Comprehensive Classification of the Capitellar Injury Concurrent with Radial Head Fracture.

BACKGROUND: Capitellar injury (CI) includes capitellar cartilage injury (CCI) and capitellar fracture (CF). A comprehensive classification of CI concurrent with radial head fracture (RHF) that can guide surgical strategy is lacking in the literature. Therefore, this study aimed to introduce a comprehensive classification of CI concurrent with RHF and investigate its value. METHODS: A total of 35 patients with CI concurrent with RHF confirmed by surgical exploration were retrospectively analyzed, includingmales in 19 cases and females in 16 cases. RHF was classified according to the Mason clas

Dynamic Anterior Shoulder Stabilization With the Long Head of the Biceps Tendon: A Biomechanical Study.

BACKGROUND: The concept of dynamic anterior shoulder stabilization (DAS) combines a Bankart repair with the additional sling effect of the l … 7 mm, P = .035). However, in 20% defects, DAS led to a relevant posterior and inferior shift of the humeral head in the ABER position and to a relevant increase in inferior glenohumeral translation. Both surgical techniques did not limit the rotational range of motion. CONCLUSION: In the context of minor glenoid bone defects, the DAS technique demonstrates le …

Recurrent Anterior Shoulder Instability With Combined Bone Loss: Treatment and Results With the Modified Latarjet Procedure.

BACKGROUND: Recurrent anterior glenohumeral dislocation in the setting of an engaging Hill-Sachs lesion is high. The Latarjet procedure has been well described for restoring glenohumeral stability in patients with >25% glenoid bone loss. However, the treatment for patients with combined humeral head and mild (<25%) glenoid bone loss remains unclear. PURPOSE/HYPOTHESIS: This study reports on the outcomes of the modified Latarjet for patients with combined humeral and glenoid defects and compares the results for patients with ≤25% glenoid bone loss versus patients with >25% glenoid bone loss. T…

Subcutaneous Anterior Transposition With Modified Procedures to Prevent Recurrence Associated With the Transposed Ulnar Nerve.

Subcutaneous anterior transposition of the ulnar nerve is a common surgical treatment for cubital tunnel syndrome. However, there are surgical failures associated with the new compressive sites at the edge of flexor carpi ulnaris (FCU) and resubluxation posterior to the medial epicondyle of the transposed nerve. To reduce the muscle volume at the edge of FCU, we approach the ulnar nerve by dividing the muscle belly of the FCU humeral heads. This procedure can reduce repeated traction forces on the transposed nerve at the edge of the FCU. To keep the transposed ulnar nerve anteriorly, we use a …

Modified-tension band suturing: 'the pulling the reins technique' - a technique for displaced proximal humeral fracture with medial column instability.

Fractures of the proximal humerus with medial column instability are challenging and present an unacceptable rate of complications and reoperations. Despite good results reported with the use of locking plates and augmentation techniques using bone graft or a second plate, varus subsidence and fixation failure have been frequently reported. We describe the case of a patient presenting with a complex, multifragmentary proximal humerus fractures successfully treated with open anatomic reduction and internal fixation using a locking plate augmented with lateral traction using three bone anchors i

Coexistence of rare variations of palmaris profundus and flexor superficialis muscle.

The superficial layer of the anterior compartment of the forearm is characterized by different morphological variations. During standard anatomical dissection, two morphological variations in the superficial compartment of the forearm were observed. The first one was the palmaris profundus, which originated from the radius and distally fused with the tendon to 3rd digit (from the flexor digitorum superficialis muscle) inserted on the medial phalanx of the third digit. The second variation was a distinct superficial flexor of the 4th digit represented by one muscle belly with originated from a

Editorial Commentary: Biomechanical Investigation of Superior Capsule Reconstruction Requires Meticulous Methods.

