[Fiberoptic determination of the position of the laryngeal mask].

The laryngeal mask airway (LMA) provides a patent airway when placed 'blindly' into the hypopharynx. At the laryngeal side it is supposed to form a seal surrounding the laryngeal inlet with the epiglottis lying outside the mask aperture. This study is designed to assess the prelaryngeal position of the mask by the fibreoptic technique. METHODS. After approval by the local ethical committee and informed consent, 100 adult patients (ASA groups I and II) undergoing general anaesthesia for extracorporal stroke wave lithotripsy (ESWL, Lithotripter HM 3, Dornier) of the kidney were studied. Anaesthe

Anesthetic management for combined mitral valve replacement and aortic valve repair in a patient with osteogenesis imperfecta.

Osteogenesis imperfecta is a rare disorder of connective tissues and presents multiple challenges, including difficult airway, hyperthermia, coagulopathy and respiratory dysfunction, for anesthesiologists, especially during cardiac surgery. We present anesthetic management of a patient with osteogenesis impertecta during double valve surgery. Dexmedetomidine infusion minimized the risks of malignant hyperthermia. Glidescope and in-line stabilization facilitated endotracheal intubation and protected his oral structures and cervical spine. Transesophageal echocardiography (TEE) diagnosed a flail

Explainable deep-learning models to predict diaphragmatic dysfunction and cognitive stress in ICU patients under mechanical ventilation.

BACKGROUND: Diaphragmatic dysfunction and acute cognitive stress/delirium are serious complications of mechanical ventilation that prolong intensive care unit (ICU) stay and are associated with increased mortality. Although accumulating evidence suggests a potential lung-brain axis linking impaired respiratory muscle function to adverse neurocognitive trajectories, early risk stratification remains challenging because clinically relevant signals are multimodal, temporally dynamic, and often partially observed. We aimed to develop an interpretable multimodal deep learning model to predict diaph

Incidence, Timing and Clinical Significance of Immune-Mediated Myocardial Injury and Myocarditis After Gene Replacement Therapy: A Systematic Review and Meta-Analysis.

BACKGROUND: Although adeno-associated virus (AAV) gene-replacement therapy is a potentially transformative therapy for severe genetic diseases, its cardiac immunotoxicity may challenge broad clinical use. METHODS: Medical Literature Analysis and Retrieval System Online, Embase, and PubMed databases were searched from January 2005 to March 2025. Studies including patients treated with AAV-replacement therapy were deemed eligible. Prespecified items (type of vector, dose, timing, and clinical significance of the adverse event) were extracted by 2 independent observers. Random-effects models wer

Correlation of Biomechanical Variables of Lower Extremity Movement During Functional Tests and Tasks in Youth League Football Players: Cross-Sectional Correlation Study.

BACKGROUND: Football is the most widely played sport globally but carries a high risk of lower limb injuries, particularly among youth athletes. Real-time biomechanical monitoring can play a critical role in injury prevention. However, traditional lab-based systems are often complex and impractical for field use. Recent advances in wearable technology, such as inertial sensors and smart socks, provide more accessible solutions for movement analysis. The DAid smart sock system is a promising tool, but further evidence is needed to support its use in lower extremity functional assessments. OBJE

Differentiating Lung From Chest Wall Mechanics Is Difficult Without Esophageal Manometry in Children With Acute Respiratory Distress Syndrome.

OBJECTIVES: Pediatric acute respiratory distress syndrome (PARDS) guidelines recommend limiting airway plateau pressure (Pplat) to 28 cm H 2 O, allowing for higher limits when chest wall compliance (C CW ) is poor since less of the pressure is transmitted to lung (transpulmonary pressure). Transpulmonary pressure depends on Pplat and the ratio of lung elastance to respiratory system elastance (E L /E RS ). E L /E RS measurement requires esophageal manometry, although it is not routinely available. We sought to determine if routinely available clinical data could reliably predict E L /E RS or c

Impact of Protein and Nutritional Support on the Muscular Status of Critically Ill Patients: A Pilot, Perspective, and Exploratory Study.

