[Suprahyoid pharyngotomy for surgical therapy of malignant and benign oral and hypopharyngeal tumors].

Many cases of oropharyngeal and hypopharyngeal neoplasms without diffuse infiltration of the larynx or mandible cannot be treated effectivel … rahyoid pharyngotomy was performed in combination with a unilateral or bilateral neck dissection. A tracheotomy was required in six patients. Three patients underwent postoperative radiation therapy. No locoregional recurrences were found in these patients, with a median follow-up of 20.8 months.

[Purulent neuroinfections in the course of septicemia in the years 1988-1992: personal experience].

Between 1988 and 1992 thirty patients with purulent infections of central nervous system in the course of bacterial septicaemia were treated in the Intensive Care Unit of the I Clinic of Infectious Diseases of Silesian School of Medicine in Bytom. These cases made about 32% of the total bacterial neuroinfections treated in the Clinic during that time. Imminent or existing acute respiratory insufficiency, increasing intracranial pressure, intensification of haemorrhagic diathesis and imminent septic shock were the indications to the hospitalization in the Intensive Care Unit. Neisseria meningit

[Surgical treatment of children with cicatricial stenoses of the larynx and trachea].

The present authors recommend a modification of Ivanov's method suggested by their treatment of 41 children, aged 4 to 15 years; 12 patients suffered from stenosis and 29 patients from atresia of the larynx and trachea, which resulted from surgical intervention required by the complications of extended nasotracheal intubation and subsequent tracheotomy. The modified procedure is as follows. In order to enlarge the laryngeal or tracheal lumen, skin is cut and one or two triangle-shaped skin flaps are dissected; their tops are sutured to the anterior wall of the above organs cut along the median…

Vocal cord paralysis resulting from neck injections in the intravenous drug use population.

Intravenous drug use patients present to the head and neck surgeon when injections are directed "in the pocket," or more appropriately, toward the internal jugular vein in the neck. The more common complications of this practice include the development of cellulitis, abscess, and venous thrombophlebitis and, potentially, pulmonary embolism and pseudoaneurysm of the carotid and subclavian arteries. Vocal cord paralysis as a result of neck injection in the intravenous drug-using population is rarely described, and a review of the literature has yielded only two reports addressing this uncommon p

The history of pediatric surgery in France.

In 1741, Nicolas Andry, counsellor of King Louis XV, published a book about "orthopedics," inventing this word. The book is interesting as the author refers to beliefs and habits of the time. In 1864, Guersant published Notes About Pediatric Surgery, a real textbook which was translated into English and German and dealt with the importance of children's psychological training, anesthesia, and water or mother's milk after the operation, and also described tracheotomy, draining of cervical adenitis, and lithotrity. The classification of bone affections was still very confused. Tuberculosis and s…

Etiology of stridor in the neonate, infant and child.

Of 219 patients whose primary presenting symptom was stridor, 150 were males, 69 were females. All were under 2 1/2 years of age; more than … according to the part of the tracheobronchial tree which was involved. Twenty-six patients required tracheotomy. Fifty-eight (26.5%) were referred with an erroneous presumptive diagnosis for which they were being treated. The mean length of time from onset of symptoms to determination of the correct diagnosis by endoscopy was four months; it varied from one d …

Utility of sleep nasendoscopy versus microlaryngotracheobronchoscopy in the diagnosis of paediatric upper airway obstruction.

OBJECTIVE: To describe the utility of sleep nasendoscopy in determining the level of upper airway obstruction compared to microlaryngotracheobronchoscopy. METHODS: A retrospective observational study was conducted at a tertiary level paediatric hospital. Patients clinically diagnosed with upper airway obstruction warranting surgical intervention (i.e. with obstructive sleep apnoea or laryngomalacia) were included. These patients underwent sleep nasendoscopy in the anaesthetic room; microlaryngotracheobronchoscopy was subsequently performed and findings were compared. RESULTS: Twenty-seven pa

Impending Rupture of Internal Carotid Artery Aneurysm Mimicking Peritonsillar Abscess.

