiatry.

[Genital actinomycosis and pelvic abscesses in a woman with a 13-year-old intrauterine device].

A 55-year-old woman who had had the same intrauterine device (IUD) for 13 years was referred to the gynaecology outpatient clinic due to constitutional symptoms, abdominal pain and vaginal discharge. Diagnostic imaging showed multiple pelvic abscesses, and severe chronic endometritis with Actinomyces was found in an endometrial biopsy. The patient underwent surgical drainage of the accessible abscesses and started long-term antibiotic treatment. This case report illustrates that actinomycosis is an important differential diagnosis in symptomatic women with IUD and suspected gynaecologic malign

Brachial Artery Aneurysm as a Limb Threatening Condition: a Case Report.

Brachial artery aneurysms are rare but potentially limb threatening condition. The presented case here is a 52-year old male referred to the emergency department complaining a sudden onset and progressive pain with coldness of his right upper extremity during brushing. The right upper extremity was pulseless and three-dimensional computed tomography showed an aneurysm of the proximal right brachial artery associated with arterial occlusion in its distal branch. Embolectomy was done, the aneurysm resected, and the artery successfully re-vascularised by interposing a saphenous vein graft.

Marfan syndrome presenting with headache and coincidental ophthalmic artery aneurysm.

A 24-year-old Ugandan woman was referred for a neurology opinion after complaining of a year long history of right-sided retro-orbital stabbing pain. Brain imaging revealed a coincidental 3 mm left ophthalmic artery aneurysm. Marfanoid habitus was noted; after further investigations she was diagnosed with mild aortic root dilatation, subtle lens dislocation and Marfan syndrome. Her symptoms were secondary to temporomandibular joint dysfunction, an under-recognised complication of Marfan syndrome. Her ophthalmic artery aneurysm is likely to be a coincidental finding.
