Contact dermatitis is very common and is among the top 20 reasons for a patient to visit a physician. This article reviews the basic features of irritant and allergic contact dermatitis and provides case examples. We emphasize the use of patch testing to diagnose specific allergens in allergic contact dermatitis. Contact dermatitis is important to recognize since there are prevention and intervention strategies that may be of great help to the patient. Accurate diagnosis and prevention of contact dermatitis would have a positive impact on medical care costs and job productivity.

A detailed survey of skin complaints amongst 114 airline employees working in a new warehouse revealed 26 cases of skin problems which originated during the 2 1/2 year operation. A clinical survey of broadly the same population confirmed 14 cases from 98 employees as chronic irritant contact dermatitis of the hands. Fiberglass is widely used for insulation and as a reinforcement filling material. Handling fiberglass products may induce contact dermatitis. We report on the first two cases of fiberglass dermatitis reported in Taiwan. A patient with dermatitis herpetiformis (DH) developed abnormal liver function tests and was found to have histological features of primary sclerosing cholangitis (PSC) on liver biopsy. The association of DH and PSC has not been previously reported and the high incidence of phenotype HLA B8 and DR3 in these disorders suggests a common genetic predisposition may exist for both diseases.

During the past decade, the reported incidence of allergic reactions to topically applied glucocorticosteroids has increased significantly. The two cases reported here typify the presenting symptoms of the glucocorticosteroid-allergic persons. These cases also illustrate the extensive cross-reactivity among the different topical corticosteroids, as well as the potential for systemic contact dermatitis to occur following parenteral exposure. These cases are reviewed in the context of the many other reports in the literature of similar allergic reactions.

Seven cases of fiberglass dermatitis among production operators in the electronics industry are reported. This was due to the repeated daily handling of printed circuit boards (PRCBs). The predominant complaint of the workers was itch of the lateral aspects of the fingers and finger webs. In six workers, unroofed vesicles, pinhead areas of postinflammatory hyperpigmentation, and excoriation marks were noted at these sites. A case of arthritis dermatitis syndrome observed after a biliopancreatic bypass for morbid obesity is described. The syndrome had begun 10 days after surgery and involved the knees, ankles, elbows and wrists and erythema nodosum on the legs. Cutaneous eruptions are commonly seen in acquired immunodeficiency syndrome (AIDS). Seborrheic dermatitis in this patient population is usually more severe and difficult to diagnose and treat. Over the past 20-years, it has been shown that the majority of patients with dermatitis herpetiformis (D.H.) suffer from coeliac disease of varying intensity. Dermatitis herpetiformis may also be associated with other autoimmune diseases but only exceptionally with chronic ulcerative colitis (U.C.).
