Microvascular digit transposition following a two-digit amputation in an infant.

Multiple digit amputations in children demand special consideration to make subsequent hand function optimal. Replanted digits in children have a comparatively lower viability rate, but those that do survive usually go on to excellent function. In this case, an amputated thumb was severely mangled and not suitable for replantation. An amputated index finger was transposed to the thumb position. A six-month postoperative follow-up of the transposed digit confirmed satisfactory joint motion, restored sensibility, and unimpaired digit growth. The 11-month-old infant described is the youngest pati

Surgical treatment of the rheumatoid wrist and aspects of the natural course when untreated.

Based on 20 years of experience in surgery of the rheumatoid hand, the author has presented diagnostic and surgical problems of tendons, ligaments and bones round the rheumatoid wrist. The results of long-term follow-up studies after wrist synovectomy have been given and also aspects of the natural course of the untreated rheumatoid wrist. The author's technique of wrist arthrodesis has been described in detail.

Hemiarthroplasty for the rotator cuff-deficient shoulder. Surgical technique.

BACKGROUND: Hemiarthroplasty is a common treatment for cuff tear arthropathy and glenohumeral arthritis associated with a massive tear of the rotator cuff; however, to our knowledge, long-term outcomes and preoperative factors affecting results have not been reported. METHODS: Thirty-four shoulders in thirty-one patients with cuff tear arthropathy or a massive rotator cuff tear with glenohumeral arthritis underwent hemiarthroplasty at an average age of seventy-two years. Outcomes in all patients were evaluated by direct physical examination and according to the limited goals criteria of Neer

Isokinetic evaluation of anterior cruciate ligament reconstruction: quadriceps tendon versus patellar tendon.

AIM: The goal of this prospective randomized study was to compare the isokinetic recovery of thigh strength after anterior cruciate ligament (ACL) reconstruction by using patellar or quadriceps tendon as graft at the 6th month follow-up. METHODS: The authors evaluated 48 patients who underwent arthroscopic ACL reconstruction using patellar tendon (PT group) or quadriceps tendon (QT group) as autograft after a 6 months follow-up undergoing the following tests: the Ergojump Bosco System springboard and Universal's FITNET computerized isokinetic system. RESULTS: The counter movement jump (CMJ)

[Synovial healing in reconstructed cruciate ligaments. Our personal experience compared in single interventions and combined reconstructions].

INTRODUCTION: The healing process of tendon grafts used in cruciate ligament reconstruction is called ligamentization. The tendon structural architecture changes progressively into the histological appearance of normal ligament. Amiel and Clancy have demonstrated that this process is time-dependent in anterior cruciate ligament (ACL) reconstruction, the tendon graft taking one year to become similar to a normal cruciate ligament. Three different maturation phases of anterior cruciate graft ligamentization can be seen at MRI: periligamentous proliferation, intraligamentous proliferation and def

Long term follow-up of medial column fusion and tibialis anterior transposition for adolescent flatfoot deformity.

We report the results of three patients (four feet) who had surgical correction of adolescent flatfeet performed over fifty years ago. The surgery involved medial column stabilization with fusion procedures and tibialis anterior transposition into the navicular (Young's tenosuspension procedure). In this small sample, we found a high rate of painful arthrosis that developed over time in the contiguous joints of the foot.

Contralateral Lower Trapezius to Triceps Transfer for Restoring Elbow Extension in Patients with Brachial Plexus Palsy - A Technical Note.

Restoration of elbow extension is seldom considered in the reconstructive plan for patients with extensive brachial plexus injury because of the scarcity of the nerve and tendon transfer options. However, restoring elbow extension could increase the outreach of hand and give better control of the elbow flexion. We propose a new technique for restoring elbow extension using the readily available contralateral lower trapezius (CLT). By extending the CLT with a fascia lata graft and transferring it to the triceps, we were able to restore elbow extension. In all four patients who underwent this pr…

Triceps to biceps transfer for restoration of elbow flexion following upper brachial plexus injury.

INTRODUCTION: Upper brachial plexus injury in adults causes loss of elbow fl exion; when the primary nerve surgery has failed or the patient seeks treatment after 12 months of injury and pedicled muscle transfers are required. Most commonly, the latissimus dorsi or the Steindler flexorplasty is used. MATERIAL AND METHODS: We have transferred one of the heads of triceps muscle to restore the elbow flexion in such cases. In addition to return of elbow flexion, extension of elbow following surgery is retained. Ten patients suffering from upper brachial plexus injuries underwent transfer of one h

Successful Treatment of War Zone Traumatic Lower Extremity Wound With Exposed Tendons Using an Autologous Homologous Skin Construct.

Extremity injuries are common in contemporary combat and have become more prevalent as fatality rates have dropped to historic lows. Traumatic extremity wounds, especially those sustained in theater, often present with exposed structures such as tendon, bone, and joint, preventing the use of split-thickness skin grafts (STSG) for coverage. Traditional reconstructive options for these complex wounds include skin substitute with delayed STSG, local flaps, debridement of tendons, pedicled distant flaps (such as cross-leg flap), free tissue transfer, and amputation. STSG, whether on top of skin su

Modification of the alignment between the tibial tubercle and the trochlear groove induced by temporary hemiepiphysiodesis for lower extremity angular deformities: a trigonometric analysis.

UNLABELLED: This study aimed to predict the modification of the alignment between the tibial tubercle (TT) and the trochlear groove (TG) that occurs during femoral or tibial hemiepiphysiodesis. MRI scans of 541 knees were retrospectively reviewed to determine the distances between the cranial insertion of the patellar tendon on the TT and the femoral physis (FP)/tibial physis (TP). Thereafter, we developed a trigonometric formula to calculate the predicted change of the TT-TG distance that occurs during hemiepiphysiodesis around the knee using both the planned angular correction as well as the

Use of the free innervated dorsalis pedis tendocutaneous flap in composite hand reconstruction.

We used the free dorsalis pedis flap including the extensor digitorum longus or the extensor hallucis brevis, and/or the superficial peroneal nerve to reconstruct composite loss of skin and tendons on the dorsum of the hand. Between February 1992 and February 1996 we treated 7 patients with composite tissue loss on the dorsal hand caused by trauma or burn. Six men and 1 woman had an average age of 26 years (range, 19-42 years). Flap size ranged from 3 x 4 cm to 9.5 x 9 cm. The follow-up period ranged from 10 to 44 months. At 1 week postoperatively, active flexion and passive extension commence

Reconstruction of traumatic absence of the thumb in the adult by pollicization.

Amputation of the thumb is a severe handicap. In an emergency situation, thumb amputation must be treated by means of reimplantation when possible. If reimplantation cannot be performed or fails, several methods of thumb reconstruction can be used according to various factors. These include the number of surviving fingers and the level of the thumb amputation. Pollicization is the first choice for amputations proximal to the metacarpophalangeal joint when four and even three fingers are present. It is the easiest and safest operation that supplies the best results both from the motor and senso
