Clinical Results of Distal Radius Intraarticular Comminuted Fractures in the Elderly: A Retrospective Comparative Analysis of Different Fixation Methods.

Introduction Distal radius intraarticular fractures in the elderly population are likely to cause impaired clinical outcomes. Intraarticular fracture treatment in the literature is a debatable issue with mixed results. Here, we aimed to present a tertiary trauma center experience with plate fixation and K wire-assisted external fixator in elderlies over 60 years old. Material and methods Patients who were diagnosed with an unstable intraarticular distal radius fracture and received surgical treatment with plate fixation or K wire-assisted external fixator between 2016 January and 2020 January

[Treatment guidelines and results in severe open tibial fractures].

27 patients with compound fractures of the lower leg were treated according to the following scheme: Exact excision and surgical cleaning of the wound, reduction of the fragments without further exposure of the bone by additional incision, minimal osteosynthesis, stabilization by means of external fixation device and secondary closure of the accidental wound. During the last year we preferred the V-shaped arrangement in contrast to the tent shaped arrangement of the external fixation device as this minimizes the trauma to the extensor muscles of the foot. After external fixation combined with

Terrible Triad Injuries of the Elbow.

The treatment of terrible triad injuries of the elbow continues to evolve. Radial head fixation and arthroplasty, coronoid process fixation, and repair of the lateral collateral ligament continue to be the mainstays of treatment. In the elbow with persistent instability after repair of these elements, application of a static external fixation, hinged external fixation, ulnohumeral joint pinning, or an internal hinge may be needed. In patients who undergo treatment after the acute injury period, the coronoid may require reconstruction using radial head autograft, iliac crest autograft, olecrano

Current treatment of congenital pseudarthrosis of the tibia: a systematic review and meta-analysis.

To compare the current treatments for congenital pseudarthrosis of tibia, a total of 33 studies were reviewed. Vascularized fibular graft with external fixation or combined fixation had the fastest time till initial union (P<0.05). Bone morphogenetic protein had no advantage in terms of initial union, time till union, and refracture rates. This meta-analysis supports that fixation methods with corticocancellous bone autograft utilizing the combined technique of Ilizarov external fixation and intramedullary rod stabilization ensure a statistically significant reduction in the number of refractu

[Biomechanical comparison of six different fixation techniques for treatment of metacarpal neck fractures].

PURPOSE: The purpose of this study was to evaluate the primary stability of commonly used fixation techniques for metacarpal neck fractures and to compare it with that of an innovative osteosynthesis device: IlluminOss™, the photodynamic bone stabilization system™. MATERIAL AND METHODS: Metacarpal neck fractures were created on the second to fifth metacarpal bones of 24 freshly frozen human cadavers using a band saw. Specimens were randomly assigned to one of six groups and treated with (1) K-wire fixation, (2) dorsal plating, (3) external fixation, (4) cross-threaded K‑wire fixation, (5) Ill

[Is use of the fixateur externe no longer indicated for the treatment of unstable radial fracture in the elderly?].

BACKGROUND: In recent years there has been an increasingly marked shift in the operative treatment of unstable fractures of the distal radius. The introduction of locking compression plates has made it possible to extend the indications for palmar stabilisation according to the principles of internal fixation to extension fractures even in osteoporotic bone, and since then the new design has been used more and more widely. First clinical results show very good and good clinical and radiological outcomes in over 80% of cases after locking compression plate osteosynthesis. All this raises the qu

Mechanics of struts in the Taylor Spatial Frame.

Taylor Spatial Frame (TSF) is a hexapod circular external fixator, i.e. built with six struts connecting two rings to support bone fragments by fixating them to the rings by wires or pins. Struts are length adjustable and have universal joints (U-joints) at both ends which attach to the rings. Behaviour of struts is crucial to the biomechanics of the TSF. However, no study on struts' mechanical characteristics has been reported. In this work, TSF struts and their components and pieces of materials were tested and their mechanical properties are reported. For doing so, the followings tests were

Safe corridors for external skeletal pin insertion in rabbit hindlimb long bones.

External skeletal fixation (ESF) is a minimally invasive method for fracture repair that requires precise anatomical knowledge to prevent soft tissue and nerve injury. This study aimed to identify and describe safe, hazardous, and unsafe corridors for ESF pin insertion in the hind limb long bones of rabbits, based on detailed topographic and cross-sectional anatomy. A total of ten 12-week-old Hyla hybrid rabbits (5 males, 5 females; avg. weight 2.5 kg) with no prior limb fractures or illnesses were used. A safe corridor was defined as an area where the bone is covered only by skin and subcutan

The effect of transfixation pins on the biomechanical properties of angled acrylic connecting bars.

With acrylic external-fixation frames for fracture repair the acrylic columns can be contoured to allow greater versatility in the placement of transfixation pins, thus minimizing damage to the surrounding soft tissue and making mandibular and transarticular fixation easier. However, contouring affects the stiffness and ultimate strength of the construct under axial compression. In this study, polymethylmethacrylate columns 21 mm in diameter angled at 0°, 30°, 45°, 60°, or 90° with clamps were constructed. For each angulation group, pins 3.2-mm long were placed in 6 columns, 2 pins at each end

The 'Retro-Fibular Wire': an anatomical study describing a safe corridor for placement of fine wires in the distal tibia.

Anatomical atlases document safe corridors for placement of wires when using fine-wire circular external fixation. The furthest posterolateral corridor described in the distal tibia is through the fibula. This limits the crossing angle and stability of the frame. In this paper we describe a new, safe Retro-Fibular Wire corridor, which provides greater crossing angles and increased stability. In a cadaver study, 20 formalin-treated legs were divided into two groups. Wires were inserted into the distal quarter of the tibia using two possible corridors and standard techniques of dissection identi

Simple solutions for difficult problems: a beginner's guide to ring fixation.

