Auditory brainstem response artefact caused by caloric testing.

In the external ear canal, residual water from caloric testing or any other irrigation may act to simulate a conductive hearing loss and interfere with subsequent auditory brainstem response recording leading to increased latencies and reduced amplitudes. This artefact in the ABR may be mistaken for a retrocochlear lesion.

Clinical significance of biphasic head-shaking nystagmus.

Biphasic head-shaking nystagmus (b-HSN) is a rather uncommon phenomenon which was first reported by Kamei in 1975 (Arch. Otorhinolaryngol. 209: 59-67, 1975). He pointed out that b-HSN appeared in cases of peripheral vestibular disturbance especially in the cases of unilateral disturbance, and that the direction of the second phase indicated the damaged side. While investigating cases of b-HSN, the author encountered some cases which did not correspond to b-HSN reported by Kamei. Nineteen patients in whom b-HSN was recorded by electronystagmography were investigated. These patients visited our

Waardenburg's syndrome: variations in expressivity.

Twenty-seven subjects with a family history of Waardenburg's syndrome were examined with respect to 18 specific characteristics of the syndrome, with particular emphasis in identifying the spectrum of the phenotypic expression of affected persons. Our results indicate that patients with the syndrome may have a variety of clinical signs, and an accurate diagnosis will therefore depend on a thorough and pertinent family history and physical examination.

Electrical vestibular stimulation and space motion sickness.

Electrical vestibular stimulation (EVS) in dynamic balance condition was studied in order to search for a new provocative test of space motion sickness (SMS). SMS is usually attributed to a sensory conflict caused by exposure to microgravity. Vestibular information is conflicting but also unusual and insignificant. EVS is in accordance with this feature because it is not the adequate stimulus of the vestibular receptors. EVS was achieved by means of binaural electrical stimulation. Effects of EVS were potentiated by compelling the subject to maintain dynamic balance on a seesaw. The quantifica

Vestibular evoked myogenic potentials in acoustic tumor patients with normal auditory brainstem responses.

In order to clarify the utility of the vestibular evoked myogenic potential (VEMP) in detecting acoustic tumors, we report two patients who were found to have normal auditory brainstem responses (ABRs) and abnormal VEMPs. To record VEMPs, electromyographic responses to brief loud clicks (0.1 ms at 95 dBnHL) were amplified and averaged on the sternocleidomastoid muscle ipsilateral to the stimulated side. The stimulation rate was 5 Hz and the analysis time 50 ms. The first case was a 54-year-old woman in whom VEMPs were absent on the affected side while caloric tests and ABRs were normal. The se

Caloric canal paresis thresholds in relation to vHIT-defined acute unilateral vestibular dysfunction: a retrospective analysis.

BACKGROUND: Caloric testing and the video head impulse test (vHIT) assess vestibular function at different frequency ranges and provide complementary information in patients with acute vertigo. Although a canal paresis (CP) threshold of 25% is commonly used to define caloric abnormality, interpreting CP values in relation to vHIT-defined unilateral high-frequency vestibular dysfunction remains clinically challenging. OBJECTIVE: We evaluated how conventional and alternative caloric CP thresholds align with vHIT-defined acute unilateral vestibular dysfunction and sought an interpretive CP thres

A comparison of duration of nystagmus as measured by the Southern California Postrotary Nystagmus Test and electronystagmography.

The Southern California Postrotary Nystagmus Test (SCPNT) and electronystagmography are methods used to evaluate one aspect of vestibular function. The SCPNT is based on the examiner's observation of eye movements and is used in combination with other information as part of a diagnostic battery for children with learning disorders. Electronystagmography is the permanent recording of eye movements through the use of surface electrodes. In this study. The SCPNT and an electronystagmograph were used to record postrotary nystagmus in 20 normal females aged 25 to 30. A significant correlation was f

Evaluating the Diagnostic Accuracy of the Side-Lying Test for Posterior Canal BPPV: Sensitivity and Specificity Analysis.

