Ventral foramen magnum meningiomas are a forbidding lesion. The stake is so high with a risk of devastating paralysis and respiratory failure. Careful preoperative clinical and radiological evaluation is necessary to implement the best treatment plan. Successful surgical intervention depends on paying high attention to minute details throughout the case, from intratracheal intubation to extubation. The neural head-on-neck position is critical to avoid further medullary compression at intubation and positioning.1 Extensive neurophysiological monitoring, including somatosensory, motor, brainstem

We review the angiographic and CT findings, precipitating factors and clinical features in nine patients with ten intracranial arterial dissections. The internal carotid artery was involved in five cases, the vertebral artery in four and the posterior inferior cerebellar artery in one. Angiography revealed irregular stenosis in four cases, irregular stenosis and a pseudoaneurysm in two, irregular stenosis and irregular dilatation in one, arterial occlusion in two and a pseudoaneurysm in one. CT demonstrated an infarct in four cases, a dense middle cerebral artery in two and subarachnoid haemor

This report describes a rare case of the right-sided aortic arch with the right subclavian artery as the last branch, which was encountered in a Japanese male cadaver in the dissecting room at Kurume University School of Medicine in 1995. In this subject, the ascending aorta arose from the left ventricle and ascended obliquely, curving forward and to the right, and became the right-sided aortic arch. The aortic arch passed upwards, reaching a summit at the level of the third thoracic vertebral body, then curved dorsally. The left brachiocephalic, the right common carotid and the right subclavi

Sixteen patients with vascular lesions of the vertebral and basilar arteries were studied with magnetic resonance imaging. The vascular abnormalities included seven cases of atherosclerotic disease with partial or complete thrombosis, six aneurysms, two cases of vertebrobasilar dolichoectasia, and one basilar artery dissection. Magnetic resonance effectively demonstrated vascular thrombosis with occlusion as high signal intensity on spin echo (SE) sequences with absence of flow void and no flow enhancement on gradient echo (GRE) images. Nonthrombosed aneurysms exhibited mixed signal intensity

STUDY DESIGN: Case report.
OBJECTIVE: To describe a patient presenting with Brown-Séquard-plus syndrome treated in a conservative manner and to discuss the possible physiopathological mechanisms causing the injury.
METHODS: The case study of a 35-year-old woman who entered the hospital with a knife that had penetrated her neck through the left upper thoracic aperture and with a rising, back, right oblique trajectory. This patient developed Brown-Séquard-plus syndrome on the right side of her body.
RESULTS: The initial computerized tomography (CT) demonstrated that the tip of the knife was i

The surgical anatomy of a transtemporal approach to the structures of the clivus was defined with the aid of dissections in 10 cadaver heads. The steps in the dissection consisted of first exposing the cervical internal carotid artery (ICA), the internal jugular vein, and the caudal cranial nerves, each at the skull base; then performing small retromastoid and temporal craniotomies; and, finally, drilling away the petrous and tympanic bone to expose the intratemporal parts of the facial nerve, the petrous ICA, the sigmoid sinus, and the jugular bulb. To expose the structures of the lower clivu

BACKGROUND: Headache or neck pain is a frequent symptom of spontaneous cervical artery dissection (sCAD).
PATIENTS AND METHODS: Patients were drawn from an ongoing hospital-based registry of consecutive cases diagnosed with sCAD. Only patients with isolated pain were included in this series. Pain topography, dynamics, severity and quality, imaging findings and outcome were analysed.
RESULTS: 20 of 245 (8%) patients with sCAD presented with pain as the only symptom (mean (SD) age 39 (8) years; 14 (70%) women). Of them, 12 had vertebral artery dissection, 3 had internal carotid dissection and …

PURPOSE: The small pedicle widths, the thin lateral cortical shell next to the vertebral artery (VA), and the strongly tilted pedicle axis anteromedially narrow the safety margin for cervical pedicle screw (CPS) placement against vertebral artery (VA) injury. We have studied whether the minimally invasive surgery (MIS) of a posterolateral approach with a table-mounted, surgeon-controlled mode of robotic assistance (group R) improves the VA safety compared to C-arm fluoroscopy-guided conventional open technique (group F).
METHODS: Group R consisted of 165 screws in 37 patients and group F, 199

The early diagnosis of cervical vascular trauma is critical in the prevention of cerebral ischemia. Traumatic blunt carotid or vertebral artery dissection is rare and frequently associated with other injuries. Diagnosis is often delayed, limiting treatment options and contributing to a poor outcome. The clinical findings and investigation of 14 patients are presented with special reference to the last three who had magnetic resonance imaging angiography. Treatment options are discussed and it is suggested that with modern methods of investigation and management, the prognosis of this condition…

Extradural tumors of the cervical spine may involve the vertebral artery on one or both sides, posing one of the limiting factors toward the radical resection of such neoplasms. A standard anterior approach may be inadequate for the management of such tumors. An anterolateral approach allows the dissection and mobilization of the vessel, which can then be preserved, resected, or reconstructed with a vein graft. An anterior approach can be supplemented with this for tumor resection and stabilization. This management strategy is described in 10 patients with a variety of tumors.

