BACKGROUND: Loss of the maxilla and midfacial bone buttresses after tumor resections can lead to severe functional and esthetic consequences. The loss of palate function may lead to oro-nasal communication, nasal speech, and oral intake difficulties. Several techniques have been proposed for maxillary defects reconstruction including prosthesis, locoregional flaps, or free flaps. The authors propose the use of anterolateral thigh free flap and iliac crest, or calvaria bone graft association for reconstruction of this kind of defects.
METHODS: Between November 2003 and June 2007, eight patient

BACKGROUND: Assessing a patient's suicide risk is challenging for health professionals because it depends on voluntary disclosure by the patient and often has limited resources. The application of novel machine learning approaches to determine suicide risk has clinical utility.
OBJECTIVE: This study aimed to investigate cross-sectional and longitudinal approaches to assess suicidality based on acoustic voice features of psychiatric patients using artificial intelligence.
METHODS: We collected 348 voice recordings during clinical interviews of 104 patients diagnosed with mood disorders at bas

AIM: Perinatal exposure to glucocorticoids has been associated with adverse cerebral effects, but little is known about their effect on cognitive development and exposure later in childhood. This study examined intellectual abilities, memory and behavioural problems in children previously treated with glucocorticoids.
METHODS: We evaluated 38 children aged from seven to 16 years, who had been treated with glucocorticoids for rheumatic disease or nephrotic syndrome, together with 42 healthy controls matched for age, gender and parental education. The median cumulative dose of prednisolone equi

BACKGROUND: Wide resection of oropharyngeal malignancies implicates the risk of velopharyngeal insufficiency, which can cause nasal regurgitation and hypernasality. A meticulous reconstruction is necessary to avoid impairment and handicap in deglutition and speech. In the classic reconstructive techniques for large oropharyngeal defects, functional outcome only regards deglutition. We also focus on nasality, because hypernasality often occurs as a consequence in this type of reconstruction.
METHODS: In four patients, the surgical defect is closed with a free radial forearm flap sutured to the

Recent research in childhood autism has provided support for the hypothesis that a central cognitive deficit involving severe language impairment underlies this disorder. In this study a group of autistic children were tested for handedness and for lateralization of speech function using a dichotic listening task. Contrary to earlier reports there were no left-handed children in the group although a number showed mixed preference. In the dischotic listening task using pairs of single syllable words the autistic group performed similarly to a matched group of normal children in terms of numbers

Objective: Language and executive functioning are two domains commonly impacted in ALS and should be assessed sensitively and briefly. This paper investigates the utility of the Brief Executive Language Screen (BELS) in ALS. Methods: ALS patients (N = 27) were compared to age, education, and pre-morbid intelligence-matched healthy controls (N = 91) at the group level using ANCOVA and t-tests. A case series was also conducted to explore individual and subgroup performance on the BELS and Edinburgh Cognitive and Behavioral ALS Screen (ECAS). Results: Groups were largely matched on neuropsycholog

Purpose The aim of this study was to determine whether parents of children with developmental language disorder (DLD) were aware of their children's language difficulties and whether a brief, classroom-based language screen can reliably identify children at risk for DLD, including those with both good and poor word reading skills. Method First- and second-grade students ( N = 97) completed a language screen and assessments of nonverbal intelligence, word reading, and language designed for linguistically diverse students. Their parents completed a questionnaire. Results Few parents of children

Fourty eight, XXXX is a rare chromosomal aneuploidy associated with neurocognitive deficits, speech and language disorders and executive dysfunction but the scarcity and variability of reported cases limit our understanding of the 48, XXXX phenotype. To our knowledge, this is the first study to report on the neurodevelopmental profile of three young females with 48, XXXX. Patient 1 (age = 11.0), Patient 2 (age = 10.9), and Patient 3 (age = 6.4) were evaluated using comprehensive neurodevelopmental assessments. Parent questionnaires were completed to assess behavioral and psychosocial domains i

PURPOSE: Covert stuttering is a type of stuttering experience that occurs when a person who stutters conceals his or her stutter from others, attempting to be perceived as a nonstuttering individual. A person who covertly stutters experiences the cognitive and emotional elements of stuttering with minimum overt behavioral symptoms. Individuals who covertly stutter are able to provide insight into their experiences in attempting to be perceived as nonstuttering individuals. Covert stuttering is a topic that continues to be in need of a formal definition. The current investigation is utilizing t

OBJECTIVE: To determine the effect of presence and location of severity labels for different types of visual analog scales (VAS) on overall severity (OS) ratings in dysphonic speech.
STUDY DESIGN: Experimental, between group comparisons.
METHODS: Dysphonic and normal voice samples from male and female speakers were presented to inexperienced listeners for judgments of OS. To rate samples, listeners used an undifferentiated 100-mm VAS labeled at the extremes, a VAS with nonlinearly distributed labels as in the "beta" version of the Consensus Auditory-Perceptual Evaluation of Voice (CAPE-V), o

Speech perception, speech production, and spoken language skills of 13 age-matched groups of prelingually deaf children, all enrolled in the same educational setting, were compared. Each group consisted of three children, two with hearing aids but different degrees of hearing loss (pure-tone average > 100 dB HL and pure-tone average between 90 and 100 dB HL) and one who had used a Nucleus multichannel implant (Cochlear Corp., Englewood, Colo.) for 3 years. The implanted group exhibited significantly better performance in all areas than aided children with greater than 100 dB losses. However, c

Adenoid cystic carcinoma (ACC) of the minor salivary glands most commonly arises in the hard palate and exhibits perineural invasion, indolent growth, and late recurrence, often requiring multimodal therapy. Reconstruction must restore oronasal separation, speech, and deglutition; data regarding the tolerance of palatal island flaps to postoperative radiotherapy and long-term functional outcomes remain limited. A 35-year-old woman with T1N0M0 ACC of the right hard palate (American Joint Committee on Cancer, 8th edition) underwent wide local excision with partial palatal ostectomy followed by r

This study gives detailed information regarding the changes of bulbar dysfunction in a long-term follow-up of four severely brain-damaged young persons. There are similarities in the pathology and symptomatology of the four and all made measurable improvements albeit these did not start for some considerable time following injury. Few gains in bulbar function were noted in the first 18 months. One subject began to improve after 24 months, another after 30 months with the most substantial changes for all subjects being seen by 48 months post-injury. Further small improvements continued to be re
