Posterior fossa partial trigeminal rhizotomy: an alternative to microvascular decompression.

Doctor Walter Dandy was a pioneer in the surgical treatment of trigeminal neuralgia. The Frazier-Spiller operation had been the standard operation for trigeminal neuralgia. Doctor Dandy pioneered the approach through the posterior fossa. He sectioned the lower-most 30 percent of the trigeminal nerve and found that trigeminal neuralgia was relieved. Anesthesia dolorosa did not develop, and the patients had only minimal sensory loss. Dandy's operation was not widely accepted, but microvascular decompression as described by Jannetta was developed by application of Dandy's principles. In twenty pa

Blood pressure and splanchnic nerve activity are reduced by a vagally mediated opioid action.

An enkephalin analogue, D-Ala2-Met5-enkephalinamide (DAME), caused a fall in blood pressure (BP) following right atrial administration (RA) in urethane-anesthetized rats that were also atropinized, paralyzed, and artificially ventilated. This reduction in BP was not related to opioid-induced bradycardia. At a dose of 250 micrograms/kg RA the maximum percentage change was -36.0 +/- 3.9% occurring within 10-15 s subsequent to administration. The fall in BP was blocked by both pretreatment with naloxone HCl (75-100 micrograms/kg i.v.) and bilateral cervical vagotomy. Similar hypotensive responses

Perioperative central retinal artery occlusion after prone posterior cervical spine surgery treated with delayed hyperbaric oxygen therapy: a case report and targeted literature review.

BACKGROUND: Perioperative central retinal artery occlusion (CRAO) is a rare but potentially devastating cause of visual loss after non-ocular surgery. Prone spinal and posterior cervical procedures may increase ocular risk through local orbital compression, impaired venous drainage and reduced ocular perfusion. Management is particularly difficult after surgery, when diagnosis may be delayed, conventional reperfusion strategies may be impractical or relatively contraindicated, and the role of delayed hyperbaric oxygen therapy (HBOT) remains uncertain. CASE PRESENTATION: A 53-year-old man with

Mobility training using a bionic knee orthosis in patients in a post-stroke chronic state: a case series.

INTRODUCTION: An emerging area of neurorehabilitation is the use of robotic devices to enhance the efficiency and effectiveness of lower extremity physical therapy post-stroke. Many of the robotic devices currently available rely on computer-driven locomotive algorithms combined with partial bodyweight-supported treadmill training that drive reflex stepping with minimal patient intention during therapy. In this case series, we examined the effect of task-oriented mobility training in patients in a post-stroke chronic state using a novel, wearable, mobile, intention-based robotic leg orthosis.

Local heating, but not indirect whole body heating, increases human skeletal muscle blood flow.

For decades it was believed that direct and indirect heating (the latter of which elevates blood and core temperatures without directly heating the area being evaluated) increases skin but not skeletal muscle blood flow. Recent results, however, suggest that passive heating of the leg may increase muscle blood flow. Using the technique of positron-emission tomography, the present study tested the hypothesis that both direct and indirect heating increases muscle blood flow. Calf muscle and skin blood flows were evaluated from eight subjects during normothermic baseline, during local heating of

Congenital diaphragmatic hernia in the adult.

INTRODUCTION: Foix-Chavany-Marie syndrome (FCMS) or bilateral opercular syndrome (OPS) is a rare pseudobulbar palsy characterized by facial, lingual, pharyngeal, and masticatory voluntary muscle paralysis resulting in anarthria with preservation of autonomic, involuntary, and reflexive functions. Damage to the posterior part of the inferior frontal gyrus and inferior part of precentral gyrus play a role in the pathogenesis of FCMS. CASE DESCRIPTION: We report a rare case of bilateral OPS following an acute right middle cerebral artery (MCA) infarct in a patient with history of glioblastoma, a

Guillain-Barré syndrome presenting with abdominal pain as the initial manifestation: a case report.

