Twenty-five whole-organ recipients treated from 1981 through September 1988 were HIV carriers. Eleven were infected before transplantation, although this was not known until later in 8 recipients. The other 14 were infected perioperatively. Ten of the 25 recipients were infants or children. The organs transplanted were the liver (n = 15), and the heart or kidney (n = 5, each). After a mean follow-up of 2.75 years (range, 0.7-6.6 years), 13 recipients are alive. Survival is 7/15, 2/5, and 4/5 of the liver, heart, and kidney recipients, respectively. The best results were in the pediatric group

Recurrent glomerulonephritis and de novo glomerulonephritis may develop in the graft after renal transplantation. Among 59 patients with a pathological diagnosis of glomerulonephritis as original renal disease, 12 (20.3%) showed recurrence of the original lesions in the graft. Two patients with hereditary nephritis developed anti-GBM disease (one patients in two grafts). The disease rapidly progressed to graft loss. A de novo membranous nephropathy was diagnosed in four patients whose original renal disease was not a glomerulonephritis. One patient had been treated with antilymphocyte globulin

The evaluation of nutrition in patients who are candidates for liver transplants often indicates the presence of a state of malnutrition with regard to calories and proteins, which may lead to posterior complications. The transplant reverts the metabolic alterations in cases of good evolution, and there is evidence of a correct hepatic treatment of amino acids in the studies performed. Thus, this group of patients may be treated with a similar nutritional support to that indicated in other types of critical patients. On the other hand, in patients with an unfavourable evolution of the graft, t…

Heart transplantation in children is being performed with increasing frequency. As experience has accrued, problems of rejection, graft atherosclerosis, and growth have been noted. Seventeen children (seven boys and 10 girls) between the ages of 5 months and 14 years have undergone heart transplantation since 1981. The preoperative diagnosis was cardiomyopathy in 13 children, congenital heart disease in two, and endocardial fibroelastosis in two. Immunosuppressive therapy has included a tapering schedule of cyclosporine, azathioprine, and prednisone. There are 13 children alive, with four hosp

In nine patients between the ages of 3 months and 18 years with endstage heart failure, orthotopic heart transplantation was performed. Seven of these patients suffered from dilative cardiomyopathy. Additional diagnoses were tricuspid atresia in one case, and hypoplastic left-heart syndrome in another case. Seven of these children (77.7%) were catecholamine-dependent before the operation. The postoperative immunosuppressive treatment consisted of a combination therapy of cyclosporine A, azathioprine, and prednisolone, similar to the treatment in adults. Diagnosis of rejection was based on both

Heart transplantation is becoming an accepted treatment for children with irreversible and profoundly disabling cardiomyopathy. The risk is much higher when there is underlying congenital heart disease, and even moderately elevated pulmonary vascular resistance is a contraindication to orthotopic heart transplantation. Heterotopic or heart-lung transplantation may be considered in patients with elevated pulmonary vascular resistance. In a few centers, heart transplantation is being performed as an alternative to palliative surgical procedures in children with hypoplastic left heart syndrome. C

The neuropathology of 18 cardiac transplant recipients was reviewed with the clinical findings. Pathological changes were noted in the central nervous system (CNS) in 94% of the patients, the most frequent being cerebral vascular in origin (72%). Eight patients (44%) had multiple cerebral infarcts and morphologically, a large number of these antedated the transplantation. In addition 4 patients had acute focal ischemic changes which occurred after transplantation. Intracranial hemorrhage was noted in 5 patients (28%), including one case of fatal intracerebral hemorrhage following an acute hype

A long period of experimental work has allowed to study successively hypothermic preservation of the small bowel, autotransplantation and finally allotransplantation. After this initial phase of experimental surgery, a certain number of intestinal transplantations in humans could be attempted as cyclosporine became available. Following a brief of report of 3 intestinal transplantations recently performed by the group of the hôpital des Enfants-Malades in Paris, indications for intestinal transplantation are provided. Subtotal resections of the small bowel in children or adolescents and extensi

The informations available for Europe show that, on December 31, 1986, 50% of the 1,616 registered children under 15 years of age had a functional renal graft. Only 5% of these children had been transplanted without prior dialysis, which is usually possible only with a graft taken from a living donor. With brain-dead donors, generally adults, numerous patients may remain on the waiting list after several years of dialysis. The preparation of recipients, the juridical aspects of such transplantations in children, the principles of selection of recipients for corpse kidney grafts are reported. T

To summarize this review: it was written concerning human cell transplantations from various human organs based on already published historical reports. It was limited to only clinical cases for therapeutic treatment. Some of the methods in the article are commonly used, well-established methods found in everyday practice. Others are still in the experimental stages. I think we can try these, or even never methods for other cell groups in the near future.

