Failed extubation in the neonatal intensive care unit.

One of the roles of the pediatric otolaryngologist in the neonatal intensive care unit (NICU) is the assessment and management of the neonate who fails a trial of extubation. This paper reviews the recent 5-year institutional experience at The Hospital for Sick Children, Toronto, with neonates who failed extubation and who subsequently underwent diagnostic endoscopy. One hundred twenty-eight neonates from the NICU underwent diagnostic endoscopy. Of these, 58 neonates underwent diagnostic endoscopy for failure to extubate. Nine neonates were extubated after diagnostic endoscopy and retrial (16%

[Space motion sickness].

This review describes symptoms of space motion sickness (SMS) observed in many cosmonauts during the first day in orbit. It also discusses similar symptoms detected in them upon return to the Earth which constitute the readaptation syndrome. The paper gives a detailed discussion of different theories that explain the etiology and pathogenesis of SMS and the post-flight readaptation syndrome, viz, sensory conflict or sensory mismatch, disorders in the labyrinthine paired function, and blood and cerebrospinal fluid (CSF) circulation. It is emphasized that none of the theories used separately can

Genetic Variants in Potassium Channel Genes and Their Clinical Implications in Kazakhstani Patients with Cardiac Arrhythmias.

Background/Objectives: Cardiac arrhythmias are among the leading causes of sudden cardiac death (SCD). Pathogenic variants in potassium channel genes play a key role in inherited arrhythmia syndromes, yet their contribution in Central Asian populations remains poorly characterized. Methods: We performed targeted next-generation sequencing (NGS) using a 96-gene custom Haloplex panel in 79 Kazakhstani patients with clinically diagnosed arrhythmias, including atrioventricular block, sick sinus syndrome, and atrial fibrillation. Detected variants in potassium channel genes were classified accordin

Accelerated Abdominal MRI: A Review of Current Methods and Applications.

MRI is widely used for the diagnosis and management of various abdominal diseases involving organs such as the liver, pancreas, and kidneys. However, one major limitation of MRI is its relatively slow imaging speed compared to other modalities. In addition, respiratory motion poses a significant challenge in abdominal MRI, often requiring patients to hold their breath multiple times during an exam. This requirement can be particularly challenging for sick, elderly, and pediatric patients, who may have reduced breath-holding capacity. As a result, rapid imaging plays an important role in routin

SpbR controls lipoteichoic acid length by directly inhibiting signal peptidase SpsB in Staphylococcus aureus.

Staphylococcus aureus is a Gram-positive pathogen that causes life-threatening infections. Its cell envelope contains anionic polymers called teichoic acids that are required for cell viability. Teichoic acids come in two forms and are made by different biosynthetic pathways. One form, lipoteichoic acid (LTA), is anchored in the cell membrane; the other form, wall teichoic acid (WTA), is covalently linked to the peptidoglycan cell wall. Although the LTA and WTA biosynthetic pathways have been characterized, regulation of teichoic acid production is not well understood. Here, we identified SpbR

Cross-Sectional Associations Between Depressive and Anxiety Symptoms and Disordered Eating Behaviors by Sex in University Students.

Depressive and anxiety symptoms are associated with a greater likelihood of disordered eating behaviors (DEBs), but the role of sex in these associations is unclear. Objectives: The aim of this study was to analyze the associations between depression, anxiety, and DEBs in a sample of Spanish university students. Methods: Depression was assessed with the Beck Depression Inventory-II (BDI-II), anxiety with the Generalized Anxiety Disorder (GAD-7) instrument, and DEBs with the Sick, Control, One stone, Fat, Food (SCOFF) questionnaire. Statistical analyses included generalized linear regression mo

Responses of Low-Prejudice Viewers to Lesbian Stereotypes in Fiction.

Research on LGBTQ+ media has predominantly focused on high-prejudice contexts and portrayals of gay men, whereas little attention has been paid to how low-prejudice individuals engage with lesbian stereotypes. This study examined responses from 480 Spanish university students (70.1% cis-women) with low-lesbophobia scores, who watched seven video stimuli-six depicting different lesbian stereotypes and one neutral control-in a randomized order. To enhance validity and reduce stimulus-specific bias, each of the seven stereotype categories was represented by two distinct videos (totaling 14 stimul

Utility of point-of-care ultrasound of the liver in the neonatal intensive care unit: experience from a case series.

INTRODUCTION: Point-of-care ultrasound (POCUS) is increasingly being utilized for the management of sick and premature neonates in the neonatal intensive care unit (NICU). While neonatologist-performed cranial, lung, and cardiac ultrasound have become the standard of care in many NICUs across the world, the use of POCUS for the evaluation of the liver remains relatively unexplored. MATERIALS AND METHODS: This case series included neonates admitted to a tertiary-care NICU between January 2021 and April 2025, with radiological liver abnormalities detected on point-of-care ultrasound by treating

Sex Related Differences in Electrocardiography.

Since its implementation into the clinical medicine by Willem Einthoven electrocardiography had become one of crucial diagnostic method in cardiology. In spite of this fact effects of gender differences on parameters of electrocardiographic recordings started to be studied only recently. Sex related differences in physiological ECG are only minimal in childhood but there are developing during adolescence reflecting rapidly evolving differences particularly in hormonal secretion and activity of an autonomic nervous system. The heart rate is approximately 7 % higher in women than in men, PQ and

Potential relationships between COVID-19 and the thyroid gland: an update.