OBJECTIVE: The posterior meningeal artery (PMA) normally arises from the vertebral artery; however, its origin varies considerably as the result of its embryological development. This gains clinical significance when associated with vascular pathology.
CLINICAL PRESENTATION: A 65-year-old man presented to his local hospital with a sudden-onset, severe headache. Computed tomography of the head revealed diffuse subarachnoid hemorrhage, mostly in the left posterior fossa. A computed tomographic angiogram demonstrated an anomalous origin of the PMA from the posteroinferior cerebellar artery (PICA

OBJECTIVE: To describe unique indications for covered stent grafts in trauma-associated cerebrovascular injuries.
PATIENTS: Between 2006 and 2018, five patients with cerebrovascular injuries were treated with a covered stent graft. We present a retrospective analysis of technique and outcomes.
RESULTS: In all cases stent deployment was successful. Endoleaks occurred in two cases requiring additional transvenous embolization of a carotid cavernous fistula (CCF) in one patient. Two cases of in-stent thrombosis were observed during intervention and 2 days postintervention in a patient with a lo

BACKGROUND: Patients with carotid artery dissection (CAD) have been reported to have different vascular risk factor profiles and clinical outcomes to those with vertebral artery dissection (VAD). However, there are limited data from recent, large international studies comparing risk factors and clinical features in patients with cervical artery dissection (CeAD) with other TIA or ischemic stroke (IS) patients of similar age and sex.
METHODS: We analysed demographic, clinical and risk factor profiles in TIA and IS patients ≤55 years of age with and without CeAD in the large European, multi-cen…

UNLABELLED: Background and purpose Although cervicocranial artery dissections (CADs) are common causes of ischemic stroke in young individuals, anatomical locations and mechanisms of ischemic strokes are still unclear. We evaluate the prevalence, location, and pathogenic mechanisms of ischemic stroke caused by CADs.
METHODS: We reviewed CAD patients who presented with acute (<7days) ischemic events and who had undergone diffusion weighted magnetic resonance imaging (MRI) and appropriate vascular imagings (MR angiography, computed tomography angiography, digital subtraction angiography, and hi

BACKGROUND AND PURPOSE: Hormonal and mechanical factors might increase the risk for cervical artery dissection (CAD) during pregnancy and the puerperium. There is uncertainty how to counsel women with a previous CAD regarding the risk of CAD recurrence during pregnancy and the puerperium.
METHODS: In an observational study of four stroke centers, all women aged 16-45 years with primary CAD in the previous decade were asked to participate in a standardized assessment on long-term follow-up with a special focus on pregnancies and recurrent CAD.
RESULTS: Ninety-two women were identified and 53

BACKGROUND AND OBJECTIVE: Injury to the carotid and vertebral arteries is an identified risk to patients after blunt high-energy cranio-cervical trauma with an associated risk of thromboembolic stroke. We sought to determine the incidence, features, and risk factors of arterial injury using selective cerebral angiography in a high-risk trauma patient subset.
METHODS: Blunt trauma patients with a high-energy mechanism were selected to undergo screening cerebral angiography if they met one of the following criteria: (1) cervical spine hyperextension/hyperflexion injury, (2) skull-base or facial

BACKGROUND: Surgical access to the cervico-thoracic junction in pediatric tumors presents substantial challenges due to complex anatomy and the proximity of critical neurovascular structures. To improve interdisciplinary training and minimize iatrogenic risks, we optimized a stepwise dissection protocol—referred to as the “Trail of Safety”—using formalin-fixed cadavers. METHODS: Sixteen bilateral cervico-thoracic dissections were conducted on eight formalin-fixed adult cadavers by a team of vascular and pediatric surgeons. A structured protocol involving supraclavicular incision, median sterno

A rare case of dissecting aneurysm of distal posterior inferior cerebellar artery (PICA) is reported. A 51-year-old woman was admitted to our hospital complaining of severe headache and nausea. CT scan revealed subarachnoid hemorrhage which was thicker in the posterior fossa. The vertebral angiography demonstrated an aneurysm on the telovelotonsillar segment (Lister's classification) of the left PICA. On the third day, the left suboccipital craniotomy was performed and the fusiform aneurysm was resected. The postoperative course was uneventful. Histological examination of the resected aneurysm
