Diagnostic associations between pleural and pulmonary tuberculosis.

OBJECTIVE: To evaluate diagnostic differences and associations between pleural tuberculosis (PLTB), and pulmonary tuberculosis alone (PTB), as well as, pleural and pulmonary co-infection (PLPTB) and PTB. METHODS: Data from a 6-year, population-based study in Houston, Texas, were analyzed by logistic regression modeling. Study personnel interviewed 2226 tuberculosis cases for TB risk assessment and reviewed medical records. Mycobacterium tuberculosis isolates from available culture-positive patients underwent genotyping. RESULTS: Among the categories analyzed [PTB, n=2070; PLTB, n=95; and PLP

Residual spatial correlation between geographically referenced observations: a Bayesian hierarchical modeling approach.

BACKGROUND: Analytic methods commonly used in epidemiology do not account for spatial correlation between observations. In regression analyses, this omission can bias parameter estimates and yield incorrect standard error estimates. We present a Bayesian hierarchical model (BHM) approach that accounts for spatial correlation, and illustrate its strengths and weaknesses by applying this modeling approach to data on Wuchereria bancrofti infection in Haiti. METHODS: A program to eliminate lymphatic filariasis in Haiti assessed prevalence of W. bancrofti infection in 57 schools across Leogane Com

Estimating disease prevalence in the absence of a gold standard.

When estimating disease prevalence, it is not uncommon to have data from conditionally dependent diagnostic tests. In such a situation, the estimation of prevalence is difficult if none of the tests is considered to be a gold standard. In this paper we develop a Bayesian approach to estimating disease prevalence based on the results of two diagnostic tests, allowing for the possibility that the tests are conditionally dependent, but not conditioning on any particular dependence structure. This involves the construction of four models with various forms of conditional dependence and uses Bayesi

Application of path analysis to urinary findings of cadmium-induced renal dysfunction.

In order to identify some causal relations among various urinary indices of cadmium-induced renal dysfunction, such as glucose, total protein, amino nitrogen, beta 2-microglobulin (beta 2-m), metallothionein (MT), and cadmium (Cd), we applied path analysis method to previous epidemiological studies targeting the residents of the Cd-polluted Kakehashi River basin of Ishikawa Prefecture, Japan. We obtained a diagram-termed path model, representing some causal relations among the above urinary indices. It shows that urinary Cd is located at the beginning point in the diagram, and Cd-induced renal

[Persistence and resurgence of sleeping sickness caused by Trypanosoma brucei gambiense in historic foci. Biomathematical approach of an epidemiologic enigma].

Since the end of the 19th century, historic endemic foci of Trypanosoma brucei gambiense sleeping sickness have proven very persistent. A five-compartment mathematical model with open vector populations was developed in order to study the dynamics of this disease in Central Africa. Of particular interest is the rate at which the disease spreads or goes to extinction at the beginning of an epidemic outbreak. A measure of this rate is the initial halving/doubling time T(o) of the numbers infected; T(o) is a doubling time when the basic reproduction number Ro > 1 and a halving time when Ro < 1. F

Sex and death: are they related? Findings from the Caerphilly Cohort Study.

OBJECTIVE: To examine the relation between frequency of orgasm and mortality. STUDY DESIGN: Cohort study with a 10 year follow up. SETTING: The town of Caerphilly, South Wales, and five adjacent villages. SUBJECTS: 918 men aged 45-59 at time of recruitment between 1979 and 1983. MAIN OUTCOME MEASURES: All deaths and deaths from coronary heart disease. RESULTS: Mortality risk was 50% lower in the group with high orgasmic frequency than in the group with low orgasmic frequency, with evidence of a dose-response relation across the groups. Age adjusted odds ratio for all cause mortality was 2

Alcohol and injury severity: reappraisal of the continuing controversy.

It is well recognized that alcohol increases the risk of injury. It is controversial, however, whether alcohol also has an effect on the degree and outcome of injury after controlling for the severity of impact. This review examines the evidence from experimental studies in animals regarding the potentiating effects of alcohol on trauma, and the methodologic issues that may have contributed to the contradictory findings of clinical and epidemiologic studies. Most experimental studies indicate that alcohol can adversely affect the degree and outcome of injury. In controlled laboratory condition

Predicting intrauterine growth retardation in sibships while considering maternal and infant covariates.

Data on 6,210 full-term sibships (gestational age greater than 36 weeks) reconstructed from birth certificates registered in Maryland between 1980 and 1984 were used to study sibship aggregation and recurrence risk of intrauterine growth retardation (IUGR) while considering maternal and infant covariates. In this study, IUGR was defined as being below the 10th percentile for race-, sex-, and gestational-age-specific birth weight based on all births registered in Maryland during 1984. Maternal factors (race, age at delivery, marital status, amount of prenatal care, concurrent illnesses, and com

Genetic and Nongenetic Risk Factors for Breast Cancer Risk Estimation.

IMPORTANCE: Most breast cancers in Africa are diagnosed at advanced stages. Improved risk prediction tools to optimize screening and earlier diagnosis are urgently needed. OBJECTIVE: To build a comprehensive breast cancer risk estimation model by integrating a polygenic risk score (PRS), pathogenic variants (PVs) in high- or moderate-penetrance genes, and a questionnaire-based risk calculator. DESIGN, SETTING, AND PARTICIPANTS: This multicenter case-control study initially enrolled women in Nigeria in 1998 and expanded to Cameroon and Uganda in 2011; enrollment ended in 2018. Women with brea

Predicting Forearm Physical Exposures During Computer Work Using Self-Reports, Software-Recorded Computer Usage Patterns, and Anthropometric and Workstation Measurements.

