Background: Arthritis of the distal interphalangeal joint (DIPJ) can result in significant functional limitations and pain. While arthrodesis is the most common surgical intervention, this can decrease grip strength and have other limitations. DIPJ arthroplasty may be an appealing alternative in select patient with this study aiming to review the outcomes of this procedure. Methods: A search was conducted according to PRISMA guidelines using PubMed, Embase and Ovid Medline from date of inception to April 2022. Relevant studies were included if they reported on complications and functional outc

The treatment of metacarpophalangeal destruction by rheumatoid arthritis is an indication for arthroplasty, the swanson spacer is one of the options. The flexible silicone spacer may be implanted with or without titanium guide sleeves ("grommets"). However, the imaginary abrasion protection for the silicone grommets may in turn lead through micro-movements to wear. A 69 years old female patient suffering from rheumatoid arthritis for 30 years is presented. 12 years ago swansen spacers were implanted to metacarpo-phalangeal joints 2 -5 of the right hand. Due to pain, limited movement and swelli

Combined dislocation of the metacarpophalangeal and interphalangeal joints of the thumb is uncommon. We know of only four previously reported cases. We report a new case characterised by dorsal dislocation of both joints. Because of entrapment of the volar plate, open reduction at the interphalangeal joint was necessary. The metacarpophalangeal dislocation was treated by closed reduction. After three weeks of immobilisation, physiotherapy resulted in a satisfactory outcome. Even if the diagnosis of dislocation of the interphalangeal joint is obvious it would be easy to overlook a simultaneous

This paper deals with the uncommon and frequently unrecognized problem of trapped dislocations of the proximal interphalangeal joint. These may be dorsal or volar. Soft tissue forming a noose, or interposed in the joint, is implicated. There is injury to one or more of the following structures: 1) extensor mechanism, 2) collateral ligament, 3) volar plate, 4) flexor tendon sheath, and 5) skin (compound dislocations). Open reduction and appropriate soft tissue repair are mandatory the essential features of diagnosis and management are discussed and four illustrative cases are presented.

Ulnar deviation is a common complication in patients with rheumatoid arthritis (RA). We report a case of an unusual radial deviation of the middle finger caused by an occult intramuscular ganglion of the second interosseous muscle (IOM) in a patient with RA. The resection of the ganglion did not resolve the problem, and the full range of motion of the metacarpophalangeal (MP) joint was achieved through dissection of the tendon of the second dorsal IOM.

The relationship between the flexor and extensor systems of the digits is both intricate and balanced, such that disruption of one system can affect the entire dynamics of the finger. The imbalance may be obvious, whereas the precipitating factor may be less obvious. These authors describe a case and provide a detailed biomechanical analysis of how a flexion contracture of the distal interphalangeal joint led to a swan neck deformity in one of their patients.

Data of 13,037 live-born infants from a hospital in Tehran, Iran were analysed for congenital malformations and genetic diseases. The results showed that the rates of joint dislocation, cleft lip, cleft palate and finger anomalies are similar to those of the other populations. The rates of chromosomal, thorax and abdominal, external genital anomalies and other syndromes were higher compared with other populations, whereas the rates of multiple births and limb anomalies were lower.

BACKGROUND: Isolated avascular necrosis of the metacarpal head, also known as Dietrich's disease, is a rare entity with few cases reported previously in the literature. It has been associated with steroid use, lupus, and trauma. We describe the clinical presentation, pathogenesis, and treatment options associated with this uncommon condition.
CASE REPORT: A 36-year-old man presented with a 1-year history of a painful right middle finger metacarpophalangeal joint (MCPJ). There was no preceding history of trauma. Routine blood investigations and plain-film imaging were unremarkable. Magnetic re

A detailed literature search was performed to investigate whether the increasing importance of evidence-based hand surgery is reflected in the actual status of Cochrane reviews. Fourteen Cochrane reviews were found and evaluated. Of these, five reviews were in the field of distal radial fractures and four concerned carpal tunnel syndrome. Cochrane reviews were also found for antibiotic treatment, rehabilitation after flexor tendon injuries, mallet finger injuries, little finger metacarpal neck fractures and thumb joint arthritis. All 14 reviews were compromised by methodological flaws and sign

INTRODUCTION AND IMPORTANCE: Oligoarticular juvenile idiopathic arthritis (JIA) is the most common JIA subtype, typically involving large joints and presenting as mono- or oligoarthritis within 6 months of onset. Small joint involvement is rare and can complicate diagnosis.
CASE PRESENTATION: We report an 8-year-old Palestinian girl presenting with isolated swelling of the proximal interphalangeal (PIP) joint of the right index finger. She experienced stiffness and restricted motion without systemic symptoms. Initial evaluations, including imaging and biopsy, suggested trauma. Persistent symp

It is well known that treatment of the old fracture-dislocations of the fifth carpometacarpal (CMC) joint is challenging. For patients with persistent pain due to this condition, numerous surgical techniques have been proposed. Here, we present a novel surgical technique, CMC joint suspension arthroplasty, using a partial slip of the extensor carpi ulnaris tendon. This procedure follows principles similar to the procedure of trapeziectomy with suspension arthroplasty for treatment of thumb CMC joint arthritis. We believe this suspension arthroplasty has several advantages compared with other s

BACKGROUND: Metacarpophalangeal joint destruction in rheumatoid arthritis leads to pain, deformity, impaired function and poor cosmesis. Arthroplasty remains a principal reconstructive option for restoring alignment and improving function.
METHODS: This narrative review summarizes the current concepts of metacarpophalangeal joint arthroplasty in patients with rheumatoid arthritis. The historical evolution of surgical techniques and implant designs is outlined, with particular emphasis on flexible silicone implants, which remain the most widely used option. Indications, contraindications, impl

AIMS: Few reports have focused on the distal interphalangeal (DIP) joint in patients with rheumatoid arthritis (RA). The purposes of this st … erved at the DIP joint in patients with RA, and these cases frequently shows bone erosions of other finger joints, such as PIP joint. In addition, bone erosion can be observed soon after the onset of RA caused by uncontrolled disease activity in some patients with RA.

Dislocations of thumb interphalangeal (IP) joint are rare injuries due to inherent stability of the joint. This report presents a case of complex irreducible dorsal IP joint dislocation of thumb due to interposition of four structures - volar plate, sesamoid bone, flexor pollicis longus tendon and digital nerve which is perhaps the only case reported so far in literature where more than three structures have been implicated for this joint irreducibility. The author presents a comparative review of similar case reports previously reported in literature listing the controversies in management an

We report the case of a 2-week-old girl born at term (by vaginal delivery and without antenatal or perinatal events) who was referred as having "bilateral talipes and bilateral proximal symphalangism of little and ring fingers." The "talipes" was atypical with marked equinus and varus, but no cavus or adductus of the midfoot. Her mother had both symphalangism (absence of proximal interphalangeal joints) of middle, ring, and little fingers bilaterally and fixed pes planus with a rigid fixed hindfoot-and these deformities had also been present from birth. The maternal grandmother was similarly a
