Extrahepatic biliary obstruction due to post-laparoscopic cholecystectomy biloma.

BACKGROUND: Jaundice presenting after cholecystectomy may be the initial manifestation of a serious surgical misadventure and requires rigor … ommon hepatic duct. RESULTS: Percutaneous catheter drainage of the biloma combined with endoscopic sphincterotomy successfully relieved the extrahepatic biliary obstruction and resolved the intrahepatic ductal leak responsible for the biloma. CONCLUSION: Although heretofore undescribed, postcholecystectomy jaundice due to extrahepatic bile duct obstruction cau …

Endoscopic anal dilatation for fissure-in-ano: a new outpatient treatment modality.

We describe a new therapeutic procedure for fissure-in-ano, endoscopic anal dilatation. Anal dilatation is performed with a two-valved anosc … rmanent, were noted. The results of endoscopic anal dilatation are comparable with those of lateral sphincterotomy, can be achieved at a lower complication rate, with only minor and temporary complications, and performed by physicians with no specific surgical training.

Surgical treatment of symptomatic biliary stones in patients with cystic fibrosis.

Patients with cystic fibrosis have a high incidence of cholelithiasis. However, few studies have addressed the operative therapy for cholelithiasis in this group of patients with poor pulmonary function. We reviewed six patients with cystic fibrosis who were treated for symptomatic biliary stones. Five patients underwent cholecystectomy for chronic cholecystitis. One patient with extremely poor pulmonary status presented with choledocholithiasis and cholangitis, which was successfully treated with endoscopic sphincterotomy followed by ursodeoxycholic acid therapy. Five of these six patients ha…

How do we manage post-OLT redundant bile duct?

AIM: To address endoscopic outcomes of post-Orthotopic liver transplantation (OLT) patients diagnosed with a "redundant bile duct" (RBD). M … oddi dysfunction were excluded. Therapy included single or multiple biliary stents with or without sphincterotomy. The incidence of RBD, the number of ERCP corrective sessions, and the type of endoscopic interventions were recorded. Successful response to endoscopic therapy was defined as resolution of RBD with normalization of associated cholestasis. Laborator …

Colitis-Induced Pancreatic Fistula with Pancreatic Ascites.

INTRODUCTION: Although cases of severe pancreatitis causing fistula formation into the colon have been documented, the reverse process of colitis causing a pancreatic fistula remains undocumented. CASE PRESENTATION: We present the case of a 79-year-old male with severe colitis resulting in perforation and pericolonic abscess formation adjacent to the pancreas, which resulted in an internal pancreatic fistula and pancreatic ascites. After 2 paracenteses, our patient ultimately underwent endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy and pancreatic duct stent placemen…

Grill Bristle Induced Perforation of the Bile and Pancreatic Ducts Managed by Endoscopic Ultrasound and Endoscopic Retrograde Cholangiopancreatography: A Unique Foreign Body Case.

Foreign body ingestion is common, but penetration into the common bile duct (CBD) and pancreas is exceptionally rare. A 74-year-old woman presented with 4 weeks of intermittent left upper abdominal pain and normal laboratory results. Computed tomography showed a linear hypodensity traversing the ampulla and pancreatic duct. Endoscopic ultrasound confirmed a hyperechoic linear object in the CBD. Endoscopic retrograde cholangiopancreatography with sphincterotomy and balloon sweeps revealed a metallic foreign body protruding from the papilla. It was removed using pediatric cold forceps and identi…

Difficult Biliary Stones: A Comprehensive Review of New and Old Lithotripsy Techniques.

Biliary stones represent the most common indication for therapeutic endoscopic retrograde cholangiopancreatography. Many cases are successfully managed with biliary sphincterotomy and stone extraction with balloon or basket catheters. However, more complex conditions secondary to the specific features of stones, the biliary tract, or patient's needs could make the stone extraction with the standard techniques difficult. Traditionally, mechanical lithotripsy with baskets has been reported as a safe and effective technique to achieve stone clearance. More recently, the increasing use of endoscop…

Endoscopic fibrin sealant closure of duodenal perforation after endoscopic retrograde cholangiopancreatography.

Traditionally, perivaterian duodenal perforation can be managed conservatively or surgically. If a large volume of leakage results in fluid collection in the retroperitoneum, surgery may be necessary. Our case met the surgical indication for perivaterian duodenal perforation after endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy and endoscopic papillary balloon dilatation. The patient developed a retroperitoneal abscess after the procedures, and a perivaterian perforation was suggested on computed tomography (CT). CT-guided abscess drainage was performed immediately. W…

Role of endoscopic retrograde cholangiopancreatography in laparoscopic cholecystectomy.

Endoscopic retrograde cholangiopancreatography was performed before laparoscopic cholecystectomy in 312 patients, with successful results in 287 patients (92.0%). Common bile duct stones were diagnosed in 26 of the 287 patients (9.1%). In eight of these patients, the diagnosis had not been suspected from the clinical or laboratory data or results of ultrasonography. Endoscopic sphincterotomy with stone removal was successful in all 23 patients who underwent this procedure. Anatomical variations were detected in 7 patients (2.4%), including origin of the cystic duct from the right hepatic duct …

[Endoscopic treatment of bile leakage following biliary tract surgery].

Thirty-two patients aged 15-89 years developed postoperative bile leakage. Twenty-eight had undergone cholecystectomy, with choledocholithotomy in 11, and four had had miscellaneous operations. Endoscopic retrograde cholangiopancreatography (ERCP) showed leakage from the cystic stump in 19 cases, from the T-tube track in five, from gallbladder and liver abscess in two and from major bile ducts in 6 cases. Major bile duct lesions were only mapped, but not managed endoscopically, whereas the remaining 26 patients were treated successfully with internal stenting (22) or endoscopic sphincterotomy …

Endoscopic ultrasound-guided resolution of right hydronephrosis secondary to local compression by an inflammatory pancreatic mass.

We present the case of 67 years-old man with unremarkable medical history. He was admitted to our department for abdominal pain compatible with choledocholithiasis associated with acute cholecystitis. ERCP was performed but direct papillary canulation attempts failed by means of conventional sphincterotome. So, pre- cut papillotomy was successfully attempted getting free access to distal choledochus and retrieving a small lithiasis. Unfortunately, the patient developed severe post-ERCP acute pancreatitis.

Early double-guidewire versus repeated single-guidewire technique to facilitate selective bile duct cannulation: a randomized controlled trial.

BACKGROUND: During endoscopic retrograde cholangiopancreatography (ERCP), access to the common bile duct (CBD) can be problematic after unintentional insertion of the guidewire into the pancreatic duct. We conducted a prospective, randomized study in order to compare biliary cannulation success rates of early double-guidewire (EDG) and repeated single-guidewire (RSG) techniques in patients with inadvertent passage of the guidewire into the pancreatic duct. METHODS: Patients with a native papilla were randomly assigned to either the EDG or RSG groups after unintentional insertion of the guidew

Double plastic stent implantation for recurrent acute pancreatitis with incomplete pancreas divisum: a case report and literature review.

Pancreas divisum (PD) is a common pancreatic malformation caused by the failure of fusion between ventral and dorsal pancreatic ducts. There is a small branch of communication between the two systems in incomplete PD, and this variation has an incidence of 15%. A 43-year-old female patient presented to our department with recurrent abdominal pain. Magnetic resonance cholangiopancreatography (MRCP) showed that the ventral pancreatic duct was curved, with a local pouchlike dilatation. Endoscopic ultrasonography supported the diagnosis of incomplete PD and showed a thin branch of communication be
