The impact of Germany's human papillomavirus immunization program on HPV-related anogenital diseases: a retrospective analysis of claims data from statutory health insurances.

PURPOSE: Human papillomavirus (HPV) is the most common sexually transmitted infection, responsible for multiple HPV-related diseases, including almost all cervical cancers. The highly effective HPV vaccination has been recommended under the German HPV national immunization program (NIP) since 2007 and is reimbursed by health insurances. Vaccination uptake rates, however, remain suboptimal and data on the real-world impact of HPV vaccination in Germany are lacking. This study aims to demonstrate the population-level impact of Germany's NIP on HPV-related anogenital diseases among young women.

Radiologic Assessment of Groin Lymph Nodes in Pelvic Malignancies.

INTRODUCTION: Metastatic involvement of groin nodes can alter radiation therapy planning for pelvic tumors. 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) can identify nodal metastases; however, interpretation of PET/CT-positive nodes can be complicated by non-malignant processes. We evaluated quantitative metrics as methods to identify groin metastases in patients with pelvic tumors by comparison with standard subjective interpretive criteria, with pathology as the reference standard. METHODS: We retrospectively identified patients with vulvar, vag

Marital status in relation to Kaposi's sarcoma, non-Hodgkin's lymphoma, and anal cancer in the pre-AIDS era.

Persons with human immunodeficiency virus/AIDS are at high risk of Kaposi's sarcoma (KS), non-Hodgkin lymphoma (NHL), and possibly anal cancers. To examine whether this risk preceded the AIDS epidemic, we used pre-AIDS era data from the Surveillance, Epidemiology, and End Results program (excluding Connecticut) from 1973 to 1976, and the Connecticut Tumor Registry from 1940 to 1976. We compared risk of being single (a surrogate to identify men who might be homosexual) to those ever married, using a case control matching study with up to 10 controls per case. Overall, no excess risk was observe

A decade of anal cancer screening in the context of HIV care.

BACKGROUND: High-resolution anoscopy (HRA) is crucial for detecting and managing high-grade squamous intraepithelial lesions (HSIL), the main precursor to anal cancer. However, program success is often hindered by long-term retention, which in some studies reaches only 50%. Patient acceptability, a major factor in adherence, is often overlooked. OBJECTIVE: To evaluate the long-term feasibility, safety, acceptability, and retention of an anal cancer screening program in an HIV clinic that prioritizes patient-centered strategies. METHODS: This single-center, longitudinal cohort study was condu

Challenges in the implementation of a high-resolution anoscopy clinic for people with HIV in an oncologic center in Mexico City.

BACKGROUND: Anal cancer incidence is increased in people with HIV (PWH), up to 60 times higher in men who have sex with men, and almost 15 times higher among women with HIV. Screening and treating high-grade lesions (HSIL) have proven to reduce the incidence of invasive anal cancer. In 2020, we started implementing a high-resolution anoscopy (HRA) clinic at INCan, a tertiary care oncologic center, as part of a screening program for PWH from the HIV clinic. OBJECTIVES: We describe the barriers and difficulties in implementing an HRA Clinic from January 2020 to April 2021, including physician t

Very pronounced bowel sparing during radiation therapy for anal carcinoma using a natural spacer (Myoma) - a case report.

BACKGROUND: Using dose-painted intensity-modulated radiation therapy, specific dose volume constraints or implantation of tissue expanders prior to radiotherapy are validated options for reducing radiation dose on the bowel and therefore minimizing acute gastrointestinal toxicity during chemoradiation for anorectal malignancies. We describe the rare case of a female patient with a locally advanced anal carcinoma where a large myomatous uterus served as a natural spacer to protect the bowel during radiation therapy. CASE PRESENTATION: Initially the patient presented with anal pain, proctoscopy

Shared Decision-Making Concerning Anal Cancer Screening in Persons With Human Immunodeficiency Virus.

BACKGROUND: Anal high-grade squamous intraepithelial lesion (aHSIL) is the immediate precursor of anal cancer. Anal cytology is a recommended screening test to identify aHSIL among people with human immunodeficiency virus (HIV; PWH). Heterogeneity of risk for invasive anal cancer among PWH suggests the value of a shared decision-making framework regarding screening. METHODS: Using a longitudinal HIV cohort with a comprehensive anal cancer screening program, we estimated the adjusted probabilities of having aHSIL on the first anal cytology. We used logistic regression models with inverse proba

INTRODUCTION: The Prevanal study is interested in anal cancer in MSM: a rare cancer linked in 90 % of cases to an infection with the human papillomavirus (HPV). Sexually transmitted despite the use of a condom, it affects in particular HIV-positive MSM who are considered a particularly exposed group. PURPOSE OF RESEARCH: It aims above all to translate an epidemiological problem into a socio-anthropological question focused on sexuality and gender identities. The challenge is to develop a multidimensional approach to this emerging and resurgent health issue among MSM. RESULTS: In this period

DNA Methylation Analysis to predict Regression of high-grade anal Intraepithelial Neoplasia in HIV+ men (MARINE): a cohort study protocol.

INTRODUCTION: Anal cancer precursors, or high-grade anal intraepithelial neoplasia (HGAIN), are highly prevalent in HIV-seropositive (HIV+) men who have sex with men (MSM). Around 30% of lesions regress within 1 year, but current histopathological assessment is unable to distinguish between HGAIN likely to regress and HGAIN likely to persist or progress to cancer. We aim to assess if host cell DNA methylation markers can predict regression of HGAIN, thus determining the need for immediate treatment or active surveillance. This could reduce overtreatment and the associated anal and psycho-sexua