BACKGROUND: Malnutrition and muscle weakness are highly prevalent in critically admitted patients. To overcome sarcopenia and muscle weakness, physical activity and neuromuscular electric stimulation have been introduced with limited efficacy. Thus, several anabolic remedies have been introduced. An adequate increase in protein support according to indirect calorimetry and body composition and methyl hydroxybutyrate (HMB) is emerging. Therefore, we wanted to investigate the impact of HMB-enriched whey formula on the nutritional status, muscle weakness, and clinical course of critically ill pat

C-terminal agrin fragment as a biomarker of muscle wasting and weakness: a narrative review.

Ageing is accompanied by an inexorable loss of muscle mass and functionality and represents a major risk factor for numerous diseases such as cancer, diabetes and cardiovascular and pulmonary diseases. This progressive loss of muscle mass and function may also result in the insurgence of a clinical syndrome termed sarcopenia, exacerbated by inactivity and disease. Sarcopenia and muscle weakness yield the risk of falls and injuries, heavily impacting on health and social costs. Thus, screening, monitoring and prevention of conditions inducing muscle wasting and weakness are essential to improve

An 8-Week Neuromuscular Training Program After Concussion Reduces 1-Year Subsequent Injury Risk: A Randomized Clinical Trial.

BACKGROUND: Existing data suggest that after concussion, athletes experience an increased risk of subsequent injury. Exploring methods that may reduce injury risk after successful postconcussion return to play may lead to new treatment approaches. PURPOSE: To examine the efficacy of a neuromuscular training (NMT) intervention on acute sports-related time-loss injury over the subsequent year relative to standard of care. STUDY DESIGN: Randomized clinical trial; Level of evidence, 1. METHODS: A total of 27 youth athletes were assessed initially postconcussion (median, 7 days postconcussion; i

Ubiquitin-Synaptobrevin Fusion Protein Causes Degeneration of Presynaptic Motor Terminals in Mice.

UNLABELLED: Protein aggregates containing ubiquitin (Ub) are commonly observed in neurodegenerative disorders, implicating the involvement of the ubiquitin proteasome system (UPS) in their pathogenesis. Here, we aimed to generate a mouse model for monitoring UPS function using a green fluorescent protein (GFP)-based substrate that carries a "noncleavable" N-terminal ubiquitin moiety (Ub(G76V)). We engineered transgenic mice expressing a fusion protein, consisting of the following: (1) Ub(G76V), GFP, and a synaptic vesicle protein synaptobrevin-2 (Ub(G76V)-GFP-Syb2); (2) GFP-Syb2; or (3) Ub(G76

Added effects of dual-task water-based exercise on global cognition and physical performance in older women: a three-arm randomized controlled trial.

INTRODUCTION: Dual-task water-based exercise has been proposed to enhance attention and executive functions and yield cognitive benefits beyond conventional aquatic aerobics exercise. However, evidence remains limited and heterogeneous, as few randomized controlled trials have directly compared dual-task aquatic interventions with conventional aquatic program of equivalent dose in older women. METHODS: This three-arm, parallel-group, superiority randomized controlled trial was conducted in Porto Alegre, Brazil, at ESEFID/UFRGS. Older women were allocated to: dual-task water-based exercise (WE

Mapping the longitudinal trajectories of postural control and functional activity after stroke using clinical and sensor-based assessments: protocol of the FOSTER prospective cohort study.

INTRODUCTION: Stroke is a leading cause of long-term disability, yet recovery trajectories are highly heterogeneous and difficult to predict at the individual level. Standardised clinical assessments primarily capture task accomplishment under controlled conditions but may overlook changes in movement quality, including the distinction between compensatory strategies and behavioural restitution. Additionally, they provide limited insight into the postural control processes that underlie functional task performance. Recent consensus initiatives emphasised the need for sensitive, functionally re

Evaluation of Different Controllers for Sensing-Based Movement Intention Estimation and Safe Tracking in a Simulated LSTM Network-Based Elbow Exoskeleton Robot.

