Current indications for (adeno)tonsillectomy in children: a survey in The Netherlands.

OBJECTIVE: Despite the fact that (adeno)tonsillectomy is one of the procedures most frequently performed on children, studies of current indications are scarce. The purpose of this study is to determine the indications for (adeno)tonsillectomy in children younger than 15 years of age according to Dutch ENT surgeons and general practitioners (GPs). METHODS: During a period of 8 months, 18 ENT surgeons in seven ENT practices and 210 referring GPs filled out standard questionnaires for 349 children listed for tonsil surgery. RESULTS: Apart from recurrent tonsillitis (ENT: 40%, GP: 35%), finding

Value-based pathology: a cost-benefit analysis of the examination of routine and nonroutine tonsil and adenoid specimens.

To study the cost-effectiveness of the histologic examination of tonsil and adenoid specimens, the histologic diagnoses for all routine (2,700) and nonroutine (71) tonsil and adenoid specimens during a 10-year period were reviewed. There were 27 routine cases (1%) and 56 nonroutine cases (79%) with a diagnosis other than normal, tonsillitis, or hyperplasia. Twelve of the 27 routine cases did not have a significant clinical history, and a potentially significant pathologic diagnosis was made in only 6 cases; in none of these cases did the pathologic diagnosis affect patient care. In all nonrout

[Cholesteatoma and chronic tubal middle ear infection in children. A 10 year overview].

Over a 10-year period we observed 99 cholesteatomas. In 62% the cholesteatomas lay in the epitympanum, in 18% they lay behind an intact tympanic membrane, and in 18% in the posterior-superior portion of the pars tensa. There were 2 cases of iatrogenic cholesteatoma following insertion of ventilation tubes. In the same period we saw 1229 children suffering from chronic serous otitis media (SOM). A total of 1918 ventilation tubes were inserted in 856 children with SOM, so that the incidence of iatrogenic cholesteatomas is extremely low at 0.1%. Our findings indicate that cholesteatomas in the po

Obstructive sleep apnea and its therapy: clinical and polysomnographic manifestations.

Obstructive sleep apnea syndrome was studied in 32 children, aged 2 to 14 years, in the sleep-wake disorders center at Montefiore Hospital and Medical Center during the years 1977 to 1980. All children underwent all-night polysomnograms; 17 of these children had surgery to relieve airway obstruction and seven had a repeat polysomnographic study 4 to 6 weeks following the surgery. There was a significant improvement in the number of obstructive apneas and in other apnea indices following surgery. There was no significant effect on the durations and the proportions of the various sleep stages, o

Long term results of treatment of middle ear effusion.

Ten years follow-up of 48 patients (33 boys, 15 girls) with middle ear effusion treated by adeno-tonsillectomy, antral lavage and paracentesis is presented. Using the modified but stricter classification of Tos and Poulsen, the results compare well (Table V). Agreement between otoscopy and impedance measurement was remarkably high. It was noted that the greater the number of repeated paracenteses, the higher the compliance rose above normal, but there was no relation to the viscosity of the fluid found. In ears in which grommets were used, only in 10 per cent was the tympanic membrane found to

Surgery for Obstructive Sleep Apnea in Children With Down Syndrome in an Australian Population.

OBJECTIVES: The prevalence of obstructive sleep apnea (OSA) is known to be higher in children with Down syndrome (DS) than the general pediatric population, with lower rates of surgical cure. This study aims to determine the prevalence and predictors of OSA and evaluate the outcomes of surgical intervention for OSA in a cohort of Australian children with DS. METHODOLOGY: A retrospective chart review was conducted on 156 patients with DS from 0 to 18 years who had undergone overnight, attended polysomnography (PSG) at Sydney Children's Hospital from January 2010 to July 2023. Logistic regressi

Impact of intraoperative ketorolac on postoperative pain in children undergoing adenotonsillectomy: a double blind, placebo-control trial.

PURPOSE: Postoperative pain control following adenotonsillectomy in the pediatric population poses a great challenge to care providers. Multi-modal pain management regimes including NSAIDs such as intraoperative ketorolac usage has been purposed for many years. However, the effectiveness of ketorolac to reduce post-tonsillectomy pain and opioid-related side effects is controversial. The study was to evaluate the opioid-sparing effect of an intraoperative intravenous single dose of ketorolac in children undergoing adenotonsillectomy. We also assessed the effectiveness of perioperative ketorolac

Pilot study of the potential of 3D ultrasound to measure tonsillar volume and hypertrophy.

OBJECTIVE: Obstructive sleep apnea (OSA), results in approximately 4-5 million outpatient visits per year in the United States. In pediatric patients, OSA is primarily caused by adenotonsillar hypertrophy, and therefore, adenotonsillectomy remains an effective surgical treatment. We investigate whether 3D ultrasound (3DUS) imaging can accurately and objectively assess tonsillar hypertrophy for the potential identification and stratification of candidates for adenotonsillectomy. METHODS: A prospective study was performed evaluating pediatric patients (N = 17) between the ages of 4-14 years who

Anesthetic management during adenotonsillectomy for twins with congenital insensitivity to pain with anhidrosis: two case reports.

BACKGROUND: Congenital insensitivity to pain with anhidrosis is a rare autosomal recessive disorder characterized by hyperpyrexia, anhidrosis, pain insensitivity, self-inflicted injuries, and intellectual disability. The anesthetic management of these patients is challenging owing to the high risk of perioperative complications resulting from their autonomic dysfunction, such as hyperthermia, hypotension, and bradycardia, which result from autonomic nervous system dysfunction. CASE PRESENTATION: Two 3-year-old Han Chinese identical male twins (weighing 13.5 kg and measuring 93 cm tall) were p

The Clinical Usefulness of Sleep Studies in Children.

Sleep disordered breathing is common in children and has the potential to have a significant impact on cognition, activity and social interaction. The overnight in-laboratory polysomnography (PSG) continues to be the gold standard instrument for the investigation of sleep-disordered breathing in children. It has the ability to rule in or rule out the need for intervention for common conditions such as obstructive sleep apnoea, assess the role of sleep quality in children and adolescents with hypersomnolence, provide physiologic data in children with hypoventilation as may be seen in neuromuscu

Children's quality of life after adenotonsillectomy.

OBJECTIVE: Although adenotonsillectomy is one of the most frequently performed surgical procedures in the pediatric population, its impact on health-related quality of life (HRQL) has not been well established. The objective of this study was to determine the impact of adenotonsillectomy on children's HRQL. METHODS: Parents of the children who underwent adenotonsillectomy were invited to our clinic approximately one year after the surgery for a face-to-face survey to evaluate their child's HRQL. In total, 119 parents of children who underwent adenotonsillectomy for different reasons were inte

Pediatric obstructive sleep apnea syndrome.

Obstructive sleep apnea syndrome (OSAS) is a common and serious cause of metabolic, cardiovascular, and neurocognitive morbidity in children. Children with OSAS have increased upper airway resistance during sleep due to a combination of soft tissue hypertrophy, craniofacial dysmorphology, neuromuscular weakness, or obesity. Consequently, children with OSAS encounter a combination of oxidative stress, inflammation, autonomic activation, and disruption of sleep homeostasis. The threshold amount of OSAS associated with adverse consequences varies widely among children, depending on genetic and en
