Since November 1984, lengthening of metacarpals and digital phalanges by a distraction apparatus has been performed in six metacarpal bones and four phalangeal bones in nine patients (four females, five males). The age ranged from 14 to 59 years (average 36). The follow-up period was from two to 28 months (average 16). Lengthening was performed by two kinds of external distraction devices, the Ikuta external fixateur for metacarpals and the mini-Hoffmann-device for phalanges. Five cases of compression of the deep palmar branch of the ulnar nerve were operated upon and the hiatus released by subperiosteal removal of the pisiform bone. The clinical and electromyographical results after a minimum follow-up of eight months showed an improvement in all cases. The authors stress the importance of accurate electromyographic examination for a correct preoperative diagnosis.

Bone homeostasis is securely controlled by the dynamic well-balanced actions among osteoclasts, osteoblasts and osteocytes. Osteoclasts are large multinucleated cells that degrade bone matrix and involve in the bone remodelling in conjunction with other bone cells, osteoblasts and osteocytes, the completely matured form of osteoblasts. Disruption of this controlling balance among these cells or any disparity in bone remodelling caused by a higher rate of resorption by osteoclasts over construction of bone by osteoblasts results in a reduction of bone matrix including bone mineral density (BMD)

BACKGROUND: Postaxial polydactyly, type B is the most common type of polydactyly. The vestigial sixth finger is attached by a narrow neurovascular pedicle to the lateral aspect of the hand or foot at the level of the metacarpal-phalangeal joint or the metatarsal-phalangeal joint. The occurrence of this type of polydactyly varies among racial groups, by sex and sidedness. Postaxial polydactyly, type A is a fully developed extra digit on the lateral aspect of the hand or foot with a bifid fifth or sixth metacarpal/metatarsal and is much less common.

Posttraumatic myositis ossificans (PTMO) is the most common form of myositis ossificans, but it rarely affects the hand. Due to the aggressive behavior of this unusual tumor, it is commonly misdiagnosed as malignancy or local infection. This report describes a case of PTMO of the hand in a 25-year-old women, and reviews the clinical, radiologic, and histologic characteristics. By understanding the disease characteristics and maturation process, physicians can avoid a misdiagnosis that may lead to functional impairment or improper treatment.

Synovial reactions do not only affect articular but also tenosynovial compartments as proven by low kV radiography in 3 views. Erosions develop in both compartments and also enthesitically at the insertions of fibrous structures. There are also localized osseous changes, which resemble localized areas of hyperparathyroidism. In cases with unilateral trauma of a lesser degree scintigraphic examination of the soft tissues and bones of the contralateral side may confirm the results. The response of the soft tissues and bones is more severe distally.

Congenital thumb hypoplasia is a rare deformity of the upper extremity. The classification of thumb hypoplasia was created by Blauth in 1967 (types I to V). In two cases a grip reconstruction by double toe-to-hand transfer is demonstrated. In the first patient all fingers of the left hand were amputated at the metacarpo-phalangeal joint level. A retrospective determination of the yield from screening bone scintigraphy in detecting bone metastasis when used for disease staging of 93 asymptomatic patients with locally extensive head and neck cancer was undertaken. This case report is focused on a patient (a 32-year-old previously healthy man) with a femoral bone tissue tumor. The case underscores the diagnostic complexity of Telangiectatic Osteosarcoma (TOS) with Aneurysmal Bone Cystic (ABC) lesion and the importance of maintaining special immunohistochemical markers for differential diagnosis between low-grade osteosarcoma, primary ABC, and suspected TOS, which is a true mimicker of ABC.
