Delayed treatment of frostbite injury with hyperbaric oxygen therapy: a case report.

The effects of hyperbaric oxygen therapy on skin microcirculation were evaluated by laser-Doppler flowmetry and vital capillary microscopy in a frostbite victim 2 wk after the injury. Laser-Doppler skin blood flow, measured in intact skin on the dorsum of the foot, decreased from 52 to 31 perfusion units during hyperbaric oxygen therapy. The number of visible nutritive capillaries in frostbitten areas on the toes increased from 2 to 12 per mm2, as measured immediately after the hyperbaric oxygen therapy. We conclude that hyperbaric oxygen therapy is capable of improving nutritive skin blood fl

Avulsion of the triceps tendon during acceleration stress.

The authors present a rare case of triceps tendon avulsion associated with +5 G-force and muscle straining. Although failure of musculoskeletal system, mainly the cervical spine, secondary to high-G exposure has been reported, this is the first report they have found of a triceps tendon avulsion. Furthermore, unlike other traumatic triceps avulsions reported, the pilot involved had no known predisposing factors. Awareness of the possibility of such an injury must be maintained when a pilot of high performance aircraft presents with elbow pain.

Double hearing protection and speech intelligibility-room for improvement.

INTRODUCTION: Double hearing protection is used in many air forces around the world for protection in noisy aircraft environments, particularly in helicopters. The usual combination is foam ear plugs under headset or helmet muffs. Much of the research that spurred the introduction of foam earplugs indicated little change in speech intelligibility in persons with normal hearing. However, aircrew often complain about having to maximize intercom volume for speech understanding, causing a situation with no reserve volume and bad sound quality. In recent years, further developments have included so

Determination of lumbar muscular activity in helicopter pilots: a new approach.

BACKGROUND: Discomfort in the lumbar region is a frequent complaint of helicopter pilots. Two factors that helicopter crewmembers relate to back pain are exposure to vibration and poor posture during flight, because during flight the seat and controls force the pilot to adopt an asymmetric posture. Repeated exposure to these adverse flight conditions could cause pathological changes in the spine. The purpose of this study was to compare right and left sided lumbar muscular activity in helicopter pilots under real flight conditions, using surface electromyography (SEMG) to demonstrate the effec

Relationship between arm pain and distension of arteries and veins caused by elevation of transmural pressure in local vascular segments.

BACKGROUND: Exposure to high +Gz forces may induce arm pain, which has been hypothesized to be caused by pressure-induced overdistension of local blood vessels. The purpose of the present investigation was to study the pressure-distension relation of veins and arteries in the human arm and the relation between arm pain and distension of local vessels. METHODS: Increased distending pressures (DP) in the vasculature of the arm were accomplished by placing the subject (n = 8) in a pressure chamber with one arm positioned through a port in the chamber door, and increasing chamber pressure to +180

The effects of age and sex on susceptibility to motion sickness.

BACKGROUND: The present investigation is concerned with the role of sex, age and experience in determining motion sickness susceptibility. HYPOTHESIS: Motion sickness susceptibility varies with sex and age, and may be related to the level of physical activity sustained by the individual. METHODS: Three surveys were conducted. The first included school age children (9-18 yr) and addressed motion experience and motion sickness on 13 forms of transport. The second questionnaire sought to determine whether these sex differences could be understood in terms of differential involvement in various

A questionnaire for the assessment of the multiple dimensions of motion sickness.

BACKGROUND: A limited number of attempts have been made to develop a questionnaire that assesses the experience of motion sickness. Further, many available questionnaires quantify motion sickness as a unidimensional construct. METHOD: Exploratory and confirmatory factor analyses of motion sickness descriptors were used to derive and verify four dimensions of motion sickness, which were defined as gastrointestinal, central, peripheral, and sopite-related. These dimensions of motion sickness were then used to construct a motion sickness assessment questionnaire (MSAQ) that was administered to i

A motion sickness maximum around the 0.2 Hz frequency range of horizontal translational oscillation.

BACKGROUND: Low frequency translational oscillation can provoke motion sickness in land vehicles, ships and aircraft. Although controlled motion experiments indicate a progressive increase in nauseogenicity as frequency decreases toward 0.2 Hz, few data are available on the existence of a definite maximum nauseogenic potential of motion around 0.2 Hz, or decreased nauseogenicity below this frequency. HYPOTHESIS: Nauseogenicity should be maximal around 0.2 Hz. METHODS: We selected 12 subjects for high motion sickness susceptibility, and they were exposed to horizontal sinusoidal motion (1.0 m

Modeling cardiac sensitization potential of humans exposed to Halon 1301 or Halon 1211 aboard aircraft.

BACKGROUND: Halon 1301 and Halon 1211 are being replaced because they contribute to the depletion of ozone. Many of the potential candidate chemicals for replacing them are, like them, halogenated hydrocarbons. These chemicals have the potential to cause cardiac sensitization at high enough exposure concentrations. METHODS: A physiologically based pharmacokinetic model, which mathematically describes the uptake, distribution, metabolism, and elimination of chemicals, was used to relate exposure to these chemicals with arterial blood concentrations resulting from the exposure. This information

Immune responses and latent herpesvirus reactivation in spaceflight.