Control of elbow exoskeletons using muscular signals, although promising for the rehabilitation of millions of patients, has not yet been widely commercialized due to challenges in real-time intention estimation and management of dynamic uncertainties. From a practical perspective, millions of patients with stroke, spinal cord injury, or neuromuscular disorders annually require active rehabilitation, and elbow exoskeletons with precise and safe motion intention tracking capabilities can restore functional independence, reduce muscle atrophy, and lower treatment costs. In this research, an inte

Hypomagnesemia and Acute Cognitive Decline in Older Adults: An Evaluation of Clinical Practice and Cognitive Outcomes at a National Health Service (NHS) Trust in England.

Introduction Magnesium is essential for regulating cardiovascular, neuromuscular, and respiratory functions. Hypomagnesemia in older adults is often overlooked and insufficiently managed. Inadequate monitoring and correction of hypomagnesemia may leave old and frail patients more vulnerable to acute cognitive decline, which in some cases can be preventable. This study assessed the current management of hypomagnesemia in older adults admitted to the geriatric wards of an NHS Trust and its association with acute cognitive decline, which is defined in this study as any sudden deterioration from b

Self-reported physical activity and risk of primary knee replacement for osteoarthritis: a competing risks cohort analysis.

BACKGROUND: Whether baseline self‑reported physical activity (PA) is associated with the risk of primary knee replacement (KR) for osteoarthritis (OA) remains uncertain, particularly across radiographic severity. METHODS: In 2,534 Osteoarthritis Initiative (OAI) participants with radiographic knee OA (RKOA; Kellgren and Lawrence (KL) ≥ 2) who completed the Physical Activity Scale for the Elderly (PASE) at baseline, we examined the association between PASE tertiles (low, moderate, high) and OA‑related primary KR over 108 months. We modeled OA‑KR as the event of interest and non‑OA primary KR a

Quantitative Muscle MRI in Patients with Neuromuscular Diseases-Association of Muscle Proton Density Fat Fraction with Semi-Quantitative Grading of Fatty Infiltration and Muscle Strength at the Thigh Region.

(1) Background and Purpose: The skeletal muscles of patients suffering from neuromuscular diseases (NMD) are affected by atrophy, hypertrophy, fatty infiltration, and edematous changes. Magnetic resonance imaging (MRI) is an important tool for diagnosis and monitoring. Concerning fatty infiltration, T1-weighted or T2-weighted DIXON turbo spin echo (TSE) sequences enable a qualitative assessment of muscle involvement. To achieve higher comparability, semi-quantitative grading scales, such as the four-point Mercuri scale, are commonly applied. However, the evaluation remains investigator-depende

Intersite reliability of vertebral bone marrow lipidomics-derived lipid composition among children with varying degrees of bone fragility undergoing routine orthopedic surgery.

BACKGROUND: Lipidomics, a branch of metabolomics, is an attractive technique to characterize bone marrow lipid composition, which may be associated with skeletal acquisition and homeostasis. However, the reliability of lipidomics-derived lipid composition of the bone marrow is unknown, especially for pediatric populations with bone fragility. The purpose of this study was to evaluate the intersite reliability and standard error of measurement (SEM) of vertebral bone marrow lipid composition at the thoracic (T11/T12) and lumbar (L1/L2) spine determined by targeted lipidomics among children with

Cross-sectional and longitudinal assessment of the upper cervical spinal cord in motor neuron disease.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a progressive neuromuscular disease characterized by both upper and lower motor neuron degeneration. While neuroimaging studies of the brain can detect upper motor neuron degeneration, these brain MRI scans also include the upper part of the cervical spinal cord, which offers the possibility to expand the focus also towards lower motor neuron degeneration. Here, we set out to investigate cross-sectional and longitudinal disease effects in the upper cervical spinal cord in patients with ALS, progressive muscular atrophy (PMA: primarily lower mo
