Bone tissue is influenced by its mechanical environment and adapts in response to its mechanical load. This is supported by studies analyzing bone adaptation in the knee and hip. Changes to the bone have also been found to precede cartilage degeneration in diseases such as osteoarthritis (OA). Our objective was to demonstrate the relationship between joint contact and bone density in the wrists of healthy adults. Static CT scans with a calibration phantom were taken to obtain measures of bone mineral density (vBMD) in 3 normalized depths; 0 - 2.5, 2.5 - 5, and 5 - 7.5 mm. Participants underwen

BACKGROUND: We present the case of a 22 y.o. male patient suffering from scaphoid non-union with avascular necrosis of the proximal pole and initial degenerative arthritis.
CASE REPORT: He referred to our institution with functional impairment and persistent pain (VAS 8\10). The patient underwent the positioning of the small size Adaptive Proximal Scaphoid Implant (APSI), without fixation, through an open dorsal approach and radial styloidectomy. The post-operative course was uneventful, and the patient could resume his daily routine without limitations. 5 years later the patient returned to

BACKGROUND: Scaphoid nonunion is very rare in childhood.
CASE REPORT: A 14 year old boy with scaphoid nonunion confirmed with CT did not past scheduled surgery. In spite of the short period of immobilisation union occurred and two years after the injury he was completely asymptomatic.
CONCLUSIONS: We suspect that injury to the scaphoid, in this case may have accelerated the union shortly before the end of ossification.

Magnetic resonance imaging (MRI) has emerged as the premier noninvasive imaging method for evaluation of the musculoskeletal system. Among the most widely recognized and accepted applications of MRI is that of trauma-related imaging. While the initial emphasis on MRI was directed toward the assessment of soft-tissue structures previously difficult to image (eg, knee menisci and cruciate ligaments), it has become increasingly apparent that MRI is superbly suited to the depiction of a wide spectrum of injuries to bone and articular cartilage. MRI has proved capable of depicting abnormalities ran

BACKGROUND Trapezoid fractures are very uncommon, accounting for less than 1% of all wrist fractures. Isolated fractures of this bone are even more rare, with just a few reports in the literature. The trapezoid bone has a very stable position within the wrist, forming a relatively immobile joint with the second metacarpal base distally. It is also connected by very strong ligaments to the trapezium, capitate and, scaphoid. Trapezoid fractures occur when a strong bending or axial force is applied to the second metacarpal base. CASE REPORT We present a clinical case of an isolated, non-displaced

In this article, a new modification technique is described to reconstruct large bone defects of the distal radius after en bloc giant cell tumor resection with a proximal vascularized fibula graft. We discuss anatomic landmarks during graft harvest as well as reconstructive considerations. We aim for a durable reconstruction with optimal mobility and, therefore, introduce a fibular-scaphoid-lunate fusion with a reconstruction of the distal fibula-ulnar joint with an "Adams-Berger-like" procedure with a palmaris longus tendon. The clinical results of 3 patients who underwent this procedure were

Scaphocaptiate fracture syndrome is a unique condition and is challenging to manage. This rare fracture develops after high-energy wrist trauma. We reported a patient with a history of falling on an outstretched hand. A 15-year-old boy had scaphocapitate fracture syndrome, which included a displaced fracture of the capitate, an avulsion fracture at the distal tubercle of the scaphoid bone with an extension to the articular surface, and a perilunate fracture. A few months after open reduction and internal fixation were performed, the patient had a satisfying outcome. In addition, we have conduc…

Seven patients with nonunion of the scaphoid were treated by a limited approach combining a palmar wedge graft with insertion of a dorsal (retrograde) Herbert screw through small incisions. All patients had palmar bone deficiency and a 'difficult' proximal fragment. They were followed up for a minimum of 12 months (12 to 38). Radiological union was achieved in all. In five patients correction of instability of the dorsal intercalary segment which was present before operation, was achieved. On a wrist-scoring chart, all patients had an excellent or good rating. The limited combined approach all

Over a period of 24 years, the author has used five different methods of bone-grafting for ununited scaphoid fractures. The clinical and radiological results have been reviewed, with a minimum follow-up of 1 year. Radiologically the best results (78% definite union) were obtained with a "wedge" graft and Herbert screw, while the worst results followed the original Russe operation. The clinical result often did not coincide with the radiological outcome. All methods led to a decrease in pain in most cases, but little or no pain was achieved most often by the modified Russe graft. With proximal

Several possible lesions of ligaments may be diagnosed using dynamic radiographs in radial or ulnar deviation, with stressing in torsion, traction, or radial, ulnar, anterior or posterior displacement. The initial position is strictly defined in the front and sagittal planes. The shapes of the bones, their relationship and the joint lines are observed. The authors have made a complete study of the normal wrist and describe some pathological conditions including dislocations, disjunctions between the scaphoid and lunate, mid-carpal sprains and radio-carpal sprains. Descriptions are given as to

Twelve patients were studied following proximal-row carpectomy. Four had a follow-up of twenty years or more. The lesions for which the operation was done included Kienböck's disease, perilunar dislocations, and fractures of the carpal scaphoid. The major complaint of pain was relieved following surgery, and a functional range of motion was obtained. Mild degenerative arthritis of the wrist was not a contraindication and progressive degenerative arthritis of the radial capitate articulation did not occur. The operation did not result in weakness and all of the patients resumed their preoperati

Current multifactorial age estimation methods are based on radiography, however, in the forensic field there is growing interest in using magnetic resonance imaging (MRI). With regard to the carpal region, MRI provides more information for defining the individual ossification nuclei and the cartilage surrounding single bones. During the phase of bone growth, the progressive reduction of the cartilage layer is accompanied by the development of a cartilage-bone interface. The aim of our study was to create a new model for age estimation, based on the ratio between the area occupied by the nucleu

Achieving union using conventional grafts has a high chance of failure in patients with recalcitrant non-union (persistent pseudarthrosis) of the scaphoid bone, an avascular proximal fragment and previous failed surgeries because of poor host bed vascularity. Eleven patients with long-standing non-union were treated with vascularized pedicle bone grafting and supplementary corticocancellous grafting. Five had screw fixation and six were fixed with K-wires. The average age of the patients was 28 years, average duration of the non-union was 39 months and mean radiological follow-up was 32 months…

PURPOSE: Although proximal row carpectomy (PRC) has increasingly been shown to have superior features to four-corner fusion (4CF), individual surgeons may remain convinced of the superiority of one procedure based on personal experience and individual biases. Hence, we sought to perform an updated meta-analysis with some of the largest studies to date to compare outcomes and complications between these procedures in the treatment of scapholunate advanced collapse and scaphoid nonunion advanced collapse wrists.
METHODS: A systematic review and meta-analysis was performed per Preferred Reportin

PURPOSE: The radiographic interfacet angle (IFA), scaphoid facet inclination (SFI), and lunate facet inclination (LFI) of the distal radius were measured in patients with 3 distinct wrist pathologies to determine whether there is an association between these radiographic measurements and these conditions.
METHODS: Posteroanterior wrist radiographs were compiled from patients with 3 types of common wrist pathologies (scaphoid waist fracture [n = 54], scapholunate [SL] dissociation [n = 23], and dorsal ganglion [n = 51]). The patients were all Caucasians aged 20 to 45 years who met strict radio