Extracranial carotid artery aneurysms are rare but important entity. Impending rupture of such aneurysms can lead to catastrophic hemorrhage, airway compromise and may prove fatal. The authors report a case of true aneurysm of cervical internal carotid artery in a four-year-old girl who presented with fever and swelling of neck and oropharynx. High clinical suspicion is required to differentiate aneurysm from peritonsillar and parapharyngeal abscess as incision and drainage can prove fatal. Securing airway beforehand and timely embolization has led to the favorable outcome in the present case.

[Right uncontrollable haemothorax revealing a liver injury with diaphragmatic rupture].

We report a case of a woman with a blunt thoracic trauma and haemorrhagic shock after a road traffic accident. The clinical and complementary examinations revealed an isolated right haemothorax, which was compressive and uncontrollable. The source of bleeding was discovered with delay and during a surgical exploration: it was a liver injury with diaphragmatic rupture but without hepatic herniation and peritoneal effusion. The diagnostic features of blunt diaphragmatic rupture are discussed.

[Application of nasal intermittent positive pressure ventilation to a case of limb-girdle muscular dystrophy].

We applied nocturnal ventilation (NV) with nasal intermittent positive pressure ventilation with custom molded mask (NIPPV-C Mclermott, 1989), as well as NV with tracheostomy intermittent positive pressure ventilation (TIPPV) to a male patient with limb-girdle muscular dystrophy who had developed chronic respiratory failure at the age of 47. NV with both methods successfully corrected nocturnal hypoxemia, improved daytime arterial blood gas values, and achieved a stable clinical course without marked deterioration for four years. Daytime PaO2 higher than 60 Torr and PaCO2 lower than 70 Torr wh

[Obstructive sleep apnea syndromes].

With the exception of cases where a lesion creates a clearly identifiable and surgically removable anatomical obstacle on the upper air duct (tumors, cysts, hypertrophic tonsils, vegetations,...), currently the continuous positive air pressure is the treatment of choice. One drawback is that in most cases it is no more than a symptomatic treatment. The most efficient alternative treatment is using extensive maxillofacial surgery. Another promising way is prevention through specific mouth breathing treatment from the earliest age on.

Variable intrathoracic upper airway obstruction due to non-small cell lung cancer. Palliation using physiologic and mechanical stenting.

A 48-year-old woman with non-small cell lung cancer involving the mediastinum and producing extrinsic tracheal compression is presented. The patient failed to respond to all conventional therapy and presented with stridor and respiratory distress due to progressive airway obstruction. Continuous positive airway pressure (CPAP) by mask was used to physiologically stent the airway until a mechanical Silastic stent could be placed by tracheostomy.

Fentanyl-O2-N2O rigidity and pulmonary compliance.

Induction of anesthesia with fentanyl has been associated with such extreme rigidity as to cause failure of bag-and-mask ventilation. The etiology of this failure has not been investigated. Therefore, fentanyl-induced rigidity was studied in 5 control patients to establish the dose at which ventilation became impossible and in 5 patients with tracheostomies to investigate changes in pulmonary compliance. Anesthesia was induced with 3 micrograms/kg/min fentanyl to a total of 30 micrograms/kg. In control patients, failure to maintain bag-and-mask ventilation occurred at a mean fentanyl dose of 1

Opitz (G) syndrome.

Opitz syndrome, or G syndrome, is a composite of congenital malformations including hypertelorism, hypospadias, and recurrent aspiration secondary to a disordered swallowing mechanism. The mode of inheritance is probably autosomal dominant with male predominance. This paper describes an infant with this syndrome who developed progressive airway obstruction requiring several endotracheal intubations. After extubation a laryngeal granuloma developed and required tracheostomy and excision. A hallmark of the patient's life has been persistent aspiration. No vocal cord paralysis or anatomic larynge
