We report our experience with Molteno implants in 27 eyes of 20 children with glaucoma. Associated ocular findings included Sturge Weber syndrome (one patient), aniridia and retinoschisis (one patient), aphakia (seven patients, eight eyes), rubella syndrome (two patients), Peters anomaly (one patient), and Treacher Collins syndrome (one patient). PURPOSE: We present a novel office-based technique using a 25-gauge needle to reopen closed peripheral iridotomies in aphakic eyes filled with silicone oil.
METHODS: We use a 25-gauge needle to enter the clear cornea and reopen the closed peripheral iridotomy, allowing for aqueous humor and silicone oil to escape and relieve elevated intraocular pressure.
RESULTS: This minimally invasive office-based approach provides immediate relief of elevated intraocular pressure due to silicone oil-induced pupillary block, avoiding the need for more invasive surgical intervention.

METHODS: A retrospective analysis of medical records was performed. Each patient had an aphakic eye with intact iris and received the pound-shape silicone oil retention suture during vitrectomy combined with silicone oil injection.

We report the long-term clinical courses of 8 aphakic eyes of 2 sets of siblings who used contact lenses for both refractive correction and amblyopia treatment following neonatal cataract extraction. Early cataract removal, aggressive contact lens use, and robust professional supervision seem to have substantially contributed to visual success in our four patients. All eyes did well visually with contact lenses, all developing acuities close to 20/20 despite contact lens and non-contact lens related complications that were managed.
