Allergic and reactive dermatoses. How to identify and treat them.

The epidermis, as the body's first line of defense against many pathogens, reacts in a variety of ways to the assault of foreign antigens. Allergic contact dermatitis, drug eruption, urticaria, and erythema multiforme are examples. The clinician must usually rely on clinical evidence for diagnosis. The first therapeutic strategy is to identify and avoid the antigen or drug causing the reaction. Beyond this, therapy consists of relief of symptoms in mild to moderate cases. In severe cases, systemic treatment can be helpful, but the therapeutic options and their efficacy are limited.

Mycoplasma pneumoniae infection detected by immunofluorescence in patients with certain skin diseases.

The presence of Mycoplasma pneumoniae and/or herpes antigens was investigated by indirect immunofluorescence (IF) reactions in exfoliated cells or biopsy specimens from 43 patients with different skin diseases (treatment-resistant cutaneous herpes, genital herpes, acne, urticaria, dermatitis, erythema multiforme, ecthyma contagiosum). Either M. pneumoniae or herpes antigen could be detected in 21 (55.26%) and 12 (31.57%) of the 38 IF-positive cases, respectively, while the associated occurrence of mycoplasma and herpes antigens was observed in 5 (13.15%) of the patients.

Ceftazidime in the treatment of lower respiratory tract infection.

Forty patients with acute lower respiratory tract infection requiring hospital admission were treated with ceftazidime. One patient developed acute dyspnoea after the first dose of the drug which was considered to be drug related and treatment was discontinued. All remaining patients were clinically assessed as being either cured or improved. In all cases that were bacteriologically assessable the pathogens were eradicated. One patient developed erythema multiforme after the end of ceftazidime treatment; this was considered to be possibly drug related.

Accelerated photoaging induced by voriconazole treated with Q-switched Nd:YAG laser: case report and review of the literature.

Voriconazole is an antifungal agent mainly used against aspergillosis. Given its wide spectrum of action and limited adverse effects, it has replaced amphotericin B as the drug of choice in the prophylactic treatment of immunocompromised patients. Several adverse effects are caused by this drug with dermatological reactions accounting for 6% of the total. Such reactions include cheilitis, erythema, erosions, discoid lupus erythematosus, erythema multiforme, photosensitivity reactions, pseudoporphyria, accelerated photoaging and skin cancer. There are few reports on the accelerated photoaging c…

[Stevens-Johnson syndrome with transition to toxic epidermal necrolysis after carbamazepine administration, heroin and alcohol abuse].

A 28 year old patient developed a severe bullous exanthem and enanthem combined with hepatitis, fever and blood count abnormalities after taking carbamazepine and consumption of heroin and alcohol. After discontinuing carbamazepine, prednisolone was given over a five day period accompanied by intravenous fluid and electrolyte substitution and local therapy which lead to improvement. Severe bullous skin reactions nowadays are classified into erythema exsudativum multiforme majus (EEMM), Stevens-Johnson syndrome (SJS), overlap Stevens-Johnson syndrome-toxic epidermal necrolysis (SJS/TEN), TEN wi

An updated overview of HPV-associated head and neck carcinomas.

BACKGROUND: Intracranial malignancies (primary and metastatic) are often complicated by seizure activity. Phenytoin (Dilantin) is typically employed as prophylactic anticonvulsant in this setting. Uncommonly, erythema multiforme (EM) can develop in such patients at the port site during or soon after cranial radiation and can rapidly progress to EM major. Herein, in addition to a comprehensive literature review of this entity, three additional patients are presented. The acronym 'EMPACT' is suggested (E: erythema; M: multiforme; associated with P: phenytoin; A: and; C: cranial, radiation; T: th…

Lupus erythematosus-specific bullous lesions.

