Mechanical properties of extrinsic foot muscles, Achilles tendon, and plantar fascia in patients with a history of diabetic foot ulcers.

BACKGROUND: Diabetic foot ulcers (DFU) are a major complication of diabetes, often leading to impaired mobility and increased risk of recurrence due to persistent biomechanical alterations. Understanding the mechanical properties of foot muscles, tendons, and fascia may provide insight into ulcer development, prevention and rehabilitation strategies. This study aimed to assess the biomechanical properties of the extrinsic foot muscles, Achilles tendon (AT), and plantar fascia (PF) in individuals with a history of DFU using myotonometry. METHODS: A total of 38 diabetic feet with a history of D

Therapeutic Management of the Posttraumatic Stiff Elbow After Open Osteocapsular Release.

Arthrofibrosis is an inevitable consequence of elbow trauma that oftentimes requires surgical release to restore range of motion and function. Although the surgical procedure for elbow osteocapsular release can result in marked improvement in elbow motion, postrelease rehabilitation is paramount to maintaining and even improving the gains achieved in surgery. There are a variety of rehabilitation protocols and modalities that can be implemented that can be divided into an early versus late stage of rehabilitation after surgery. Irrespectively, patient ownership in the rehabilitative process an

Two case reports-Use of relative motion orthoses to manage extensor tendon zones III and IV and sagittal band injuries in adjacent fingers.

STUDY DESIGN: Case report. INTRODUCTION: Injuries to adjacent fingers with differing extensor tendon (ET) zones and/or sagittal band pose a challenge to therapists as no treatment guidelines exist. PURPOSE OF THE STUDY: This report highlights how the relative motion flexion/extension (RMF/RME) concepts were combined into one orthosis to manage a zone IV ET repair (RME) and a zone III central slip repair (RMF) in adjacent fingers (Case 1); and how a single RME orthosis was adapted to limit proximal interphalangeal joint motion to manage multi-level ET zone III-IV injuries and a sagittal band

The effects of prosthetic foot stiffness on transtibial amputee walking mechanics and balance control during turning.

BACKGROUND: Little evidence exists regarding how prosthesis design characteristics affect performance in tasks that challenge mediolateral balance such as turning. This study assesses the influence of prosthetic foot stiffness on amputee walking mechanics and balance control during a continuous turning task. METHODS: Three-dimensional kinematic and kinetic data were collected from eight unilateral transtibial amputees as they walked overground at self-selected speed clockwise and counterclockwise around a 1-meter circle and along a straight line. Subjects performed the walking tasks wearing t

Use of a footrest to reduce low back discomfort development due to prolonged standing.

Prolonged standing is common in many occupations and has been associated with low back discomfort (LBD). No recent studies have investigated a footrest as an intervention to reduce LBD associated with prolonged standing. This study investigated the effect of a footrest on LBD and sought to determine if LBD changes were accompanied by changes in muscle fatigue and low back end-range posture and movement. Twenty participants stood for two 2-h trials, one with and one without a footrest. LBD, lumbar erector spinae electromyography, upper lumbar (UL) and lower lumbar (LL) angles were measured. A s

Short-term cortical plasticity associated with feedback-error learning after locomotor training in a patient with incomplete spinal cord injury.

BACKGROUND AND PURPOSE: For rehabilitation strategies to be effective, training should be based on principles of motor learning, such as feedback-error learning, that facilitate adaptive processes in the nervous system by inducing errors and recalibration of sensory and motor systems. This case report suggests that locomotor resistance training can enhance somatosensory and corticospinal excitability and modulate resting-state brain functional connectivity in a patient with motor-incomplete spinal cord injury (SCI). CASE DESCRIPTION: The short-term cortical plasticity of a 31-year-old man who

Effects of the Dual AFO on gait parameters in stroke patients.

This study investigated how to improve the walking patterns in stroke patients by using the Dual AFO, which was devised as an alternative AFO consisting of two thin pierced plastic pieces for the lower leg and foot. Fifteen individuals with hemiplegia were recruited. Participants were seven males and eight females. Gait parameters were measured with the GAITRite system to analyze the spatial and temporal parameters of walking by stroke patients. The Dual AFO significantly improved the step time of the non-affected foot compared with barefoot walking. There was no significance difference in the

Visualisation to enhance biomechanical tuning of ankle-foot orthoses (AFOs) in stroke: study protocol for a randomised controlled trial.

BACKGROUND: There are a number of gaps in the evidence base for the use of ankle-foot orthoses for stroke patients. Three dimensional motion analysis offers an ideal method for objectively obtaining biomechanical gait data from stroke patients, however there are a number of major barriers to its use in routine clinical practice. One significant problem is the way in which the biomechanical data generated by these systems is presented. Through the careful design of bespoke biomechanical visualisation software it may be possible to present such data in novel ways to improve clinical decision mak

Gait evaluation of a novel hip constraint orthosis with implication for walking in paraplegia.

The aim of this study was to determine the effects of a newly developed reciprocal gait orthosis (RGO) with a variable constraint hip mechanism (VCHM) on the kinematics and kinetics of normal gait. The VCHM was compared with the isocentric reciprocating gait orthosis (IRGO) for walking after paraplegia. Both the VCHM and the IRGO were evaluated with able-bodied volunteers with the hip reciprocating mechanisms coupled and uncoupled. The VCHM was further evaluated with context-dependent coupling based on a finite-state control algorithm utilizing information from brace-mounted sensors. Walking p

[Aims of hand therapy in treatment of rheumatoid hand].

When dealing with rheumatoid arthritis, the main issues of hand therapy are the treatment of hand and finger joints. We emphasize mobilisation of joints, muscle strengthening, correcting deformities by low temperature splints and joint protection. Further tasks of occupational therapy are: Informing patients about adaptive devices and training their use as well as educational programs for patients with reduced mobility of the upper limb. The most common surgery on the rheumatoid hand, that need postoperative treatment by a hand therapist are complete and limited arthrodesis of the wrist, surgi
