Managing spondylitis tuberculosis in a patient with underlying diabetes and hypothyroidism: A case report.

BACKGROUND: Tuberculosis (TB) remains one of the highest Asia's health problems. Spondylitis TB in diabetes mellitus (DM) and hypothyroidism patients is a rare case of extrapulmonary tuberculosis. However, there is a lack of therapeutic guidelines to treat spondylitis TB, particularly with type 2 DM (T2DM) and hypothyroidism as comorbidities. Here we present a case of spondylitis TB with T2DM and hypothyroidism in a relatively young patient and its therapeutic procedure. CASE SUMMARY: We report the case of a 35-year-old male patient from Surabaya, Indonesia. Based on anamnesis, physical exami

A Rare Case of Mycobacterium Monacense Osteomyelitis: A Case Report.

Mycobacterium monacense (M. monacense) is a yellow-pigmented, rapidly growing non-tuberculous mycobacterium (RGM). It is a rare pathogen in humans, and only a very few cases of skin and lung infection related to it have been reported. In this report, we present the case of a 70-year-old Hispanic male who was brought to the hospital with back pain for 11 months. His physical exam on admission showed point tenderness in the lumbar and thoracic spine. MRI demonstrated severe spinal stenosis, discitis, and adjacent osteomyelitis at the T11-T12 vertebral bodies. Mycobacterium culture with fluorochr

Percutaneous management of complications of tuberculous spondylodiscitis: short- to medium-term results.

PURPOSE: Psoas abscesses are the most frequent complication of tuberculosis with skeletal involvement. The aim of this paper is to report our experience with the systematic application of percutaneous drainage to tuberculous psoas abscesses. MATERIALS AND METHODS: Between January 1997 and December 2005, 23 patients (14 men and nine women; age range 21-48 years), after a previous study with computed tomography (CT) and/or magnetic resonance (MR) imaging, underwent percutaneous drainage of a tuberculous fluid collection in the psoas muscles. Follow-up consisted of monthly clinical and laborator

Pott's Spine Unveiled: A Comprehensive Case Report and Surgical Intervention.

This case report describes the presentation, diagnosis, and surgical management of a 61-year-old female admitted to a tertiary care hospital with a two-month history of neck pain and weakness in all four limbs. Despite the absence of a clear history of trauma, a detailed examination revealed restricted neck flexion, paraspinal muscle spasm, and neurological deficits. Contrast-enhanced MRI indicated vertebral osteomyelitis and discitis at the C5-C6 level, with a suspected infective etiology, possibly tuberculosis spondylitis. The patient underwent anterior cervical decompression, corpectomy of

Post-tuberculosis Sequelae in Children.

Tuberculosis (TB) is a global health problem especially in the Indian subcontinent imposing significant physical, psychosocial and economic burden on the society. Most national programs define TB cure as completion of treatment with improvement in clinical symptoms, microbiological and radiological clearance. However, follow up these patients for long-term sequelae or complications has not got adequate attention. Post-TB lung disease, neurological deficits and spinal deformities are some of the post-TB sequelae reported in adults, with scanty data available for children. With this review autho

Case Report: Multifocal Tubercular Osteomyelitis of the Spine and Bilateral Dactylitis.

We report a case of a 13-year-old immunocompetent male with multifocal tubercular osteomyelitis involving several spinal segments, small bones of the hands, and the scalp, who started with progressively back pain and enlarging painful swelling on the palms of hands, fatigue, and irregular fever. All the hand lesions were firm, mildly tender, and covered by ulcerated skin with serous discharge from the site. Magnetic resonance showed lesions of the right fifth metacarpal, of the right intermediate phalanx of the fourth finger, of the left second metacarpal, and of most vertebral bodies of the c

Obesity as the initial manifestation of central nervous system relapse of acute lymphoblastic leukemia: case report and literature review.

A 6-year-old boy with acute lymphoblastic leukemia in remission experienced hyperphagia, obesity, and emotional disorders. Cytomorphologic examination of cerebral spinal fluid (CSF) and cranial MRI did not help in differentiating between central nervous system leukemia (CNSL) and other CNS diseases including tuberculosis in this boy. Flow cytometric CSF analysis on repeated lumber puncture detected lymphoblasts, while microscopic CSF examination did not definitively show relapse disease. The diagnosis of CNSL was thus made and confirmed by the response to leukemia treatment. Obesity can be the

A case of persistent hyponatraemia due to reset osmostat.

We report a case of a 65 year old Malay lady with long-standing diabetes mellitus, who presented to our institution with a one month history of worsening neck pain and progressive upper and lower limb weakness. She was stable despite severe hyponatraemia which was initially treated as syndrome of inappropriate anti-diuretic hormone (SIADH). This was consistent with her underlying illness which was concluded as cervical tuberculosis (TB) with spinal cord compression. She underwent decompression and bone grafting. Despite continuous treatment her serum sodium levels remained low. There were no o

Pyogenic osteomyelitis of the spine--a review of 16 consecutive cases.