BACKGROUND OBJECTIVE: The side-lying (SL) test is a diagnostic maneuver used to identify posterior canal benign paroxysmal positional vertigo (PC-BPPV). It is often used as an alternative to the Dix-Hallpike (DH) test, particularly for patients who have mobility limitations. The objective of this study is to evaluate the sensitivity and specificity of the SL test in diagnosing PC-BPPV compared with the DH test. METHODS: A cohort of 320 patients presenting with symptoms of vertigo underwent both the SL test and the DL maneuver. Two groups-group A and group B-were randomly formed. Group A patie

Topographic analysis of the skull vibration-induced nystagmus test with piezoelectric accelerometers and force sensors.

Vibration-induced nystagmus is elicited by skull or posterior cervical muscle stimulations in patients with vestibular diseases. Skull vibrations delivered by the skull vibration-induced nystagmus test are known to stimulate the inner ear structures directly. This study aimed to measure the vibration transfer at different cranium locations and posterior cervical regions to contribute toward stimulus topographic optimization (experiment 1) and to determine the force applied on the skull with a hand-held vibrator to study the test reproducibility and provide recommendations for good clinical pra

[Extratympanic electrocochleography in Ménière's disease].

INTRODUCTION: Electrocochleography is the registration of an electrophysiological event which takes place in the cochlea after an acoustical stimulus. Most of the authors consider an increased summating potential (SP)/action potential (AP) ratio as characteristic of endolymphatic hydrops. METHODS: A longitudinal, prospective study of a unilateral Ménière's population diagnosed according to the American Academy of Otolaryngology-Head and Neck Surgery classification was carried out. A complete clinical history and bedside examination were performed, in addition to complete auditory and vestibul

Clinical characteristics of benign paroxysmal positional vertigo in Korea: a multicenter study.

Benign paroxysmal positional vertigo (BPPV) is characterized by episodic vertigo and nystagmus provoked by head motions. To study the characteristics of BPPV in a large group of patients in Korea, we retrospectively analyzed clinical features of 1,692 patients (women: 1,146, 67.7%; men: 54.6, 32.3%; mean age: 54.8+/-14.0 yr), who had been diagnosed as BPPV by trained neuro-otologists Dizziness Clinics. The diagnosis of BPPV was based on typical nystagmus elicited by positioning maneuvers. Posterior semicircular canal was involved in 60.9% of the patients, horizontal canal in 31.9%, anterior ca

Visual-vestibular conflict induced by virtual reality in humans.

Conflicting inputs from visual and vestibular afferents produce motion sickness and postural instability. However, the relationship of visual and vestibular inputs to each other remains obscure. In this study, we examined the development of subjective sickness- and balance-related symptoms and objective equilibrium ataxia induced by visual-vestibular conflict (VVC) stimulation using virtual reality. The subjective symptoms evaluated by Graybiel's and Hamilton's criteria got gradually worse during the VVC. The objective postural instability was not observed during the VVC, but it occurred immed

Drop attacks in older patients secondary to an otologic cause.

The clinical features and treatment of seven patients with drop attacks attributable to inner ear disease presenting after age 65 are described. A neurologic or cardiovascular cause of drop attacks was initially suspected. Audiovestibular testing documented a unilateral inner ear disorder. The salient clinical features of these cases are discussed. The patients underwent ablative vestibular surgery, and all compensated well and were free of vertigo and falls up to 10 years postoperatively.

Diagnosis and documentation of central nervous system dysfunctions with craniocorpography after surgical removal of acoustic neurinomas.

Among other tests, craniocorpography (CCG) was performed in 21 patients after acoustic neurinoma surgery. After surgery, 17 patients (81%) had a developing vestibular compensation or an already normal CCG pattern; 3 patients (14%) had signs of persisting central nervous system dysfunction, either localized to the brain stem or in combination with a cerebellar dysfunction, and 1 patient showed a delayed but sufficient compensation after removal of a neurinoma that compressed central nervous system structures. Brain stem and cerebellar dysfunctions caused by tumor compression demonstrated a bett
