Long-term development of white matter fibre density and morphology up to 13 years after preterm birth: A fixel-based analysis.

BACKGROUND: It is well documented that infants born very preterm (VP) are at risk of brain injury and altered brain development in the neonatal period, however there is a lack of long-term, longitudinal studies on the effects of VP birth on white matter development over childhood. Most previous studies were based on voxel-averaged, non-fibre-specific diffusion magnetic resonance imaging (MRI) measures, such as fractional anisotropy. In contrast, the novel diffusion MRI analysis framework, fixel-based analysis (FBA), enables whole-brain analysis of microstructural and macrostructural properties

Significance of the corpus callosum and inferior fronto-occipital fasciculus in recovery after traumatic brain injury.

OBJECTIVE: To analyze the correlations between the consciousness state scores and the fractional anisotropy (FA) values in various segments of the Corpus Callosum (CC) and Inferior Fronto-Occipital Fasciculus (IFOF) at different stages of recovery after traumatic brain injury (TBI). METHODS: Diffusion tensor imaging (DTI) was performed in 43 TBI patients and 22 healthy volunteers. The consciousness levels were estimated with the CRS-R scale. The tracts were divided into segments using a template based on tractograms of healthy volunteers, and the correlations between the CRS-R scores and the

An iterative ROC procedure identifies white matter tracts diagnostic for traumatic brain injury: an exploratory analysis in U.S. Veterans.

OBJECTIVE: Understanding the pathophysiology of traumatic brain injury (TBI) is crucial for effectively managing care. Diffusion tensor imaging (DTI) is an MRI technology that evaluates TBI pathology in brain white matter. However, DTI analysis generates numerous measures. Choosing between them remains an obstacle to clinical translation. In this study, we leveraged an iterative receiver operating characteristic (ROC) analysis to examine white matter tracts in a group of 380 Veterans, consisting of TBI (n = 243) and non-TBI patients (n = 137). METHOD: For each participant, we obtained a whole

Does the Apparent Diffusion Coefficient Value Predict Permanent Cerebral Ischemia/Reperfusion Injury in Rats?

RATIONALE AND OBJECTIVES: Variation in tissue damage after cerebral ischemia/reperfusion (I/R) can cause uncertainty in stroke-related studies, which can be reduced if the damage can be predicted early after ischemia by measuring the apparent diffusion coefficient (ADC). We investigated whether ADC measurement in the acute phase can predict permanent cerebral I/R injury. MATERIALS AND METHODS: The middle cerebral artery occlusion model was established using the intraluminal suture method to induce 60 minutes of ischemia followed by reperfusion in rats. T2-weighted images and diffusion-weighte

Structural and Functional Integrity of the Intraparietal Sulcus in Moderate and Severe Traumatic Brain Injury.

Severe and moderate traumatic brain injury (sTBI) often results in long-term cognitive deficits such as reduced processing speed and attention. The intraparietal sulcus (IPS) is a neocortical structure that plays a crucial role in the deeply interrelated processes of multi-sensory processing and top down attention. Therefore, we hypothesized that disruptions in the functional and structural connections of the IPS may play a role in the development of such deficits. To examine these connections, we used resting state magnetic resonance imaging (rsfMRI and diffusion kurtosis imaging (DKI) in a c

Diffusion-weighted imaging changes in cerebral watershed distribution following neonatal encephalopathy are not invariably associated with an adverse outcome.

AIM: Patterns of injury in term-born infants with neonatal encephalopathy following hypoxia-ischaemia are seen earlier and are more conspicuous on diffusion-weighted magnetic resonance imaging (DW-MRI) than on conventional imaging. Although the prognostic value of DW-MRI in infants with basal ganglia and thalamic damage has been established, data in infants in whom there is extensive injury in a watershed distribution are limited. The aim of this study was to assess cognitive and functional motor outcome in a cohort of infants with changes in a predominantly watershed distribution injury on ne

Multimodal imaging of striatal degeneration in Amish patients with glutaryl-CoA dehydrogenase deficiency.

Despite early diagnosis, one-third of Amish infants with glutaryl-CoA dehydrogenase deficiency (GA1) develop striatal lesions that leave them permanently disabled. To better understand mechanisms of striatal degeneration, we retrospectively studied imaging results from 25 Amish GA1 patients homozygous for 1296C>T mutations in GCDH. Asymptomatic infants had reduced glucose tracer uptake and increased blood volume throughout gray matter, which may signify a predisposition to brain injury. Nine children (36%) developed striatal lesions: three had sudden motor regression during infancy whereas six

Acute and Chronic Effects of Multiple Concussions on Midline Brain Structures.

BACKGROUND AND OBJECTIVES: To test the hypothesis that a history of concussion (HOC) causes greater disturbances in cerebral blood flow (CBF) and white matter microstructure of midline brain structures after subsequent concussions, during the acute and chronic phases of recovery. METHODS: In this longitudinal MRI study, 61 athletes with uncomplicated concussion (36 with HOC) were imaged at the acute phase of injury (1-7 days after injury), the subacute phase (8-14 days), medical clearance to return to play (RTP), 1 month after RTP, and 1 year after RTP. A normative group of 167 controls (73 w

[Values of diffusion-weighted magnetic resonance imaging in the early detection of periventricular leukomalacia in preterm infants].

OBJECTIVE: Periventricular leukomalacia (PVL), the principal form of brain injury in the premature infant, is characterized by overt focal necrotic lesions in periventricular white matter and diffuse cerebral white matter injury. The early detection of the disease is not consistently possible with cranial ultrasonography or conventional magnetic resonance imaging (MRI). Recently, diffusion-weighted imaging (DWI) has been shown to be highly sensitive in detecting acute ischemic cerebral injury. This study aimed to evaluate possible role of DWI in early diagnosis of PVL. METHODS: Images and cli

Normal appearing white matter and disability in multiple sclerosis.