BACKGROUND: Guillain-Barré syndrome (GBS) is an acute immune-mediated polyneuropathy, characteristically presenting with ascending limb weakness and sensory disturbances. However, GBS presenting with abdominal pain as the initial manifestation is relatively rare, abdominal pain is not a characteristic manifestation, its occurrence may delay diagnosis of GBS. CASE PRESENTATION: A 62-year-old male patient presented to our hospital in January 2024 with a 5-day history of abdominal pain in the right upper quadrant. He was initially diagnosed with acute cholecystitis. However, the patient subseque

To Talk or Not to Talk About Sexual Orientation? That Is the Question: Experiences of Sexual Minority Women With Cancer in Brazil and United Kingdom.

This study aims to understand how Brazilian and British sexual minority women who have had cancer make the decision whether or not to disclose their sexual orientation to health professionals. This is a qualitative, interpretative and cross-sectional study. Four Brazilian and three British sexual minority women who were diagnosed with cancer participated in the research. To build the corpus for analysis, individual face-to-face interviews were conducted. The interviews were digitally recorded with the participants' consent, and the audio-recorded content was transcribed verbatim and in full. S

Characterizing sweating problems in multiple sclerosis: insights from QSART, SUDOSCAN, and COMPASS-31: Sweating Problems in Multiple Sclerosis.

OBJECTIVE: To characterize sweating issues in people with multiple sclerosis (pwMS) using the Quantitative Sudomotor Axon Reflex Test (QSART) and SUDOSCAN. METHODS: In 75 pwMS, sweating symptoms were evaluated using the Composite Autonomic Symptom Score (COMPASS-31). Sweating function was assessed with QSART and SUDOSCAN. RESULTS: Symptomatic sweating problems were observed in 22 (29.3 %) pwMS. Pathological results of the QSART (sudomotor index (SI) >0) were identified in 13 (17.3 %) pwMS, while SUDOSCAN results showed pathology in 8 (10.7 %) pwMS. We found a positive correlation between the

Nanoencapsulation of general anaesthetics.

General anaesthetics are routinely used to sedate patients during prolonged surgeries and administered via intravenous injection and/or inhalation. All anaesthetics have short half-lives, hence the need for their continuous administration. This causes several side effects such as pain, vomiting, nausea, bradycardia, and on rare occasions death post-administration. Several clinical trials studied the synergetic effect of a combination of anaesthetic drugs to reduce the drug load. Another solution is to encapsulate anaesthetics in nanoparticles to reduce their dose and side effects as well as ac

Diplopia due to a neurovascular compression.

A 36-year-old female patient presented to our clinic with a two months history of diplopia and dizziness. The symptoms appeared gradually and increased in frequency and intensity. She had no significant medical history and she did not take any medication. A full ophthalmological consult was performed, which revealed restricted ocular motility in the left eye (LE), in left gaze. Otherwise, the examination showed no pathological findings: best corrected visual acuity (BCVA) both eyes (OU) 1 (Snellen chart), normal slit lamp examination and pupillary reflexes, normal intraocular pressure (IOP) an

Stimulation of pulmonary vagal C-fibers by trans-4-methyl-β-nitrostyrene induces bradycardiac and depressor reflex in rats: Role of vanilloid TRPV1 receptors.

Previously, we showed that the synthetic nitroderivative trans-4-methyl-β-nitrostyrene (T4MeN) induced vasorelaxant effects in rat isolated aortic rings. Here, we investigated the mechanisms underlying the cardiovascular effects of T4MeN in normotensive rats. In pentobarbital-anesthetized rats, intravenous (i.v.) injection of T4MeN (0.03-0.5 mg/kg) induced a rapid (onset time of 1-2 s) and dose-dependent bradycardia and hypotension. These cardiovascular responses to T4MeN were abolished by bilateral cervical vagotomy or selective blockade of neural conduction of vagal C-fiber afferents by peri
