Huge facial desmoid tumors with neck extension: a case report.

INTRODUCTION: Desmoid tumors are very rare, benign fibrous neoplasms arise from the musculoaponeurotic structures throughout the body. CASE REPORT: The patient was a seven-year old boy with a large mandibular mass growing over a period of six months. His CT-scan showed a large mass, 13 cm in diameter in the cheek area extending to the neck and trachea. Biopsy was compatible with desmoid fibromatosis. He was given neoadjovant treatment with vinblastin and methotrexate. The patient underwent a tracheostomy. Then a complete hemimandibulectomy and submandibular gland excision was performed. Final

A new non-endoscopic intraoral approach for open reduction and internal fixation of subcondylar fractures of the mandible.

Open reduction and internal fixation (ORIF) of subcondylar fractures of the mandible is a widely accepted treatment concept aimed at reconstruction of the vertical height of the mandibular ramus and restoration of occlusion. Here, we describe new instruments designed for ORIF via an intraoral approach using a single inverted y-miniplate. The special design allows sufficient visualisation of the fracture site and enables the establishment of a standardised procedure for the treatment of both dislocated and non-dislocated fractures. Several manoeuvres are described and some clinical cases are pr

Rerouting the internal thoracic pedicle: a novel solution for maxillofacial reconstruction in vessel-depleted situations? A preliminary anatomic study.

PURPOSE: Microsurgical reconstruction in a vessel-depleted neck is a challenge due to the lack of reliable vessels in or nearby the host site. The use of the internal thoracic pedicle (ITP) by rib section or sparring is a limited option due to the small length of the pedicle of some flaps. However, in cardiac surgery, the internal thoracic artery (ITA) is widely used for myocardial revascularization, providing a long and versatile pedicle. We aimed at determining precise anatomical bases for the use of the ITP, approached by sternotomy and rerouted in the neck, as recipient vessels for free-fl

Gingival salivary gland choristoma in a pediatric patient: periodontal reconstruction of a rare oral heterotopia with 4-year follow-up.

BACKGROUND: Choristoma is identified as the presence of histologically normal tissue growing in an atypical location. Gingival salivary gland choristomas are exceptionally rare in the literature and mostly observed as; asymptomatic, smooth surfaced masses without any bone involvement. Its treatment involves excisional biopsy and histopathological examination; however, the surgical reconstruction of the periodontal tissues has not yet been described in the literature. CASE PRESENTATION: A 14-year-old female patient presented with an asymptomatic gingival overgrowth on the mandibular right bucc

Submental island flap for oropharyngeal reconstruction: UK experience of 25 cases.

Since the first description of the submental island flap 24 years ago, advances in techniques have expanded the indications for its use and improved its characteristics to make it a favourable reconstructive option for orofacial oncological defects. We describe our experience (particularly perioperatively) of its use, the complications, and the precautions adopted. We retrospectively reviewed the cases of 25 patients and focused on the operating time, use of tracheostomy, duration of postoperative inpatient stay, oropharyngeal function, and associated morbidities. Eighteen patients had defects

Dental microwear evidence for a dietary shift between two nonmaize-reliant prehistoric human populations from Indiana.

Several recent studies have demonstrated the efficacy of dental microwear analysis (DMA) for dietary reconstruction among nonhuman primates, early hominids, and prehistoric humans. The current study seeks microwear evidence for a paleobotanically suggested change in the types of foods that were consumed by two temporally distinct populations of the North American eastern woodlands. This particular dietary shift between the Late Archaic and the Early/Middle Woodland periods did not include the introduction of maize or any other tropical cultigen. In contrast, most dietary reconstructions from t

Microvascular free flaps in head and neck reconstruction. Report of 200 cases and review of complications.

OBJECTIVE: Microvascular free-tissue transfer to the head and neck has become an accepted method of reconstruction owing to increased success rates and superior aesthetic and functional results. Although the large number of arteries and veins in the neck make free-flap revascularization easier than in other recipient sites of the body, there are also unique problems that pose significant risks to the success of the procedure. We report our experience with 200 microvascular free flaps performed between 1987 and 1992. SETTING: This study was conducted at a tertiary referral center. PATIENTS: T

Management of Invasive Cervical Root Resorption in a Mandibular Canine Using Biodentine as a Restorative Material: A Case Report.

Invasive cervical root resorption (ICR) is the reversible/irreversible loss of tooth structure in the connective tissue attachment zone with unclear etiology. In the present case a history of periodontal surgery was presumed to be the predisposing factor. Early diagnosis and proper treatment may lead to long-term retention of the tooth. The tooth is usually asymptomatic and diagnosis is commonly made as a result of a routine radiographic finding. Cone-beam computed tomography (CBCT) is extremely useful in diagnosis and treatment planning of ICR. The treatment should aim toward complete removal

Double rhomboidal flap for reconstruction of large surgical defect of the labial commissure.

Closure of perioral surgical defects involving the oral commissures is highly challenging. We describe a 69-year-old male patient with a large basal cell carcinoma of the right perioral region, extending to the right oral commissure. This lesion was radically excised, and the resulting surgical defect was closed using a homolateral double opposing rhomboidal flap. The final result was esthetically very satisfactory, with total preservation of lip function. Double opposing rhomboidal flaps are viable surgical options for the reconstruction of surgical defects involving the perioral area and ora

Aneurysmal bone cyst of the mandible--a case report.

Aneurysmal bone cysts originating in the craniofacial region have seldom been reported in Taiwan. A case of aneurysmal bone cyst arising in the right mandible of a 15-year-old Chinese boy was presented with both the roentgenographic and microscopic findings. In view of the high recurrent rate and the large destructive mandibular lesion, segmental resection with immediate bone grafting for reconstruction and a conscientious follow up regimen was employed. The etiology and pathogenesis of this interesting lesion remained unclear but several theories were reviewed and the differential diagnosis o

The isolated mandibular ramus - a hitherto rarely described anomaly of the mandible. Pathogenesis and treatment.

INTRODUCTION: A very famous paper by Sam Pruzansky, published in 1969, was entitled "Not all dwarfed mandibles are alike". This is the topic of this paper: to describe the shape and discuss the possible pathogenesis of an extremely rare congenital dysplasia found in a unilaterally hypoplastic mandible, namely the isolated mandibular ramus. MATERIAL AND METHODS: A unique malformation of the lower jaw was found in more than 75 patients with developmental abnormalities of the mandible diagnosed and treated by the two authors in two different university hospitals over the last 40 years. Finite element analysis of mandibular biomechanical stress under vertical loading in models with angular defects.

INTRODUCTION: Pathological fractures of the mandible, often secondary to intraosseous lesions or post-enucleation bone loss, pose significant structural and clinical concerns. While miniplate fixation is routinely used for reinforcement, its mechanical reliability lacks standardized assessment. This study employs finite element analysis to model stress distribution across various mandibular defect scenarios, quantify the biomechanical performance of miniplates, and evaluate postoperative stability in structurally compromised regions. METHODS: This study involved 3D reconstruction of mandibula
