Image-guided biopsy of the lung or of the mediastinum is a minimally invasive method for acquiring material for microbiologic or histologic examinations. Given careful selection and preparation of the patient, image-guided biopsy is a safe procedure with a relatively low risk of complications. The aim of this tutorial is to summarize the special aspects concerning the intervention, including evaluation, the technical needs, different kinds of access to the target, and risk and complication management.

Therapeutic embolization of a thoracoacromial artery perforated by placement of a deep venous catheter.

An 82-year-old man presented with massive external bleeding from the puncture site after placement of a deep venous catheter in the right subclavian vein. Emergent angiography revealed contrast extravasation along the central venous catheter tract due to perforation of the right thoracoacromial artery. Superselective catheterization of the bleeding artery followed by microparticle embolization definitively stopped the hemorrhage. During follow-up no recurrence of external bleeding was noted. Percutaneous embolization using microparticles is an effective tool to definitively treat iatrogenic ar

[Gastric wall necrosis owing to its incarceration through a rupture of the diaphragm into the left thoracic cavity].

A 73 year-old female patient was admitted to the 3rd Medical Department of Semmelweis University with a painful haematoma in the left loin and respiratory disorders. Her general condition was getting progressively worse. Chest X-ray demonstrated a left sided hemopneumothorax caused by a fractured rib. Thoracic drainage was planned, but the tube introduced on the usual place into the left thoracic cavity perforated the stomach which was incarcerated in the chest. After this an urgent operation was carried out. We found an incarcerated, twisted stomach prolapsing through a rupture of the diaphra

Is a routine chest x-ray necessary for children after fluoroscopically assisted central venous access?

PURPOSE: The aim of this study was to determine in a pediatric population whether a routine chest x-ray after central venous access is neces … ve fluoroscopy was used. There were 17 (1.6%) complications; 9 (0.9%) were pulmonary (pneumothorax, hemothorax, or an effusion). All children with pulmonary complications experienced clinical signs and symptoms suggestive of the complication after their central venous catheter insertion but before their postprocedure chest x-ray. CONCLUSIONS: The number of …

Penetrating chest injuries in the firearm era.

BACKGROUND: firearm wounds of the chest are now common at our institution. The management algorithm for firearm wounds has not been evaluated for this mode of injury. METHODS: records of all patients with penetrating chest injuries admitted to an urban tertiary hospital over 1 year were retrieved and analysed. RESULTS: there were 473 stab and 116 firearm wounds. In comparison to stab injuries firearm wounds had significantly more normal X-rays (14 vs. 5%), fewer pneumothoraces (15 vs. 37%), and more contusions (43 vs. 2%). The frequency of haemothoraces (34 vs. 23%) and haemopneumothoraces (

Hereditary Hemorrhagic Telangiectasia and Arterio-Venous Malformations-From Diagnosis to Therapeutic Challenges.

Hereditary hemorrhagic telangiectasia is a rare autosomal dominant vascular disease defined by the presence of mucosal and cutaneous telangiectasia and visceral arterio-venous malformations. The latter are abnormal capillary-free direct communications between the pulmonary and systemic circulations with the following consequences: arterial hypoxemia caused by right-to-left shunts; paradoxical embolism with transient ischemic attack or stroke and brain abscess caused by the absence of the normally filtering capillary bed; and hemoptysis or hemothorax due to the rupture of the thin-walled arteri…

Presumptive antibiotics in tube thoracostomy for traumatic hemopneumothorax: a prospective, Multicenter American Association for the Surgery of Trauma Study.

BACKGROUND: Thoracic injuries are common in trauma. Approximately one-third will develop a pneumothorax, hemothorax, or hemopneumothorax (HPTX), usually with concomitant rib fractures. Tube thoracostomy (TT) is the standard of care for these conditions, though TTs expose the patient to the risk of infectious complications. The controversy regarding antibiotic prophylaxis at the time of TT placement remains unresolved. This multicenter study sought to reconcile divergent evidence regarding the effectiveness of antibiotics given as prophylaxis with TT placement. METHODS: The primary outcome mea…