BACKGROUND: Increased frequency and severity of herpesvirus infections are common in individuals with impaired cellular immunity, a phenomenon observed in both the elderly and astronauts alike. This study investigated immune responses and latent herpesvirus reactivation during a 9-d spaceflight. In addition, adrenocortical and immune responses of an elderly astronaut (payload specialist-2, PS2; age 77) who flew on this mission were compared with that of younger crewmembers. HYPOTHESIS: Spaceflight and associated stresses will decrease cellular immunity and reactivate latent herpesviruses. ME

Visually-induced reorientation illusions as a function of age.

We reported previously that supine subjects inside a furnished room who are tilted 90 degrees may experience themselves and the room as upright to gravity. We call this the levitation illusion because it creates sensations similar to those experienced in weightlessness. It is an example of a larger class of novel static reorientation illusions that we have explored. Stationary subjects inside a furnished room rotating about a horizontal axis experience complete self rotation about the roll or pitch axis. We call this a dynamic reorientation illusion. We have determined the incidence of static

Exposure to soda-lime dust in closed and semi-closed diving apparatus.

BACKGROUND: Chronic exposure to hyperbaric hyperoxia and venous gas microembolism have been shown to contribute to the long term health effects of diving, especially diver's lung function. Factors related to special diving equipment may add to these effects. This study was conducted to evaluate possible additional hazards for respiratory function of divers employing closed and semi-closed diving apparatus. METHODS: We analyzed soda-lime dust found in the air-intake loop of a closed-circuit oxygen rebreathing diving apparatus which had passed through the filter screen of the diving apparatus's

The role of the flight surgeon in Greece.

From 1915 [corrected], the first time a flight surgeon participated in an aeromedical evacuation, to the present, the role has become more fundamental by working in wars and operations, experiencing search and rescue aeromedical evacuations from mass accidents or motor accidents, treating seriously ill individuals, and caring for wounded victims of attacks either at the scene or at the hospital. The improvements in cognitive and technological standards of medical science and in the education available have contributed in upgrading the role of flight surgeons; however, their presence in flight

Exercise rhabdomyolysis in military aircrew: two cases and a review of aeromedical disposition.

INTRODUCTION: Exercise rhabdomyolysis (ER) is a rare, sometimes catastrophic condition where muscle fibers breakdown in response to exertion and release their breakdown products into the circulation. While ER was brought to the medical community's attention largely through reports from military training establishments we are unaware of ER having resulted from the activities of military aircrew. This paper reports two cases of ER in military aircrew and discusses the condition and the approaches taken in determining their future aeromedical disposition. CASE REPORTS: Rhabdomyolysis occurred in

Vestibular reactions to spaceflight: human factors issues.

Vestibular function, along with other sensory systems influencing spatial orientation, can have a profound influence on the ability of astronauts to function effectively. Beyond the well-known problems of space motion sickness, vestibular effects can influence astronaut well-being and performance during all phases of a space mission. This paper discusses some of the major vestibular reactions affecting human factors encountered in all space missions, and covers them chronologically in the following sequence: launch, early on-orbit, late on-orbit, EVA, artificial gravity, re-entry, and post-lan

Effect of seating, vision and direction of horizontal oscillation on motion sickness.

BACKGROUND: Low frequency horizontal oscillation can cause motion sickness in some forms of transport, but the influence of the characteristics of the motion and the visual and postural conditions of the body on sickness are not known. HYPOTHESES: It was hypothesised that body position, vision and direction of motion will have an effect on motion sickness. METHOD: There were 72 seated subjects who were exposed to horizontal oscillation at 0.25 Hz, 0.7 ms(-2) r.m.s. (peak-to-peak displacement of 0.8 m) for up to 30 min while in 1 of 6 conditions. Three conditions involved fore-and-aft motion

Hoffmann-reflex is delayed during 6 degree head-down tilt with balanced traction.

BACKGROUND: Increased spinal height due to the lack of of axial compression on spinal structures in microgravity may stretch the spinal cord, cauda equina, nerve roots, and paraspinal tissues. HYPOTHESIS: Exposure to simulated microgravity causes dysfunction of nerve roots so that the synaptic portion of the Achilles tendon reflex is delayed. METHODS: Six healthy male subjects were randomly divided into two groups with three in each group. The subjects in the first group underwent horizontal bed rest (HBR) for three days. After a two week interval they underwent bed rest in a position of hea

The effects of roll vs. pitch rotation in humans under orthostatic stress.

BACKGROUND: It has been known since 1953 that pre-exposure to less than +1 Gz will reduce subsequent +Gz-tolerance. With few exceptions, during operational flying, the transition from hypogravity to hypergravity involves roll as well as pitch rotation. We examined the effect of roll vs. pitch rotation while undergoing transition from hypogravity to +1 Gz on a tilt table. METHODS: Twelve subjects (28-47 yr old) were rotated at 45 degrees x s(-1) from head-up (HU) at 15 degrees relative to gravitational vertical to 135 degrees head-down (HD) and back to the HU position after different HD dwell