Lupus erythematosus (LE)-specific bullous lesions are often difficult to distinguish from other bullous diseases presenting in patients with systemic lupus erythematosus. Herein, we describe a 49-year-old woman with systemic lupus erythematosus with recurrent tense bullae on the forearms. Clinical, histopathologic, and serologic findings led to the diagnosis of LE-specific bullous lesions. We also summarize the diagnostic clues for distinguishing LE-specific bullous lesions, bullous systemic lupus erythematosus, and erythema multiforme-like lesions in LE (Rowell syndrome).

Severely Crusted Cheilitis as an Initial Presentation of Systemic Lupus Erythematosus.

Lupus erythematosus (LE) is an autoimmune disease which may initially present solely with lip lesions. Due to a wide spectrum of presentation, these features may initially be misdiagnosed as other oral diseases such as lichen planus, erythema multiforme (EM), and actinic cheilitis, leading to a delay in diagnosis and treatment. We discuss a case of severely crusted cheilitis which was initially diagnosed as EM, with subsequent development of subacute cutaneous LE, and progression to systemic LE. We will discuss the clinical and histological features of lupus cheilitis.

Stevens-Johnson syndrome induced by sorafenib for metastatic renal cell carcinoma.

Sorafenib is an orally administered active multikinase inhibitor for metastatic renal cell carcinoma that is now considered a standard agent. Skin toxicity, such as hand-foot skin reaction, is one of the frequent adverse effects of sorafenib. On the other hand, sorafenib-induced erythema multiforme is very rare, and Stevens-Johnson syndrome and toxic epidermal necrolysis induced by sorafenib have not been reported. We report the first case of Stevens-Johnson syndrome caused by sorafenib for metastatic renal cell carcinoma.

Acute hemorrhagic edema in a four-year-old Japanese boy.

A case of acute hemorrhagic edema (AHE), a unique form of leukocytoclastic vasculitis limited only to the skin, was seen in a four-year-old boy. It was characterized by an abrupt onset of fever, peripheral edema, and rosette-shaped purpuric plaques. Although the skin lesion might be confused clinically with other dermatoses such as Henoch-Schönlein purpura, Sweet's disease and erythema multiforme, his laboratory data were normal, and the disorder follows a benign course with spontaneous resolution within one week.

Immunologic diseases.

Selected equine cutaneous diseases for which there is strong evidence of an immunological aetiology are discussed in this section. The reactive disorders of urticaria, Culicoides hypersensitivity, erythema multiforme, drug eruptions and systemic granulomatous disease are included. The autoimmune diseases of the horse with cutaneous manifestations (pemphigus foliaceus, pemphigus vulgaris, bullous pemphigoid and cutaneous lupus erythematosus) are discussed. The clinical signs and diagnostic features of these immunologically induced equine dermatoses are defined, and the challenges in making a de…

Nonneoplastic disorders of the eccrine glands.

Eccrine glands are uniquely susceptible to a variety of pathologic processes. Alteration in the rate of sweat secretion manifests as hypohidrosis and hyperhidrosis. Obstruction of the eccrine duct leads to miliaria. The excretion of drugs into eccrine sweat may be a contributory factor in neutrophilic eccrine hidradenitis (NEH), syringosquamous metaplasia (SSM), coma bulla, and erythema multiforme (EM). Alterations in the electrolyte composition of eccrine sweat can be observed in several systemic diseases, most notably cystic fibrosis. This article summarizes current knowledge of eccrine glan…

Perforating ENL: A variant of type 2 lepra reaction.

Lepra reactions are acute episodic inflammatory reactions that occur during illness due to abrupt changes in the body's immunological response against Mycobacterium leprae. These are of two types, type 1 and type 2. Type 2 reaction is also called Erythema Nodosum Leprosum (ENL). The common skin lesion in ENL is a red, tender, raised lesion. Subcutaneous nodules can also appear. The skin lesions may present as vesicles, pustules, or bulla. They can also develop ulcers and become necrotic. Some patients may experience skin lesions that resemble those seen in erythema multiforme. A rare desquamat…
