Ocular bobbing and myoclonus in central pontine myelinolysis.

Ocular bobbing and myoclonus were unusual findings in a patient with central pontine myelinolysis. Pathological findings confirmed the diagnosis of pontine and extrapontine myelinolysis.

Pontine hematomas.

Pontine hematomas are very rare lesions that in the past were found at postmortem examination rather than diagnosed from the clinical presentation. The advent of computed tomography allowed more frequent identification of those lesions which has shown a more favorable prognosis than formerly thought. Three patients suffering from intrapontine hematomas, revealed by computed tomography, are discussed. The etiopathogenesis, location, and clinical features of this lesion are discussed in an attempt to establish general criteria from adequate management. All 3 patients (2 treated conservatively an

Demyelination of the lateral geniculate nucleus in central pontine myelinolysis.

Postmortem examination of the central nervous system in 6 of 9 patients with central pontine myelinolysis (CPM) revealed demyelination in the lateral geniculate nucleus (LGN). Histologically, the geniculate lesions consisted of myelin swelling and disintegration, largely in the central part of the nucleus, with preservation of axons and neuronal perikarya. The pathological changes resembled those found in the basis pontis in CPM.

[Pontine hemorrhage in a 12 year old boy. Demonstration by cranial computer tomography (author's transl)].

The diagnosis of pontine hemorrhage was made by computerized axial tomography in a 12 year old boy, who presented with acute central hemiparesis and contralateral abducens paresis.

Pontine infarction manifesting as isolated cranial nerve palsies.

Isolated ipsilateral fifth, seventh, tenth and twelfth cranial nerve palsies in a 74-year-old woman were shown at autopsy to result from an inferior lateral pontine infarction. The clinical features and pathogenesis of this uncommon lesion are discussed.

Intracranial dissecting aneurysm. Report of a case.

The case of a 47-year-old man who died one month after a history of paroxysmal occipital headaches, vertigo, vomiting, weakness, and sweating is presented. The death was due to a pontine softening caused by a subintimal dissecting aneurysm of the two vertebral, the basilar and the right posterior inferior cerebellar arteries. No etiological factor of the illness could be found. The clinical signs resembled those of a flap-valve tumor of the IIIrd ventricle.

Identification of cells of origin of tectopontine fibers in the cat superior colliculus: an experimental study with the horseradish peroxidase method.

The pontine projections from the superior colliculus in the cat have been studied by means of retrograde axonal transport of horseradish peroxidase (HRP). Following injections of HRP in the dorsolateral pontine nucleus, where the tectopontine fibers terminate, a fair number of labeled cells are found throughout the rostrocaudal extent of the ipsilateral superior colliculus. Relatively few of the labeled cells are of medium size (25-40 micron in diameter), more than 80% are small (10-25 micron), but no large cells are labeled. The cell bodies giving rise to tectopontine fibers are distributed i

The diagnosis of hypertensive intracerebral hemorrhage: the contribution of computed tomography.

Sixty-eight cases of hypertensive intracerebral hemorrhage were reviewed and the CT scans studied. The CT scan was accurate in localizing and measuring intracerebral hemorrhage. There were three cases of pontine hemorrhage which did not appear on the CT scan. Hemorrhages at other sites were always seen on the CT scan. The size of putaminal and thalamic hemorrhage as calculated from CT scan allowed accurate prognosis to be made at the onset. CT scanning has become the diagnostic test or first choice when intracerebral hemorrhage is suspected.

Neuro-ophthalmic complications following eighth cranial nerve microvascularx decompression.

PURPOSE: To discuss a novel diagnosis of post-surgical iatrogenic AICA vasospasm following eighth cranial nerve microvascular decompression as the most likely etiology for a lateral pontine infarct. OBSERVATIONS: A 49-year-old male presented with tinnitus, vertigo, binocular and monocular diplopia following left occipital decompressive craniectomy and microvascular decompression of cranial nerve VIII. Magnetic resonance imaging demonstrated patent vasculature with evolution of a subacute infarct of the left lateral pons and middle cerebellar peduncle in the territory of the left AICA. CONCLU…

Central Pontine Myelinolysis: A Rare and Life-Threatening Adverse Effect of Clobazam and Quetiapine.

Central pontine myelinolysis (CPM) is a rare neurological disorder characterized by demyelination within the central portion of the pons. While hyponatremia is a well-known precipitating factor, other etiologies, including medication use, have been reported. We present a case of a 69-year-old male with a history of obsessive-compulsive disorder, stroke, and type 2 diabetes mellitus who developed confusion, altered sensorium, and weakness in all four limbs. An MRI brain imaging revealed characteristic findings suggestive of CPM. Despite normal serum sodium levels, discontinuation of clobazam an…

Metronidazole-induced encephalopathy in a patient with cirrhosis.

Metronidazole is a commonly used antibiotic with anaerobic bacterial, protozoal, and microaerophilic bacterial coverage. Encephalopathy and peripheral neurotoxicity are rare but known adverse events with prolonged metronidazole use, which can be difficult to distinguish from other causes of delirium in acutely ill patients. Definitive diagnosis can be made by brain magnetic resonance imaging (MRI), which often reveals symmetric bilateral hypersignal demyelination lesions typically involving the dentate nuclei, splenium of the corpus collosum, midbrain, dorsal medulla, and pons. This case repor…

Homozygous EXOSC3 c.395A>C Variants in Pontocerebellar Hypoplasia Type 1B: A Sibling Pair With Childhood Lethal Presentation and Literature Review.

Pontocerebellar hypoplasia type 1B (PCH1B) is an autosomal recessive neurodegenerative disorder that involves hypoplasia or atrophy of the cerebellum and pons. PCH1B is caused by mutations in EXOSC3, which encodes a subunit of the RNA exosome complex. The most frequently observed mutation in PCH1B patients is a c.395A>C (p.D132A) missense variant, for which the homozygous mutation typically results in milder symptoms compared to compound heterozygous mutations or homozygous mutations for other pathogenic variants. In the present study, we report on a sibling pair harboring homozygous EXOSC3 c.…

Central Pontine Myelinolysis and Hypokalemic Paralysis as Presenting Manifestations of Sjogren's Syndrome.

Neurological involvement in Sjogren's syndrome can have varied manifestations and can precede the classical sicca symptoms of Sjogren's syndrome. A 32-year-old woman presented with acute quadriparesis and dysarthria. She had severe hypokalemia, and an MRI of the brain showed a lesion in the central pons that was hyperintense on T2 and fluid-attenuated inversion recovery (FLAIR) sequences sparing the periphery, a trident appearance characteristic of central pontine myelinolysis (CPM). On further evaluation, she was found to have distal renal tubular acidosis (dRTA) due to primary Sjogren's synd…

Esophageal carcinoma as a portal of entry for brainstem abscess formation.

Cerebral abscess formation is a serious and life-threatening clinical entity, secondary to contiguous spread, hematogenous dissemination or direct inoculation. We present the case of a 61-year-old woman with a recent diagnosis of a locally advanced squamous cell carcinoma of the esophagus who was diagnosed with a brainstem abscess. In literature we only found three cases reporting cerebral abscess formation in patients with esophageal carcinoma. Our case report is considered exceptional given the abscess localization in the pons. The abscess was successfully treated with stereotactic drainage …
