Maternal patterns of antenatal and postnatal depressed mood and the impact on child health at 3-years postpartum.

OBJECTIVE: The consequences of maternal depressed mood on children's growth, health, and cognitive and language development are examined ove … oyed or to have electricity; were more likely to report problematic drinking of alcohol, experience food insecurity, interpersonal partner violence, and to be HIV seropositive. At 36 months, the pattern of maternal depressed mood over time was significantly associated with children's compromised physical growth, both in weight and height, and more internalizing a …

Tryptophan depletion in context of the inflammatory and general nutritional status of a low-income South African HIV-infected population.

BACKGROUND: The essential amino acid tryptophan cannot be synthesised in the body and must be acquired through dietary intake. Oxidation of … eactive protein. The most probable causes of the lower tryptophan levels seen in our population are food insecurity and higher levels of inflammatory activity. CONCLUSIONS: We contend that inflammation-induced tryptophan depletion forms part of a much wider effect of pro-inflammatory activity on the nutritional profile of HIV-infected patients.

OBJECTIVE: To determine which combination of risk factors from Community Care of North Carolina's (CCNC) Pregnancy Medical Home (PMH) risk s … itized by the PMH program remained in our final model (ROC = 0.66). The odds of PTB associated with food insecurity and obesity differed by parity. The influence of unsafe or unstable housing and short interpregnancy interval on PTB differed by race/ethnicity. Beyond therapeutic feeding: a call for integrated strategies to end child malnutrition in Somalia.

Somalia records one of the highest global burdens of child wasting and stunting, with approximately 1.7-1.85 million children under five estimated to be affected by acute malnutrition annually during 2022-2025, including approximately 421,000-513,550 children with severe acute malnutrition. For over two decades, the dominant policy response has been community-based management of acute malnutrition (CMAM), a curative intervention that treats clinical endpoints rather than underlying drivers. Despite meeting international cure-rate standards, CMAM alone cannot address the food insecurity, water,…

Multilevel Modeling for Causal Inference in Oral Health Research.

Multilevel modeling (MLM) is increasingly applied in oral health research to reflect the hierarchical structure of dental data, in which tee … limited consideration of cross-level interactions. Using a school-based example, we illustrate how food insecurity mediates the association between household income and active dental caries, highlighting the value of analytical approaches that jointly address individual- and contextual-level determinants. Overall, MLM provides a robust framework for clarifying c …

What's in the Bin? A Waste Audit of an Australian Tertiary Ear, Nose and Throat Outpatient Clinic.

BACKGROUND: Healthcare contributes 4.4% of global emissions. Climate change contributes to vector-borne diseases, food insecurity and morbidity and mortality associated with extreme weather. Ear, nose and throat (ENT) outpatient clinics are particularly resource intensive, yet their environmental impact has not been quantified. The aim of this study was to quantify waste generation in an ENT outpatient clinic and estimate associated carbon equivalent emissions produced. METHODS: Disposable waste from 20 consecutive ENT outpatient clinics at a tertiary Australian hospital was collected, counte…

Design of Heart Well: A randomized pilot study of a community health worker-delivered nutrition intervention for low-income adults with hypertension.

BACKGROUND: Suboptimal diet and food insecurity contribute to hypertension disparities experienced by individuals with low-income. Clinic-based nutrition counseling is limited by time constraints and low access to dietitians. Integrating community health workers (CHWs) with basic nutrition training into care teams may be an effective strategy to promote sustainable dietary modifications. OBJECTIVE: Describe the design of Heart Well, a study to determine whether adding nutrition counseling to a primary care-based CHW hypertension program is feasible and acceptable and improves diet and blood p…

Artificial Intelligence and Machine Learning for Identifying Social Determinants of Health in Low-Income Populations Within United States Health Systems: A Scoping Review.

BACKGROUND AND AIMS: Low-income populations in the United States experience disproportionate exposure to adverse social conditions. This scoping review examined artificial intelligence (AI) and machine learning (ML) approaches used in United States health systems to identify financial hardship, housing instability, food insecurity, neighborhood deprivation, and other social determinants of health (SDOH), and characterized interventions implemented in response. METHODS: Following the Arksey and O'Malley framework and PRISMA-ScR guidance, MEDLINE, CINAHL Plus with Full Text, PsycINFO, and Acade…

Variations in the viral hepatitis C prevalence and treatment status by material hardship and cumulative risk among people who inject drugs.

People who inject drugs (PWID) are disproportionately impacted by viral hepatitis C (HCV). Among PWID, homelessness, poverty, and material insecurity elevate infectious disease risk. We hypothesized a positive association between material hardship (difficulty accessing basic needs) and lifetime diagnosis of HCV, and a negative association between cumulative risk (material hardship and years since first injection) and HCV treatment. From 2021-22, we conducted a survey among community-recruited PWID that included items on HCV outcomes, material hardship (a sum score of usually (4) to never (1) h