OBJECTIVES: Alternative techniques to assess physical exposures, such as prediction models, could facilitate more efficient epidemiological assessments in future large cohort studies examining physical exposures in relation to work-related musculoskeletal symptoms. The aim of this study was to evaluate two types of models that predict arm-wrist-hand physical exposures (i.e. muscle activity, wrist postures and kinematics, and keyboard and mouse forces) during computer use, which only differed with respect to the candidate predicting variables; (i) a full set of predicting variables, including s

Epidemiology, Clinical Features, and Outcomes of Coccidioidomycosis, Utah, 2006-2015.

On the basis of a 1957 geographic Coccidioides seropositivity survey, 3 counties in southwestern Utah, USA, were considered coccidioidomycosis-endemic, but there has been a paucity of information on the disease burden in Utah since. We report findings from a recent clinical and epidemiologic study of coccidioidomycosis in Utah. To describe clinical characteristics, we identified all coccidioidomycosis cases in an integrated health system in the state during 2006-2015. For epidemiologic analysis, we used cases reported to the Utah Department of Health during 2009-2015. Mean state incidence was

Multistage carcinogenesis: Impact of age, genetic, and environmental factors on the incidence of malignant mesothelioma.

The current paradigm of carcinogenesis as a cellular evolutionary process driven by mutations of a few critical driver genes has immediate logical implications for the epidemiology of cancer. These include the impact of age on cancer risk, the role played by inherited tumor predisposition syndromes, and the interaction of genetics and environmental exposures on cancer risk. In this paper, we explore the following logical epidemiological consequences of carcinogenesis as a clonal process of mutation accumulation, with special emphasis on asbestos-related cancers, specifically malignant mesothel

Is there a role for diet in ameliorating the reproductive sequelae associated with chronic low-grade inflammation in polycystic ovary syndrome and obesity?

A 2013 ASRM committee opinion titled "Optimizing natural fertility" stated that "there is little evidence that dietary variations such as vegetarian diets, low-fat diets, vitamin-enriched diets, antioxidants, or herbal remedies improve fertility …." However, there are emerging epidemiologic data demonstrating that certain components of the diet may influence reproductive health outcomes. Furthermore, translational work with human specimens and animal models lends biologic plausibility to the epidemiologic data, particularly in the context of female reproductive diseases associated with inflamm

[Different potentials of a geographic information system for studies in epidemiology: example of animal trypanosomiasis in the Sudan region].

The epidemiology of vector-borne diseases is complex due to variability in the ecology of the different actors involved, i.e., hosts, parasites and vectors. Transmission of animal trypanosomiasis in the West African savannah region is an excellent example of this complexity. Because of the importance of place and time in determining the interface between host and vector, a broad view of the overall environment is necessary to understand the variety of situations that can be encountered in an epidemiological system. Spatial distribution is a key factor for risk evaluation. State-of-the-art geog

Chaotic and Stochastic Components in an Influenza Surveillance Series: Nonlinear Dynamics and Predictive Modeling Study.

BACKGROUND: Chaotic dynamics has been the subject of both theoretical and empirical research in epidemiology, with the most recent research strongly focusing on SARS-CoV-2. However, few empirical studies have been undertaken with respect to influenza, even though evidence of chaos has also been found in influenza surveillance data. Furthermore, empirical studies on chaos are focused on reconstructing hidden attractors in epidemiological time series to filter out noise; however, dynamical noise affecting chaotic dynamics can have relevant epidemiological features that are, in this way, left unr

Making psychiatric epidemiology useful: the contribution of epidemiology to government policy.

This paper aims to discuss the contribution of epidemiology to aspects of public policy that have either a direct influence on mental health and mental disorders, or an indirect effect by influencing environmental factors which influence mental health. Both kinds of public policy will need to be considered by governments wishing to protect, promote, and improve the mental health of their populations. The paper draws on information from both relatively wealthy and low-income countries. The paper defines epidemiology and mental health policy, sets out the range of government policies which may h

Estimating the causal effects of exposure mixtures: a generalized propensity score method.

BACKGROUND: In environmental epidemiology and many other fields, estimating the causal effects of multiple concurrent exposures holds great promise for driving public health interventions and policy changes. Given the predominant reliance on observational data, confounding remains a key consideration, and generalized propensity score (GPS) methods are widely used as causal models to control measured confounders. However, current GPS methods for multiple continuous exposures remain scarce. METHODS: We proposed a novel causal model for exposure mixtures, called nonparametric multivariate covari

Feasibility of achieving the 2025 WHO global tuberculosis targets in South Africa, China, and India: a combined analysis of 11 mathematical models.

BACKGROUND: The post-2015 End TB Strategy proposes targets of 50% reduction in tuberculosis incidence and 75% reduction in mortality from tuberculosis by 2025. We aimed to assess whether these targets are feasible in three high-burden countries with contrasting epidemiology and previous programmatic achievements. METHODS: 11 independently developed mathematical models of tuberculosis transmission projected the epidemiological impact of currently available tuberculosis interventions for prevention, diagnosis, and treatment in China, India, and South Africa. Models were calibrated with data on
