A Case of Fingernail Onychomycosis due to Aspergillus flavus.

A 56-year-old woman on insulin therapy for diabetes visited our clinic due to whitish discoloration on the right index finger. Despite topical application of 1% lanoconazole solution, the lesion grew, causing paronychia. Direct microscopy revealed non-dermatophyte molds. Based on the morphological features and genetic analysis of the isolate, the pathogen was identified as Aspergillus flavus. The patient was diagnosed with proximal subungual onychomycosis due to A. flavus. Following itraconazole pulse therapy, she was cured in 6 months. To our knowledge, this is the first reported case of fing

Contact Allergy to Surfactants in a Hypoallergenic Liquid Cleanser.

: Surfactants are a relatively rare cause of allergic contact dermatitis (ACD) and testing patients to personal care products containing these ingredients has historically been difficult given their irritant properties. Using the semiopen technique, we were able to identify ACD to a hypoallergenic liquid cleanser in 2 patients who presented to our patch test clinic only months apart. Additional patch testing to individual ingredients led to subsequent identification of 3 novel surfactant allergens (sodium lauroyl sarcosinate, isostearamidopropyl morpholine lactate, and disodium lauroamphodiace

Unilateral hand allergic contact dermatitis due to occupational exposure.

BACKGROUND AND OBJECTIVE: Hand dermatitis is the most common subtype of occupation-related skin disease. Regardless of its underlying cause, hand dermatitis results in significant impairment and a negative impact on quality of life. Any case of unilateral hand dermatitis should have a high index of suspicion for an allergic contact dermatitis. CONCLUSION: We report two cases of unilateral hand dermatitis. The first case is a 53-year-old female with a unilateral dermatitis of the left hand. The second case is a 53-year-old male with dermatitis of the right hand. Both cases highlight an interes

Cryotherapy-induced milia en plaque: case report and literature review.

BACKGROUND: Cryotherapy-induced milia is a rarely described cutaneous reaction that may occur in patients who have received cryotherapy with liquid nitrogen. Cryotherapy-induced milia is characterized by 1-2 millimeter white dermal cysts that develop at the healed cryotherapy site. Milia en plaque, an erythematous plaque containing numerous milia, has not previously been described following treatment of a skin lesion with liquid nitrogen cryotherapy. PURPOSE: We describe a man who developed cryotherapy-induced milia en plaque after receiving cryotherapy to his dorsal hand for the treatment of

[Digital ulcers in systemic scleroderma].

Digital ulcers (DU's) are one of the main symptoms of systemic scleroderma and occur in approximately 60% of all scleroderma patients. Due to possible complications such as infections, gangrene or amputation, they require regular medical attention and a good wound treatment by doctors and nursing staff. A definition of DU's has not yet been established. In 2009 the European League Against Rheumatism (EULAR) published guidelines for the treatment of DU's. An improvement of the healing of active ulcers has been described with Iloprost. Bosentan significantly reduced the frequency of occurrence o

Cumulative life course impairment in other chronic or recurrent dermatologic diseases.

Skin diseases are visible, and identifying abnormal skin generally does not require specialist knowledge. Dermatology is therefore a ripe field for studies of cumulative life course impairment, because of the many diseases that affect not only the patients, but also their psychosocial interaction with others. Dermatological patients are visibly sick. The stigma associated with visible as well as hidden skin diseases is considerable and may have a major negative impact on the life course of patients. Stigma and psychosocial relations are however not the only sources of impairment for patients w

Coin exposure may cause allergic nickel dermatitis: a review.

Nickel is used in coins because the metal has beneficial properties, including price, colour, weight, and corrosion resistance, and also because it is easy to stamp. It has often been claimed that the duration of skin contact with coins is too short to cause nickel release and dermatitis. However, it is well known by dermatologists specialized in occupational skin diseases, and by their nickel-allergic patients, that hand eczema in cashiers and other professionals who handle coins may be caused or aggravated by nickel release from coins. In this review, we present evidence from past studies sh

Bullous pyoderma gangrenosum of the bilateral dorsal hands.

Pyoderma gangrenosum (PG) shows characteristic non-infectious ulcers that are commonly associated with systemic diseases such as inflammatory bowel diseases, myeloproliferative disorders or aortitis syndrome. The typical clinical appearance is undermining ulcers with reddish and irregular borders on the legs. As PG has these notable signs, the diagnosis is relatively easy and its treatment depends on the severity of underlying complications. We report a case of a 60-year-old Japanese man, diagnosed with bullous PG, who also had been suffering from myeloperoxidase antineutrophil cytoplasmic ant

Blastomycosis-like pyoderma in association with recurrent vesicular hand eczema: good response to acitretin.

Blastomycosis-like pyoderma, also known as pyoderma vegetans, is an uncommon disease characterized by vegetating skin lesions mimicking blastomycosis or warty tuberculosis, often associated with staphylococcal and streptococcal infections. It is more commonly observed in elderly patients with impaired immunological capacity, and is often unresponsive to various therapeutic modalities. We describe a case of BLP that occurred on the palmar aspect of the left hand in an elderly female, affected with recurrent vesicular hand eczema. The disease resolved completely following the administration of o