Pyogenic osteomyelitis of the spine is an uncommon disease. A series of 16 cases is reported. The site of involvement includes the lumbar, cervical, and thoracic segments, in that order of frequency of occurrence. Drug addiction and diabetes mellitus are important predisposing factors. The spectrum of clinical presentation is described. Late presentation is usually masked by old age, debilitation, and previous antibiotic treatment. Rapid progression with septicemia is also encountered. The importance of differentiating this condition from tuberculosis spondylitis is emphasized. Definitive diag

[Information value of x-ray computed tomography for the detection and control of spondylitis].

The information content of early bone changes in computed tomography (CT) of pyogenic non-tuberculous spondylitis and spinal tuberculosis is analysed. The CT findings are compared with conventional x-ray images including tomography. In 3 of 16 patients no indications of osteomyelitis were found by conventional x-ray images, whereas CT revealed paravertebral soft tissue swelling or abscesses and osteolysis and thus established the diagnosis of spondylitis. In one patient the process caused neurologic symptoms by extending into the epidural space. The contributions of CT to the diagnosis of spon

Chronic relapsing experimental allergic encephalomyelitis in the Lewis rat.

Chronic relapsing experimental allergic encephalomyelitis (CR-EAE) was induced in rats with an emulsion of guinea-pig spinal cord tissue (GPSC) in complete Freund's adjuvant (CFA) enriched with Mycobacterium tuberculosis H37RA (Tbc). 78% of the sensitized rats developed a CR-EAE showing 2 to 3 clinical relapses during the first 40 days. After 60-80 days, approximately half of the rats with CR-EAE had a further relapse which was followed by complete recovery in only 35% of the cases. The remaining 65% of these animals showed a progressive state of the disease, characterized by paralysis or seve

[Tuberculous otitis media, mastoiditis associated with meningitis: report of one case].

Tuberculosis of the middle ear is rare; associated meningitis is even more unusual. This report contains our experience with a one and a half year old psychomotor retarded boy. He had a poor healing left postauricular abscess four months prior to admission. Also noted was postprandial vomiting and left sided involuntary movement. On admission spinal tapping was done. Results showed leukocytosis, with lymphocytes being predominant, as well as high protein and low glucose levels. A cranial CT revealed left mastoiditis, hydrocephalus, basal cistern abnormal enhancement and a prominent posterior f

Tuberculosis and non-pulmonary malignancies : study of ten cases.

INTRODUCTION: Tuberculosis (TBC) is a major public health problem with high mortality especially in developing countries. It is associated with a higher risk of developing pulmonary and non-pulmonary malignancies including solid and hematologic cancers. Association between TBC and nonpulmonary malignancies is rarely described in the literature. AIM: To describe the epidemiological, clinical, therapeutic modalities and the evolutive aspects of patients treated for cancer and TBC. METHODS: This is a retrospective study conducted over a period of 19 years (between 1993 and 2012), including 10 p

Tuberculous retropharyngeal abscess with Pott disease and tuberculous abscess of the chest wall: A case report.

RATIONALE: Tuberculous retropharyngeal abscess is rare, but it can be fatal if not treated appropriately. It usually occurs secondary to tuberculosis of the cervical spine. Moreover, tuberculous abscess involving the chest wall is relatively rare in skeletal tuberculosis. Although the optimal treatment is controversial, most clinicians suggest a combination of sufficient antituberculous medication and complete resection to prevent recurrence and increase therapeutic efficacy. Herein, we present an unusual case of retropharyngeal abscess with cervical Pott disease and tuberculous abscess of the

Tuberculosis of the lower cervical spine (C5-C6) in a 24-month-old infant.

BACKGROUND: Tuberculosis (TB) of the cervical spine occurs rarely in younger children whereas the presentation and outcome are different from those of adult cervical spine. Because cervical spinal TB in younger children is rarely reported, the clinical characteristics, the treatments, and the expected outcome of treatments in younger children are still unknown. METHODS: We present a case of cervical spine TB in a 24-month-old boy that grounds severe vertebral destruction and an extradural abscess. This child presented with neurological deficit in the form of quadraparesis. We performed anteri

Tuberculosis of the spine presenting with a cold abscess through the lumbar triangle of Petit.

The increasing incidence of human immunodeficiency virus (HIV) has resulted in an increase in the number of Mycobacterium tuberculosis (TB) infections worldwide. Musculoskeletal tuberculosis often involves the spine. Due to HIV, patients may present atypically and, as a result, a high clinical suspicion is necessary to avoid the catastrophic consequences of untreated Pott's disease of the spine. A cold abscess as a result of tuberculosis can emerge in a number of anatomical regions, and perhaps most notably as a psoas abscess. We report a less common anatomical location of a cold abscess, pres

[Morphology and endocrinology of a fatal course of Addison crisis].

A 36 year-old man fell ill with violent dorsalgia seemingly related to spondylosis. The diagnosis of tuberculous spondylitis was made as late as 18 months after the onset of symptoms. The disease rapidly progressed to death in spite of adequate tuberculostatic and surgical therapy. The post-mortem examination revealed a massively caseating tuberculosis of both adrenal glands suggestive of Addison's disease. Severe adrenocortical insufficiency was confirmed by low steroid hormone levels in the post-mortem blood; the validity of these results is indicated by comparative hormone level determinati
