The Involvement of HIF-1α and BDNF in Neonatal Hypoxic-Ischemic Insult to the Cerebral Germinal Matrix.

Perinatal asphyxia is a major contributor to neonatal morbidity and mortality, particularly among preterm infants, whose brains are highly vulnerable to hypoxic-ischemic injury. The germinal matrix (GM), owing to its vascular fragility and high metabolic demand, is especially susceptible in this context. This study analyzed 118 germinal matrix samples from neonates, stratified into three groups according to gestational age-Extremely Preterm (EP), Late Preterm (LP), and Term (T)-to investigate the immunopositivity of hypoxia-inducible factor 1-alpha (HIF-1α) and brain-derived neurotrophic facto

Correlation of cardiac troponin T levels with inotrope requirements, HIE stages and outcome in asphyxiated neonates.

BackgroundAsphyxial event can compromise the function of many vital organs like brain, heart, and kidneys. Cardiac dysfunction results from hypoxic injury to subendocardial tissue, papillary muscle, and myocardium. The aim of this work was to assess correlation between cardiac troponin T levels, myocardial dysfunction, inotrope requirements, hypoxic ischemic encephalopathy (HIE) stages, and outcome in asphyxiated neonates.MethodsThis cohort study was conducted on 48 neonates aged (1-28) days old, both sexes, exposed to perinatal asphyxia, and admitted in NICU unit in Benha University Hospital

OptiNeoCare: optimisation of routine care in the management of severe perinatal asphyxia in full-term or near-term newborns - study protocol for analysis of suboptimal care by confidential inquiries and e-self report.

INTRODUCTION: Severe perinatal asphyxia at term or near term remains a critical public health issue, associated with high risks of neonatal death and hypoxic-ischaemic encephalopathy (HIE). Despite improved clinical guidelines, suboptimal care persists in many cases, and previous audits have demonstrated that up to 50% of asphyxia cases could be associated with suboptimal care. OptiNeoCare is a French study which aims to assess the prevalence and determinants of suboptimal obstetric and neonatal care and evaluate its potential impact on neonatal outcomes. MATERIALS AND METHODS: This prospecti

The role of endothelial frequency in the cerebral blood flow control during neonatal asphyxia: a retrospective longitudinal study.

BACKGROUND: Cerebral blood flow dynamics can be explored through analysis of endothelial frequencies. Our hypothesis posits a disparity in endothelial activity among neonates with perinatal asphyxia, stratified by the presence or absence of neuronal lesions. METHODS: We conducted a retrospective longitudinal study involving newborns treated with hypothermia for moderate to severe asphyxia. Participants were grouped based on the presence or absence of neuronal damage to investigate temporal endothelial involvement in cerebral blood flow regulation. Regional cerebral oxygen saturation (rScO2) w

Hypoxia - Reoxygenation in neonatal cardiac arrest: Results from experimental models.

In this review, we summarize the results of studies that investigated the effects of hypoxia and reoxygenation in cardiac arrest, including the use of different fractions of inspired oxygen, in neonatal animals. The studies were heterogenous in terms of anaesthetic regimens, and definitions of cardiac arrest and circulatory recovery. Cardiopulmonary resuscitation with 100% oxygen increased oxidative stress in maturing rats. Studies in fetal/neonatal lambs and post-transitional neonatal piglets indicate no consistent differences between ventilation with 21% vs. 100% oxygen with regards to recov

A study of microRNAs from dried blood spots in newborns after perinatal asphyxia: a simple and feasible biosampling method.

BACKGROUND: The potential of microRNAs (miRNAs) as bedside biomarkers in selecting newborns with hypoxic-ischemic encephalopathy (HIE) for neuroprotection has yet to be explored. Commonly, blood-based biomarker tests use plasma or serum which don't allow evaluation of both intracellular and extracellular changes. METHODS: We describe a technique to extract and compare expression of miRNAs from a single small 6-mm-diameter dried blood spot (DBS) stored at room temperature with those from EDTA-blood, plasma, and urine. Three miRNAs (RNU6B, let7b, and miR-21) were quantified via extraction and q