Selected patients with terminal lung disease have been managed effectively by lung transplantation. Strict selection criteria for donors and recipients, attention to technical detail, and avoidance of perioperative corticosteroids increase the likelihood of success. The underlying pulmonary disease determines the appropriate procedure. Single-lung transplantation is most appropriate for patients with pulmonary fibrosis. Patients with emphysema or septic pulmonary disease who have adequate or recoverable cardiac function can be well served by double-lung transplantation. However, such patients

The application of the principles underlying successful renal transplantation in children has led us to increasing success in achieving our goals, which involve more than simple renal function replacement and include effective growth in stature, skeletal and central nervous system development, as well as social development. To succeed in these diverse areas, all modalities of therapy for ESRD may be needed at various times in some patients. Pretransplant therapy must be judiciously selected, and post-transplant protocols often modified to reflect individualized needs.

Cyclosporine is a new immunosuppressive drug that acts early in the exposure of a host to allogeneic stimulation. It is a peptide of fungal origin. It has selective action on T cells, leaving the other cells of the immune system intact. It acts by preventing the function of the early activation signals of T cells, such as the acquisition of receptors for Il 2 and Il 1. It is lipophilic, moderately well absorbed by the gut, and metabolized by the liver. Factors affecting absorption or hepatic metabolism alter the amount of cyclosporine available in the circulation. Circulating levels can be mea

Liver transplantation has become a clinical therapeutic modality for end stage liver diseases. The results achieved in children are better than in adults: in T.E. Starzl unique experience in Pittsburgh, USA, the survival rate at one and four years are 75 and 70% respectively. Complete rehabilitation of these children can nowadays be expected. Between March 1984 and June 1985, 8 children received an orthotopic liver transplantation at the University of Louvain Medical School in Brussels, Belgium; one child received two transplantations after acute and irreversible rejection of a first ABO incom

Continuous ambulatory peritoneal dialysis (CAPD) is believed to improve the immune competence of end-stage renal failure patients and to increase the risk of graft rejection following subsequent renal transplantation. At this centre, 220 consecutive renal transplants have been studied in patients treated by either CAPD or haemodialysis (HD). Patient and graft survival was not significantly different for the two treatment groups over a five year follow-up. When only first cadaver recipients were considered (152 grafts) one-year graft survival (non-immunological failures excluded) was 77 per cen…

The authors report four cases of pregnancy in a series of 174 women who had undergone renal transplantation. In one case the pregnancy was complicated by a pre-eclamptic attack and premature delivery. A review of the literature shows that the risks to the mother and the kidney are marginal. The risk to the child, however, is more serious. Pregnancy in transplant patients is therefore a high-risk pregnancy requiring advance precautions and careful surveillance.

From January 1, 1979 through October 31, 1984, 58 heart transplantations have been performed in 57 patients. Conventional immunosuppression was utilized in the first 32 patients. Since January 1, 1983 cyclosporine and a low dose of prednisone have been the immunosuppressive agents of choice. The annual survival rates were 75% in 1979, 67% in 1980, 75% in 1981, 57% in 1982 and 60% in 1983. Patients whose age ranged from 50 to 57 years had similar survival to the younger recipients. Early deaths resulted from infection in 3 patients, acute rejection in 4, acute failure of the donor heart in 2 an

Within the spectrum of presently accepted candidates for heart transplantation, end-stage heart failure in dilated cardiomyopathy has become the principle indication. Although several indicators of poor prognosis have been specified, the decision for heart transplantation is primarily made on clinical grounds. Expected long-term survival after transplantation is 60 to 80% at one year and more than 50% at five years. Since July, 1983, 50 patients underwent orthotopic heart transplantation, 38 of whom had been suffering from dilated cardiomyopathy. Ages ranged from nine to 54 years with a mean o

The results of renal transplantation for end-stage diabetic nephropathy in 17 patients--11 receiving cadaver (CD) grafts and 6 related living donor (RLD) grafts--are reported. The transplants were rejected in 5 cases, in 4 acutely, and these patients were returned to haemodialysis; 3 of them subsequently died. One patient died of heart failure, but the graft was still functioning. The remaining 11 patients enjoy good renal function. The outcome was superior to results on dialysis, particularly for RLD grafts, and was comparable to results of transplantation for non-diabetic renal failure. Visu…
