Diffuse non-specific aortitis with multiple saccular aneurysms and aorto-enteric fistula.

A rare variant of non-specific aortitis was found at necropsy in a young hypertensive woman. The aorta showed severe, extensive, non-specific aortitis with multiple saccular aneurysms containing thrombi throughout its length except for 2 cm from its bifurcation. A fistulous tract was identified leading from an aortic aneurysm at the level of the renal arteries to the second part of the duodenum. Both renal artery ostia were narrowed. The small intestine contained 2 litres of blood, at necropsy. Bilateral ischaemic atrophy of the kidneys was present.

Action of ethanol on gastrin release in the dog.

In conscious dogs with gastric and duodenal Thomas fistulas, we studied the effect of ethanol on plasma concentrations of gastrin. Ethanol was given either as an infusion into a peripheral vein (1.95 M, 200 ml/hr) or into the duodenum (0.95 M, 400 ml/hr) or as an intragastric bolus injection. The effect of the intragastric bolus injection of 200 ml of different concentrations (0.3 M, 1.7 M, 6.85 M) of ethanol was compared with that of equimolar solutions of urea and sucrose (0.3 M, 1.56 M), and with that of sodium taurocholate (0.06 M) and distilled water. The gastrin responses to an oral mixe

Conservative surgery for vesicocolic fistula.

Vesicocolic fistula due to diverticular disease of the colon is conventionally treated by resection of the diseased bowel and closure of the vesical defect. Three cases are described in which separation of the viscera and simple suture of the vesical and colonic defects was carried out. A greater omental graft was interposed between the rectum and bladder. Two of the patients are well 12 and 26 months postoperatively. The third patient died a year postoperatively from a complication of her diverticular disease.

Percutaneous management of abdominal abscesses with GI tract fistulous communication: report of 2 cases.

Percutaneous drainage of abdominal abscesses is an alternative to surgical treatment. Two cases of abscesses with fistulous communication to the GI tract, percutaneously drained successfully, are presented. Reevaluation of these and other relative contraindications of this technique are reviewed.

Ruptured abdominal aortic aneurysm associated with an aorto-enteric fistula. A case report.

A patient with a ruptured abdominal aortic aneurysm and a primary aortoduodenal fistula is reported on. Treatment included aneurysmectomy, suture of the duodenal fistula, closure of the proximal and distal abdominal aortic stumps, and construction of an axillobifemoral bypass. The postoperative course was complicated by spinal cord ischaemia and pataplegia, Gram-negative septicaemia, secondary intra-abdominal haemorrhage and multiple abscess formation.

Spontaneous biliary enteric fistulas.

Biliary enteric fistulas usually occur as a complication of chronic cholelithiasis, may be difficult to document preoperatively, and often pose problems in surgical management. We reviewed 13 cases of spontaneous biliary enteric fistulas to identify methods of diagnosis, management, and complications. There was no specific set of clinical symptoms or signs that led to the diagnosis of a fistula. In six cases the diagnosis was made preoperatively by x-ray films showing pneumobilia, gallstone in the small bowel, or fistula. Initial surgical treatment included cholecystectomy in six cases, small

Positive blood culture as an aid in the diagnosis of secondary aortoenteric fistula.

The successful management of aortoenteric fistula (AEF) requires early diagnosis. To evaluate the accuracy of our diagnostic approach, the hospital course of seven patients with AEF was reviewed. In six patients, the initial bleeding episodes were of the low volume type characterized by hematemesis and melena or by melena alone. All patients were febrile. In six patients, blood cultures obtained preoperatively were positive for enteric organisms identical to those found in cultures obtained intraoperatively from the AEF site. Roentgenographic examination of the upper gastrointestinal (GI) trac

Endoscopic choledochoduodenostomy for choledocholithiasis through choledochoduodenal fistula.

A new endoscopic method for treatment of choledocholithiasis in patients with choledochoduodenal fistula is described. In 2 patients with common bile duct stones a choledochoduodenostomy was performed by means of a diathermy snare. In both patients the calculi passed into the duodenum within some days. No complications occurred. It is concluded that the described technique may be helpful when surgery is contraindicated and when endoscopic papillotomy is difficult to perform.

Double pylorus: a complication of chronic gastric ulcer?

A case of double pylorus with a chronic ulcer in one of the two channels is described. The patient, a middle-aged man with active rheumatoid arthritis, required partial gastrectomy to allow continued treatment of the arthritis with anti-inflammatory drugs. Detailed histological examination of the surgical specimen revealed features consistent with intramural penetration of an ulcer across the pyloric ring, resulting in a gastro-duodenal fistula. The findings provide further support for the hypothesis that the double pylorus is an acquired lesion, which occurs as an uncommon complication of chr

Appendicovesical fistula in childhood: a rare complication of ruptured appendix.

The diagnosis of appendicovesical fistula is difficult and usually delayed. This is most unfortunate, since surgery is uniformly successful. The case we report reemphasizes the diagnostic value of the rectal examination, intravenous pyelogram, and foiding cystogram in a child with subacute or chronic abdominal pain. Only an awareness of this condition on the part of the attending physician will lead to prompt diagnosis and definitive therapy.