Ring external fixation is a surgical method for leg lengthening and deformity correction. It allows limited surgical exposure and dissection and can be tolerated for prolonged periods as compared to threaded uniplanar or multiplanar constructs. Because of the complexity in application and adjustment of the frame constructs, most orthopedic surgeons avoid using these devices. Surgeons are advised to apply these techniques initially to less complicated clinical situations, expanding application with comfort and proficiency. This discussion introduces ring fixation and provides guidelines for sim

Comparison of the LISS and a retrograde-inserted supracondylar intramedullary nail for fixation of a periprosthetic distal femur fracture proximal to a total knee arthroplasty.

Simulated supracondylar fractures were created proximal to posterior cruciate ligament-retaining total knee arthroplasty components in paired human cadaver femora and stabilized with either a retrograde-inserted locked supracondylar nail or the Less Invasive Stabilization System (LISS; Synthes USA, Paoli, PA). Loads were applied to create bending and torsional moments on the simulated fracture stabilized with either no gap or a 10-mm gap. The LISS exhibited less torsional stability with anterior (P<.001) and posterior loads (P<.01). When varus loads were applied to 10-mm-gap specimens, the spe

Percutaneous pin placement in the medial calcaneus: is anywhere safe?

OBJECTIVE: To redefine the medial calcaneal anatomic safe zone for pin placement with respect to reproducible palpable landmarks. DESIGN: Anatomic study. SETTING: Medical school anatomy laboratory. INTERVENTIONS: Thirty-three fresh-frozen adult cadaveric feet were used. Three palpable anatomic landmarks were identified on each ankle and labeled as Point A (posteroinferior medial calcaneus), Point B (inferior medial malleolus), and Point C (navicular tuberosity). The medial neurovascular bundle was carefully dissected, and the medial calcaneal nerve, the most posterior branch of the lateral

Intra-articular fractures of the distal aspect of the radius.

Intra-articular distal radius fractures are a heterogeneous group of injuries with different fracture patterns. The existing classification systems are helpful for describing the fractures but not for assessing their stability or for deciding which surgical approach to use. Patients who have a fracture with at least 1.0 mm of displacement of the articular surface may benefit from open surgical treatment. Improved diagnostic imaging with CT is helpful for fracture classification and surgical planning. The options for surgical treatment include limited open reduction and internal fixation, arthr

Treatment options for fractures of the distal femur.

The correct use of the available implants is technically demanding, but the technique can be mastered if the surgeon follows a methodical approach to developing skills and confidence. If facilities are available, practice on artificial bones will improve your understanding of the instruments and your skills, as will assisting on as many cases as possible with experienced surgeons. Begin with relatively simple fractures and progress gradually to the more difficult ones. In order to achieve a good result with surgical treatment of distal femur fractures, the basic principles developed by the AO/

The transfixational plaster cast technique.

Unstable fractures of the forearm and leg as well as combined fractures of the arm and forearm are often difficult to treat adequately by closed reduction and plaster fixation. In tropical countries, however, open reduction is relatively contra-indicated because of increased risk of peri-operative infection, inadequate training of medical personnel, and lack of adequate equipment. Given these conditions, the use of the transfixational plaster cast offers an alternative which is simple, inexpensive, and with few complications. The principle and technique of the transfixational plaster cast are

The Cost-Effectiveness of Surgical Fixation of Distal Radial Fractures: A Computer Model-Based Evaluation of Three Operative Modalities.

BACKGROUND: There is no consensus on the optimal fixation method for patients who require a surgical procedure for distal radial fractures. We used cost-effectiveness analyses to determine which of 3 modalities offers the best value: closed reduction and percutaneous pinning, open reduction and internal fixation, or external fixation. METHODS: We developed a Markov model that projected short-term and long-term health benefits and costs in patients undergoing a surgical procedure for a distal radial fracture. Simulations began at the patient age of 50 years and were run over the patient's life

Promises and difficulties with the use of femoral intra-medullary lengthening nails to treat limb length discrepancies.

Limb lengthening using external fixation may be associated with problems such as pin-track infections, poor patient acceptance, muscle trans … theatre. Femoral lengthening with an intramedullary lengthening nail is a reasonable alternative to external fixators, thereby avoiding problems associated with callotasis using external fixation methods. It is however, important to counsel patients regarding possibilities of significant obstacles including failure and multiple visits to theatre during the process.

High-energy fractures of the tibial plateau.

High-energy tibial plateau fractures are often the result of blunt trauma. In addition to extensive bony injuries, soft-tissue problems are very common. Preoperative planning for surgical repair of these injuries is invaluable. Contemporary internal fixation techniques using ligamentotaxis, percutaneous fixation, and antiglide techniques provide the basis for internal fixation of these injuries. When extensive comminution and damaged soft tissues prohibit their use, the circular external fixator provides an excellent option for management of these complex injuries. Whether internal or external

An Efficient and Reproducible Protocol for Distraction Osteogenesis in a Rat Model Leading to a Functional Regenerated Femur.

This protocol describes the use of a newly developed external fixator for distraction osteogenesis in a rat femoral model. Distraction osteogenesis (DO) is a surgical technique leading to bone regeneration after an osteotomy. The osteotomized extremities are moved away from each other by gradual distraction to reach the desired elongation. This procedure is widely used in humans for lower and upper limb lengthening, treatment after a bone nonunion, or the regeneration of a bone defect following surgery for bone tumor excision, as well as in maxillofacial reconstruction. Only a few studies clea