In this review, I aim to provide a complete overview of recent advances in knowledge regarding severe acute respiratory syndrome-coronavirus 2 (SARS-CoV-2)-induced thyroid dysfunction. I discuss the findings regarding the role of SARS-CoV-2 in the development of thyroid dysfunction, including subacute thyroiditis, Graves' disease, non-thyroidal illness, thyrotoxicosis and Hashimoto's thyroiditis during and subsequent to coronavirus disease 2019 (COVID-19). The thyroid gland and the entire hypothalamic-pituitary-thyroid (HPT) axis may represent key targets of SARS-CoV-2. Thyroid dysfunction dur

Role of DVET in severe neonatal sepsis in an era of high antibiotic resistance: a retrospective observational study.

AIM: Sepsis is a significant cause of mortality and morbidity in the NICU despite the potent armamentarium of antibiotics. Recently, there has been a considerable increase in antimicrobial resistance even to high-end 3rd line antibiotics. Thus, there is a need to look into adjunctive therapies. This study aims to study the role of double volume exchange transfusion (DVET) in reducing mortality due to severe sepsis. METHODS: All neonates with severe sepsis admitted in NICU from January 2017 to September 2019 were included in the study. Seven neonates who met inclusion criteria and received DVE

[Hyperglycemia as a risk factor for mortality in critically ill neonates].

BACKGROUND: The sick neonate is susceptible to uncontrolled hyperglycemia by several factors. Our objective was to determine the mortality-predictive role of hyperglycemia in critically ill neonates. METHODS: A cohort study was conducted in neonates admitted during the first hour of life in the intensive care unit. Prenatal and perinatal variables were recorded including ventilatory management, comorbidities, arterial blood gas, blood chemistry and blood count. Serum glucose greater than or equal to 126 mg/dL and greater than or equal to 180 mg/dL was considered consistent with hyperglycemia

Risk factors associated with Mycoplasma agalactiae infection of small ruminants in northern Jordan.

Serological detection of Mycoplasma agalactiae was carried out in 104 small ruminants flocks consisting of 18 sheep, 27 goat and 59 flocks containing both sheep and goats in northern Jordan between 2002 and 2003. At least 5 serum samples per flock were tested using an indirect ELISA for antibodies to M. agalactiae. To increase the chances of detecting this mycoplasma, sick or older animals were sampled. A high seropositivity to M. agalactiae was found in small ruminants suggesting a major role for M. agalactiae in contagious agalactia in northern Jordan. There was no significant difference in

Brachial artery thrombosis in infants: an algorithm for limb salvage.

BACKGROUND: Arterial occlusion in infants, although uncommon, is usually an iatrogenic event associated with arterial vascular access. Most common in the upper limb, the consequences of iatrogenic arterial occlusion may be gangrene and limb loss. Even if there is adequate collateral flow and limb loss is avoided, long-term growth disturbances may be seen. There are few published data to guide the management of arterial occlusion in premature or sick infants. In general, there is agreement regarding the importance of early diagnosis and the reestablishment of limb perfusion with the fewest risk

The validity and reliability of a brief measure of cocaine craving.

INTRODUCTION: Although craving plays an important role in relapse, there are few brief, valid and reliable instruments to measure the desire to use cocaine in routine clinical practice. The 45-item Cocaine Craving Questionnaire-Now (CCQ-Now) is widely used in research, but its length makes its use in everyday clinical work relatively impractical. This study sought to determine the psychometric properties of the CCQ-Brief, a measure composed of 10 items from the CCQ-Now, in treatment-seeking cocaine abusers. METHOD: Subjects with cocaine abuse or dependence (n=247) completed the CCQ-Brief, the

Bacteriology of acute otitis media in a cohort of Finnish children followed for the first two years of life.

BACKGROUND: Timely information on the bacteriology of primary, noncomplicated acute otitis media (AOM) may today be needed more than ever, because of the increasing antimicrobial resistance of its major bacterial causes and because of the potential of new pneumococcal and other bacterial vaccines for prevention of AOM. METHODS: The study followed 329 children from 2 to 24 months of age at scheduled healthy visits and sick visits at the study clinic. Whenever AOM was diagnosed during the follow-up, myringotomy was performed and middle ear fluid was aspirated for bacterial culture. RESULTS: At

Neurogenic switching: a hypothesis for a mechanism for shifting the site of inflammation in allergy and chemical sensitivity.

Neurogenic switching is proposed as a hypothesis for a mechanism by which a stimulus at one site can lead to inflammation at a distant site. Neurogenic inflammation occurs when substance P and other neuropeptides released from sensory neurons produce an inflammatory response, whereas immunogenic inflammation results from the binding of antigen to antibody or leukocyte receptors. There is a crossover mechanism between these two forms of inflammation. Neurogenic switching is proposed to result when a sensory impulse from a site of activation is rerouted via the central nervous system to a distan

[Development of health care services in Bosnia-Herzegovina during the Ottoman empire].

Organized health service in Bosnia and Herzegovina practically started by the treatment of the sick in the Hadji Sinan's Tekke that was founded in 1768. Before that, the civilian population had been mainly treated in their homes, while the army were treated in barracks or hired hans (inns). As late as the nineteenth century, health care was provided by quack doctors, treating physicians or surgeons and barbers, who were trained, beside the circumcision of male children, for tooth extraction, broken bone setting, and sometimes even for performing minor surgical operations (a specially trained b
