OBJECTIVE: To study the effect of various middle ear effusions on the structure of the lamina propria of the tympanic membrane.
METHODS: Sterile and infective middle ear effusions were induced by obstruction of the eustachian tube in specific pathogen-free (SPF) rats and in rats with upper airway infections (URI), respectively. The condition of the tympanic membrane was monitored at regular intervals. After varying survival times, the animals were killed and the tympanic membranes processed for light and electron microscopy.

A young female developed persistent nodules at sites of ear piercing with gold earrings and patch testing demonstrated a positive allergic response to gold sodium thiosulphate. Histological examination of the nodules demonstrated a prominent sarcoidal-type granulomatous tissue reaction. This is in contrast to previous reports of lymphocytoma cutis type histology and was associated with the occurrence of epithelioid granulomata at the site of a strongly positive and long-lasting patch-test reaction.

The tubal function of 68 patients with a unilateral traumatic perforation of the tympanic membrane and 116 ears of 94 patients with chronic otitis media were examined by equalization manometry. The values obtained in the traumatic group are regarded as being characteristic of normal tubal function: the patients were able to equalize +/- 30 hPa difference in pressure with between three and seven swallows. Only 25% of the ears with a dry central perforation and none with an infected central perforation were able to fulfil this requirement.
