Sprains are the most frequent injuries of the ankle, especially in sports. Up to 85% of cases affect the lateral ligament complex. Multi-ligament injuries with associated lesions of the external complex, deltoid, syndesmosis, and sinus tarsi ligaments are also common. Most ankle sprains respond to conservative treatment. However, up to 20 to 30% of patients can develop chronic ankle pain and instability.New concepts have been recently developed, based on arthroscopic advances, such as microinstability and rotatory ankle instability. These entities could be precursors of mechanical ankle instab

There are specific injuries that are common in golf and racquet sports. These abnormalities have a predilection for specific structures as well and can be divided into two categories on the basis of etiology as either chronic repetitive injury or acute trauma. With golf injuries, upper extremity abnormalities prevail and include rotator cuff disease, epicondylitis, wrist tenosynovitis, and hamate hook fracture. Thoracolumbar spine pain can also occur. The order of frequency of these ailments is different for professional and recreational athletes. With racquet injuries, as in tennis, lower ext

Disorders of the hip abductor mechanism are a frequent cause of lateral hip pain and functional impairment. This article focuses on MRI evaluation of the gluteus medius and minimus tendons, emphasizing the relationship between tendon pathology, muscle quality, and biomechanics. Key imaging features of tendinopathy, partial and full-thickness tears, tendon elongation, and fatty infiltration are discussed, together with their clinical and surgical implications. The value of MRI in both preoperative assessment and postoperative follow-up is highlighted, particularly in distinguishing adaptive cha

CASE: A 36-year-old soldier presented for foreign training fitness with a history of a fall 18 years previously, that injured his nondominant left wrist. Initially, the injury was managed conservatively with a below elbow cast. Since then, he had been unable to extend the thumb at the interphalangeal joint but adapted well to daily activities. Surgical reconstruction using an extensor indicis proprius (EIP) to extensor pollicis longus (EPL) tendon transfer yielded favorable recovery.
CONCLUSION: This case highlights neglected EPL injury over 18 years and confirms that EIP transfer, when preci

We describe our experience of managing extensor hood injuries in boxers (57 fingers). The diagnosis was mostly clinical, with imaging only if the diagnosis was equivocal. The middle (61%) and index (26%) digits were most frequently injured. On exploration, 26% had no hood tear, however all required tenolysis from the adherent capsule. Of 42 hood tears, 15 were central splits between adjacent extensor tendons in the index or little fingers,15 tears were on the ulna side of the extensor tendon and 12 tears were on the radial side. A pseudobursa was encountered in 35%, capsular tears in 28% and c

CASE: We present 2 cases of spontaneous index finger (IF) flexor tendon ruptures because of previously undiagnosed osteonecrosis (OA) of the capitate. Imaging revealed fragmentation of the capitate, and patients were treated with excision or fixation of the bony fragments with tendon repair. At the final follow-up, both patients had functional digital range of motion.
CONCLUSIONS: OA of the capitate is relatively rare and may present as atraumatic (IF) flexor tendon ruptures because of attritional wear when associated with bony fragmentation.

PURPOSE: No therapeutic consensus has been established about proximal ruptures of the rectus femoris muscle. The objective of this literature review is to determine a therapeutic course of action.
METHODS: We conducted a literature review on the PubMed database using the following keywords (in French and English, respectively): "quadriceps/quadriceps", "droit antérieur/rectus femoris", "proximal/proximal", "chirurgie/surgical", "avulsion/avulsion". We collected 266 articles, 36 of them were selected, which were related to our topic: proximal rupture of the anterior rectus femoris. Patients wi

Different ultrasound approaches have been proposed to evaluate the distal biceps brachii tendon, such as lateral, posterior, medial, and anterior, all allowing good visualization of the distal biceps tendon and its pathologic appearances. Here we describe a new method of ultrasound evaluation of the distal biceps tendon insertion with the patient in the so-called crab position. The crab position is used to evaluate the posterior compartment of the elbow but also the lateral and medial compartments. This position allows for coverage of 75% the elbow in a single position, with less discomfort fo

The Achilles tendon is the strongest tendon in the human body. Rupture of the Achilles tendon prevents the transmission of forces from the calf muscles to the foot and thus the push-off phase of walking and running is no longer possible. The diagnostics of Achilles tendon rupture are typically based on the symptoms and clinical examination. A palpable gap and the Thompson test are sometimes difficult to evaluate in situations with a large hematoma. Benchmarking for a definitive rupture is the inability to perform a one leg heel raise. Sonography is used to determine the localization of the Ach

The focus of this article is the diagnosis and treatment of peroneal tendon tears. The article first describes mechanisms of injuries resulting in peroneal brevis and longus tears. Associated pathologies, such as ankle instability, hindfoot varus, hypertrophied peroneal tubercle, are discussed. Following sections on diagnosis and conservative treatment, the article describes operative treatment for isolated peroneus brevis tear, isolated peroneus longus tear, and tears of both the peroneus longus and brevis. The authors also discuss hamstring allograft reconstruction, the silicone rod techniqu

We report a case of the flexor digitorum profundus tendon rupture of the little finger, which was predisposed by an anatomic variation of the tendon. Intraoperative findings and magnetic resonance imaging of the opposite hand suggested that the flexor digitorum profundus tendons of the ring and the little finger bifurcated. The patient had tendon reconstruction and regained function. We believe that reconstructing the tendon so that it resembles the normal anatomy prevents the recurrence of tendon rupture.

Achilles tendon ruptures are common, and their incidence is increasing. The evidence for best management is controversial, and, in selected patients, conservative management and early mobilization achieves excellent results. Surgery is associated with an increased risk of superficial skin breakdown; however, modern techniques of percutaneous repair that are performed under local anesthesia and followed by early functional rehabilitation are becoming increasingly common, and should be considered when managing such patients.

A typical rupture of Achilles tendon occurs as a complete rupture about 4-6 cm proximal to its insertion to the calcaneus. The morphology of the tear is most often random without any distinct recognizable pattern. The authors describe four cases of atypical "Z" rupture of the Achilles tendon. In two, the orientation of the "Z" was in the coronal plane and other two in the sagittal plane. All four cases were treated by open repair.

Immediate mobilization of flexor tendons repairs creates a conflict between muscle strength and suture solidity. Non removable sutures are brittle and require to prevent extension by means of either a cast in flexion of the wrist and of the metacarpo-phalangeal joints or a rubber band traction. Pull-out sutures do not permit early mobilization because of their anchorages. The authors present a new technique they have been using for the last five years. It consists in a simple solid slip-knot which tightening the tendon when the muscle pulls. This knot is fixed at the distal bone insertion of t
