We present a case report describing the surgical technique for the reconstruction of the proximal interphalangeal (PIP) joint collateral ligament using autograft palmaris longus tendon graft. We accomplished this successfully in a high performance professional athlete presenting with chronic instability of the PIP joint. Our surgical reconstructive technique recreated anatomic ligament position, while correcting multidirectional instability without disruption of the other dynamic anatomic structures of the PIP joint. Our results demonstrated excellent clinical and functional results, and we of

Although magnetic resonance imaging (MRI) studies of psoriatic arthritis (PsA) are fewer than those of rheumatoid arthritis (RA), interest in this field is growing. The type and site of the lesions, rather than the mere severity of synovitis, can help differentiate PsA from other arthritides. Extracapsular enhancement and enthesitis are features emphasized as typical of PsA, but their relevance for the diagnosis is more quantitative than qualitative. Erosions in PsA are probably less frequent and progressive than in RA. Bone edema is unlikely to predict the appearance of erosions in patients w

We present 2 cases of symptomatic nondissociative dorsal intercalated segment instability (CIND-DISI) collapse after trapeziectomy and partial trapezoidectomy for pantrapezial arthritis. Preoperative radiographs showed normal carpal alignment in 1 case and a mild DISI alignment in the other. After surgery both developed increased CIND-DISI and carpal symptoms that differed from their original thumb-base problems. One required carpal realignment and fusion; to date the other has responded to nonsurgical treatment. Such problems may be recognized insufficiently by surgeons and thus may be more c

Volar dislocations of the proximal interphalangeal joint, if missed, with extensor tendon entrapment will lead to permanent impairment. Prompt diagnosis followed by open reduction and aggressive rehabilitation is necessary.

Contrast arthrography was performed in 20 patients with duplicated thumbs where the interphalangeal, metacarpophalangeal, and carpometacarpal joints were involved. Three groups were apparent, those with no joint alteration, those with joint involvement and separate articular impressions, and those with abnormal duplex joints.

We report the case of a 23-year-old man who fell from a third-floor window and injured his left hand. Physical examination and radiographic evaluation revealed open dorsal metacarpophalangeal dislocation of four fingers. The index finger dislocation was reduced in the emergency department, and the other three fingers responded to closed reduction during surgery. The patient made an uneventful recovery without complications. Metacarpophalangeal dislocations are unusual, with multiple dislocations rarely described.

Many procedures have been described for treatment of unstable, deformed, or painful digital joints; most of these are various types of arthrodeses. These procedures have not been applied to children who have an open epiphysis for fear of damage to the growth plate. This study evaluates an arthrodesis technique that can be used for digital joint instability or deformity in skeletally immature patients. The diagnosis in most of the children in this series was related to non-traumatic congenital or developmental conditions. The joint most often fused was the thumb metacarpophalangeal joint. Resul

DIP joint arthritis may be painful and interfere with function of a finger or the entire hand. When conservative measures are no longer adequate to control the symptoms, operative intervention may be recommended. Arthrodesis is the procedure of choice in most situations, although specialized functional requirements may justify an attempt to maintain motion. Treatment of mucous cysts requires removal of the offending osteophyte as well as the cyst itself. Technique of skin closure depends on the situation and the surgeon's preference. Careful attention to the indications for surgery, to the edu

We describe the vascularised transfer of a second metatarsophalangeal joint to the hand to correct a congenital deformity in a thirteen-year-old boy. Technical refinements are described, allowing inversion of the joint to the optimal position for subsequent mobility and retention of a large overlying dorsalis pedis flap to provide adequate cover.

We reviewed 13 patients with a complex fracture-dislocation of the proximal interphalangeal joint of a finger and one patient with a complex fracture-dislocation of the interphalangeal joint of thumb. We had treated these injuries using a pins and rubbers traction system which had been modified to avoid friction of the pins against the bone during mobilisation of the joint in order to minimise the risk of osteolysis. A Michigan hand outcome questionnaire was used for subjective assessment. The active range of movement (AROM) of the proximal and distal interphalangeal joints and the grip streng

BACKGROUND: Fractures of the fingers and metacarpal bones are the most common fracture type in the upper limb, with an incidence of 114 to 1483 per 100 000 persons per year. The clinical importance of closed finger and metacarpal fractures is often underestimated; inadequate diagnostic and therapeutic measures may result in serious harm. This review concerns the basic elements of the diagnosis and treatment of finger and metacarpal fractures.
METHODS: This review of the incidence, diagnosis and treatment of finger and metacarpal fractures is based on pertinent publications retrieved by a sele

Camptodactyly (MIM 114200) is a digit deformity characterised by permanent flexion contracture of fifth fingers at the proximal interphalangeal (PIP) joints. The sporadic cases are common but a familial occurrence is not much appreciated. In an attempt to identify the genetic basis of camptodactyly, we have analysed a large German family with camptodactyly segregating in an autosomal dominant fashion. The affected family members exhibited clinical features of fifth finger camptodactyly and knuckle pads on the crooked fifth finger and on fingers 2-3. Typically, women were more severely affected

In this paper we describe a class of analog algorithms based on the concept of cellular neural networks (CNNs) for detecting osteoarthritis (OA) from x-ray images. The indicator of OA that we examine is the presence of bony spurs or osteophytes in the vicinity of the weight bearing joints of the fingers. Results on a series of hand x-ray images are shown to be promising.

A 15 year old boy had permanent flexion contractures at proximal interphalangeal joints of the little and ring fingers of both hands, along with slight hemiatrophy of face and bilateral mild ptosis. The hand deformity superficially resembled. Dupuytren's contacture. This combination of features does not fit with the syndromes described so far, whose one of, the components is camptodactyly.

A case of mosaic trisomy 9 (46,XX/47,XX,+9) is described. The main clinical findings included intrauterine growth retardation, failure to thrive, hydrocephalus, deeply set eyes, prominent antihelix, highly arched palate, subluxation of the interphalangeal joints of some fingers, hip dislocation, excessive sweating, and punctate mineralization in developing cartilages.

Fibroma of the tendon sheath (FTS) is an uncommon benign soft tissue tumor of the tendon sheath. Clinical and radiological features are not distinctive enough to clinch the diagnosis preoperatively. FTS occurs mostly around small joints such as the fingers, hands, and wrist. However, it rarely arises around a large joint (knee, shoulder, elbow, or ankle). This case report describes a rare presentation of fibroma within the patellar tendon. The patient, a 35-year-old male, presented with progressive pain and swelling in his left knee. Clinical examination, imaging studies, and histopathological