BACKGROUND AND OBJECTIVES: Multiple sclerosis (MS) is characterized by lesions and atrophy on conventional MRI, yet these often fail to explain disability. Diffusion MRI (dMRI) detects microstructural injury with diffusion tensor (DTI) and kurtosis imaging (DKI) offering sensitivity, and Standard Model Imaging (SMI) providing biologically interpretable parameters. We evaluated whether these clinically feasible dMRI metrics are associated with disability beyond volumetric and lesion measures, and whether effects arise from normal-appearing white matter (NAWM). METHODS: This cross-sectional stu

APOE 𝜀4-related blood-brain barrier breakdown is associated with microstructural abnormalities.

INTRODUCTION: Blood-brain barrier (BBB) dysfunction occurs in Alzheimer's disease (AD). Yet, the stage at which it appears along the AD time course and whether it contributes to neurodegeneration remain unclear. METHODS: Older adults (61 to 90 years) from cognitively normal (CN) to mildly cognitively impaired (CI), enriched for APOE 𝜀4 and amyloid positivity, underwent dynamic contrast-enhanced (DCE) magnetic resonance imaging (MRI) and diffusion MRI to measure BBB permeability and brain microstructure. Analysis of variance compared BBB permeability according to cognitive status, amyloid beta

Structure-function coupling in highly sampled individual brains.

Structural connectivity (SC) between distant regions of the brain support synchronized function known as functional connectivity (FC) and give rise to the large-scale brain networks that enable cognition and behavior. Understanding how SC enables FC is important to understand how injuries to SC may alter brain function and cognition. Previous work evaluating whole-brain SC-FC relationships showed that SC explained FC well in unimodal visual and motor areas, but only weakly in association areas, suggesting a unimodal-heteromodal gradient organization of SC-FC coupling. However, this work was co

Partial Recovery in Toxic Leukoencephalopathy: Is It Really a Slow Improvement or a Warning Sign?

A 55-year-old African American man who was found down by a friend nine hours after being last seen at the same place was brought to the emergency department (ED) with encephalopathy, lactic acidosis, rhabdomyolysis, elevated troponin, acute kidney injury (AKI), and transaminitis. His urine drug screen (UDS) was positive for cocaine. Intravenous (IV) Narcan was given with minimal improvement in mental status. A computed tomography (CT) scan of the head and a CT scan of the cervical spine in the ED showed no acute findings. Due to hypoxia, the patient was eventually intubated. The patient also r

Cerebral Blood Flow of the Neonatal Brain after Hypoxic-Ischemic Injury.

OBJECTIVE: Hypoxic-ischemic encephalopathy (HIE) in infants can have long-term adverse neurodevelopmental effects and markedly reduce quality of life. Both the initial hypoperfusion and the subsequent rapid reperfusion can cause deleterious effects in brain tissue. Cerebral blood flow (CBF) assessment in newborns with HIE can help detect abnormalities in brain perfusion to guide therapy and prognosticate patient outcomes. STUDY DESIGN: The review will provide an overview of the pathophysiological implications of CBF derangements in neonatal HIE, current and emerging techniques for CBF quantif

Safety and Efficacy of Remote Ischemic Preconditioning in Patients With Severe Carotid Artery Stenosis Before Carotid Artery Stenting: A Proof-of-Concept, Randomized Controlled Trial.

BACKGROUND: Remote ischemic preconditioning (RIPC) can inhibit recurrent ischemic events effectively in patients with acute or chronic cerebral ischemia. However, it is still unclear whether RIPC can impede ischemic injury after carotid artery stenting (CAS) in patients with severe carotid artery stenosis. METHODS: Subjects with severe carotid artery stenosis were recruited in this randomized controlled study, and assigned to RIPC, sham, and no intervention (control) groups. All subjects received standard medical therapy. Subjects in the RIPC and sham groups underwent RIPC and sham RIPC twice

Brain structural changes in COPD patients with cognitive impairment.

Chronic obstructive pulmonary disease (COPD) is increasingly recognized as a systemic condition associated with an elevated risk of cognitive impairment, particularly in domains of executive function and attention, presenting a pattern distinct from Alzheimer's disease. This review synthesizes evidence from multimodal neuroimaging studies to characterize the cerebral alterations in COPD and frame them within the context of accelerated brain aging. Patients with COPD exhibit widespread structural brain changes, including reduced gray matter volume in cognitively critical regions, such as prefro

Early Influences of Microbiota on White Matter Development in Germ-Free Piglets.

Abnormalities in the prefrontal cortex (PFC), as well as the underlying white matter (WM) tracts, lie at the intersection of many neurodevelopmental disorders. The influence of microorganisms on brain development has recently been brought into the clinical and research spotlight as alterations in commensal microbiota are implicated in such disorders, including autism spectrum disorders, schizophrenia, depression, and anxiety via the gut-brain axis. In addition, gut dysbiosis is common in preterm birth patients who often display diffuse WM injury and delayed WM maturation in critical tracts inc

The Effect of Thymoquinone on the Characteristics of the Brain Extracellular Space in Transient Middle Cerebral Artery Occlusion Rats.

The extracellular space (ECS) is the space between the neurons and the capillaries in the brain. The volume fraction (α) and the tortuosity (λ) are the main parameters used to describe its characteristics. Thymoquinone has been proved to possess anti-oxidant and anti-inflammatory activity. In this study, we used a gadolinium-diethylenetriaminepentacetate (Gd-DTPA)-enhanced magnetic resonance imaging (MRI) system to determine the effects of thymoquinone on ECS parameters in transient middle cerebral artery occlusion rats (tMCAO) to prove the neuroprotective effect of thymoquinone on brain tissu