One of the treatment options for a massive irreparable rotator cuff tear is superior capsule reconstruction (SCR). Biomechanically, SCR depresses and centers the humeral head on the glenoid so that the remaining rotator cuff muscles and the deltoid can provide function. Therefore, in SCR, graft characteristics such as graft type, graft thickness, and graft tension are the critical parameters contributing to the biomechanical effectiveness. SCR was originally developed with fascia lata autografts, but in the United States, the most popular graft used is acellular human dermal allograft. Both ha…

Application of three-dimensional prototyping in planning the treatment of proximal humerus bone deformities.

OBJECTIVE: To describe the use of three-dimensional prototyping or rapid prototyping in acrylic resin to create synthetic three-dimensional models in order to promote the understanding of bone deformities of the shoulder. METHODS: Five patients were analyzed between ages of 11 and 73 years old, treated between 2008 and 2013 with glenohumeral deformities that required a more thorough review of the anatomical alterations, for whom three-dimensional prototyping was performed. RESULTS: Patient 1 was treated conservatively and is awaiting humeral head arthroplasty if symptoms get worse. Patient 2…

Arthroscopic latissimus dorsi transfer.

The patient is placed in lateral decubitus. A 6-cm incision made in the axilla allows access to the latissimus dorsi tendon and its neurovascular pedicle. Holding the arm in internal rotation, the surgeon detaches sharply the tendon off the humeral shaft and then reinforces it with wrapping sutures. Pulling the free limbs of the sutures exposes the under surface of the muscle and helps to identify the neurovascular pedicle. Special lighting retractors suited for a large diameter scope are helpful. Mobilization is completed when 2 cm of the tendon crosses the posterior edge of the acromion. The

Effect of rotator interval closure on glenohumeral stability and motion: a cadaveric study.

The effect of rotator interval closure, which is performed as an adjunct to arthroscopic stabilization of the shoulder, has not been clarified. Fourteen fresh-frozen cadaveric shoulders were used. The position of the humeral head was measured using an electromagnetic tracking device with the capsule intact, sectioned, and imbricated between the superior glenohumeral ligament and the subscapularis tendon (SGHL/SSC closure) or between the superior and middle glenohumeral ligaments (SGHL/MGHL closure). The direction of translational loads (10, 20, and 30 N) and arm positions were (1) anterior, po…

Bilateral posterior fracture dislocation of the shoulders following seizure.

Bilateral posterior fracture dislocation is a rare injury known to be associated with seizures. Convulsion was found to be the cause of fracture dislocation in 78% of the cases reported. The mechanism of injury was described by Shaw in 1971. The management depends largely on the severity of the injury. In many cases reported, the fracture was a large compression defect in the anteromedial aspect of the articular surface of the humeral head. It has been suggested that for defects that involve less than 20% of the articular surface closed reduction can be attempted. Rush nail or percutaneous K w…

Tardy ulnar nerve palsy caused by cubitus varus deformity.

Fifteen patients with tardy ulnar nerve palsy caused by cubitus varus deformity were studied. All patients had a history of previous fracture of the humerus during childhood. The mean interval between fracture and onset of symptoms was 15 years. The severity of the palsy was classified as McGowan's grade I in 12 patients, grade II in 2 patients, and grade III in 1 patient. The mean carrying angle was -2 degrees before surgery. X-ray films showed a shallow ulnar nerve groove, a dysplastic humeral trochlea, medial shift of the ulna, and deformity of the medial epicondyle. The ulnar nerve was exp

Profunda Brachii Vein Revisited: Anatomy of the Vein in the Posterior Upper Arm and Its Relationship to the Triangular Interval.

INTRODUCTION: The profunda brachii vein (PBV) accompanies the profunda brachii artery through the radial groove and triangular interval. However, anatomical descriptions of the PBV remain limited and are sometimes inconsistent across textbooks and the literature, potentially leading to misunderstandings in medical education and surgical practice. Therefore, this study aimed to clarify the detailed anatomy of the PBV and classify variations in its course within the radial groove and triangular interval. METHODS: A total of 129 upper limbs from 65 adult Japanese cadavers (26 males and 39 female
