OBJECTIVE: Previous descriptions of MR imaging of carpal tunnel syndrome used limited study populations and volunteers as controls. We reeva … lly increased size, flattening of the median nerve, increased median nerve signal intensity, flexor tenosynovitis, retinacular bowing, decreased deep tendon fat, and deep palmar bursitis were retrospectively and independently evaluated by two observers who were unaware of patient diagnosis.
RESULTS: None of the previously described signs was sensitive for the …

BACKGROUND: The ankle and foot joints are frequently affected by rheumatic and musculoskeletal disorders (RMDs) at all ages, causing pain and limited movement. Ultrasound (US) is an effective tool for assessing soft tissue and monitoring patients with inflammatory and degenerative RMDs.
OBJECTIVE: To examine the frequency and classifications of soft tissue lesions, including tenosynovitis, tendon injuries, and ligament damage, as well as synovitis in the ankle and foot, clinically and with ultrasound evaluation in patients presenting associated symptoms.
METHODS: A cross-sectional study at t…

Background: Flexor sheath infections require prompt diagnosis, and management with intravenous antibiotics and/or surgical washout followed by hand therapy. Complication rates as high as 38% have been reported. Our unit takes a relatively conservative approach to the management of flexor sheath infections and select patients are managed non-surgically via our outpatient antibiotic service where they are clinically reviewed and receive a once daily dose of intravenous antibiotics. The aim of this study is to determine if outpatient management of flexor sheath infections was associated with an i

PURPOSE: Finkelstein's test is the classic diagnostic test for de Quervain's disease. Finkelstein hypothesized that the entry of the muscle bellies of the extensor pollicis brevis (EPB) and abductor pollicis longus (APL) tendons into the first extensor compartment was responsible for the findings observed in his now eponymous test. We agree with Finkelstein's hypothesis and further hypothesize that this position would induce measurable bulk (muscle mass within the retinaculum) and tethering (stretching of synovial tissue) effects within the compartment. To test this latter hypothesis we measur

Twenty nine patients with 31 trigger digits were treated by percutaneous release under local anaesthesia using an 18 gauge needle. One patient was lost to follow up, and the remainder were examined at a mean follow-up of 14 months. One patient (one thumb) experienced recurrent symptoms, and required an open release. The remaining 27 patients with 29 trigger digits had complete relief of their symptoms.

Of 1310 patients surgically treated for carpal tunnel syndrome (CTS), 134 (10.2%) were observed to have one or more trigger fingers or de Quervain's disease. Of a total of 205 fingers, the middle finger was affected 75 times, the ring finger 50 times, the thumb 47 times, the small finger 17 times, and the index finger 16 times. Chronic nonspecific synovitis is commonly suspected as the cause of both diseases. In all cases of carpal tunnel syndrome, the patient should therefore be examined for symptoms of concomitant trigger finger and vice versa. Both can be cured surgically under local anesth

This two-part study evaluates the efficacy of functional distal interphalangeal joint (DIP) splinting for the treatment of trigger finger. Thirty-one fingers from 21 meat packing plant workers were treated with DIP splinting. A single corticosteroid injection was offered if triggering was stage 4 or greater. All workers returned to work immediately. Eighty-one percent of the digits were treated successfully (mean follow-up: 1 year). Treatment failure correlated with duration of symptoms and stage of triggering but did not correlate with age, race, sex, disease in multiple digits, or prior trea

Extraarticular wrist syndromes are relatively common causes of underperformance or nonparticipation in sports. Many of these conditions may be caused by nonathletic activities. The spectrum of pathologic signs ranges from tendon inflammation, stenosis, and subluxation to neuropathies, vascular conditions, congenital anomalies, cysts, calcarea, and callosities. Appropriate treatment requires an accurate diagnosis and specific measures directed toward the condition. Moreover, many of these problems may be preventable by modification of sports equipment, proper conditioning, alteration of athleti

In our 3 years of clinical experience, we have found that diagnostic ultrasound provides the veterinarian with a valuable diagnostic tool. It allows the clinician to quantify morphologic change that has occurred as a result of soft-tissue injuries, even when the clinical findings are ambiguous or insufficient. In cases in which aggressive postinjury therapy has been instituted prior to presentation, diagnostic ultrasound is often the only noninvasive method that can ascertain the extent of the horse's injury. Diagnostic ultrasound provides the technology to detect injuries before they become p

There is a group of painful inflammatory conditions that involve the soft tissues of the hand, wrist, and forearm. These generally occur without underlying systemic diseases, and the pain frequently results from tendon or nerve compression. Recognition of these difficulties can lead to appropriate treatment, often nonoperative, resulting in symptomatic cure. Occasionally, surgical treatment is required.

At present, the term "thoracic outlet syndrome" (TOS) is used as a term to include all factors compressing the nerves and vessels situated in the outlet of the thorax and the costo-clavicular area. It is marked by neurological and vascular disorders; their manifestation can be either spontaneous or posttraumatic. In case of surgical treatment the transaxillary approach (according to Roos) proved to be the best, and is indicated in therapy-resistant TOS, in certain cases of arterial complications and in outlet obstructions of the subclavian vein. Nerve injuries in the forearm and hand are common, especially with concomitant tendon lacerations. The optimal time for repair of a clean sharp laceration is within the first 24 to 48 hours using magnification to achieve precise surgical reapproximation. Transfer to a facility equipped to perform replantation is indicated for multiple digit amputations, any thumb amputation, transmetacarpal amputation, wrist and arm amputation, and major amputations in children. Acute paronychial infections are treated with warm soaks, antibiotics, elevation, and immobilization. Surgical drainage is often indi

Wrist pain is common and can be attributed to tendon pathologies. This review aims to give a structured review of tendon anatomy, discuss anatomical pitfalls, and provide an overview of typical tendon pathologies of the wrist based on the current literature.Typical tendon pathologies of the wrist include de Quervain tenosynovitis, proximal and distal intersection syndrome, extensor and flexor pollicis longus tendon ruptures, and extensor carpi ulnaris subsheath injury. Typical pitfalls are multiple bundles of the abductor pollicis longus tendon or the centrally increased signal of the extensor …

Psoriatic arthritis is an inflammatory arthritis that affects around 30% of patients with psoriasis. The disease spectrum includes peripheral arthritis, enthesitis, tenosynovitis, dactylitis, axial involvement, and skin and nail psoriasis in most patients. In addition to the cutaneous and musculoskeletal manifestations, several comorbidities can complicate the disease course, including cardiovascular disease, diabetes mellitus, metabolic syndrome, gout, anxiety, and depression. The management of patients with psoriatic arthritis begins with a careful assessment of the skin and joints and scree…
