Correlation of Preoperative Renal Insufficiency With Mortality and Morbidity After Aortic Valve Replacement: A Propensity Score Matching Analysis.

Preoperative end-stage renal disease carries a high mortality and morbidity risk after aortic valve replacement (AVR), but the effect of renal insufficiency remains to be clarified. Through propensity score analysis, we compared the preoperative demographics, perioperative profiles, and outcomes between patients with and without renal insufficiency. From August 2005 to November 2014, 770 adult patients underwent AVR in a single institution. Patients were classified according to their estimated glomerular infiltration rate (eGFR) as renal insufficiency (eGFR: 30-89 mL/min/1.73 m) or normal (eGF

Known Unknowns: Unpacking Controversies and Uncertainties in the 2025 Guidelines for Management of Acute Coronary Syndromes.

PURPOSE OF REVIEW: The 2025 ACC/AHA/ACEP/NAEMSP/SCAI Guideline for managing patients with acute coronary syndromes (ACS) is the first major update since the 2013-2015 ST-elevation myocardial infarction (STEMI) and non-ST elevated myocardial infarction (NSTEMI) guidelines. It brings both conditions together into a single framework. This review covers the most important new and revised recommendations and points out where evidence is still lacking. RECENT FINDINGS: New findings show that several significant updates are based on recent large-scale trials. The use of bivalirudin with high-dose in

Off-pump coronary artery bypass surgery: evaluation of extubation time and predictors of failed early extubation.

Anesthesia during and after off-pump surgery is critical for the outcome of the procedure. Intubation time has been shown to correlate with ICU time and length of stay. This study is to evaluate the extubation time and predictors of prolonged extubation in this institution. One hundred and sixty consecutive patients during Jan 2001-June 2002, excluding pre-operative tracheostomy (n = 1) were retrospectively reviewed. Anesthetic agents include fentanyl, rocuronium Bromide, midazolam and sevoflurane. Phenylephrine and nitroglycerine were used to maintain adequate arterial pressures. Post-operati

Recurrent Takotsubo Cardiomyopathy Triggered by COVID-19 Infection Complicated by Ventricular Tachycardia Arrest and Cardiogenic Shock: A Case Report.

Takotsubo cardiomyopathy is a transient stress-induced cardiomyopathy typically triggered by emotional or physical stress. Since the emergence of coronavirus disease 2019 (COVID-19), several cardiovascular complications have been reported, including stress cardiomyopathy. Recurrent Takotsubo cardiomyopathy complicated by malignant ventricular arrhythmias and cardiogenic shock remains uncommon. A 73-year-old Hispanic female with a prior history of Takotsubo cardiomyopathy in 2013, subclinical hyperthyroidism, and hyperlipidemia presented with worsening malaise and subjective fever. She tested p

National Outcomes of Nonintubated Extracorporeal Membrane Oxygenation as a Bridge to Heart Transplantation.

BACKGROUND: Nonintubated venoarterial extracorporeal membrane oxygenation (ECMO) has been described in patients with refractory cardiogenic shock, but there are limited data on outcomes of this technique as a bridge to heart transplantation. We investigated this practice in a national cohort. METHODS: Adult heart recipients bridged with ECMO from October 18, 2018, to September 30, 2023, in the United Network for Organ Sharing database were categorized by use of ECMO alone (ECMO-only) and ECMO with mechanical ventilation (ECMO+VENT) at transplant. Between groups, we compared posttransplant str

Impella vs. IABP in Non-emergent High-Risk PCI: Outcomes and LVEF Recovery from a Large US EHR Study.

INTRODUCTION: High-risk percutaneous coronary intervention (HR-PCI) involves patients with complex coronary disease, adverse hemodynamics, and/or severe comorbidities who are often ineligible for surgery. Mechanical circulatory support (MCS) may reduce procedural risk and facilitate complete revascularization. However, comparative data on outcomes and left ventricular ejection fraction (LVEF) recovery are limited. We hypothesized that the benefits of MCS-supported PCI observed in prior studies would extend to contemporary, real-world, non-protocolized all-comer datasets, where outcomes and LVE

Beating Heart Transplant Procedures Using Organs From Donors With Circulatory Death.

IMPORTANCE: The use of ex vivo normothermic organ perfusion has enabled the use of deceased after circulatory death (DCD) donors for heart transplants. However, compared with conventional brain death donation, DCD heart transplantation performed with ex vivo organ perfusion involves an additional period of warm and cold ischemia, exposing the allograft to multiple bouts of ischemia reperfusion injury and may contribute to the high rates of extracorporeal membrane oxygenation usage after DCD heart transplantation. OBJECTIVE: To assess whether the beating heart method of DCD heart transplantati

Mortality risk factors in patients receiving ECPR after cardiac arrest: Development and validation of a clinical prognostic prediction model.

BACKGROUND: Previous studies have shown an increasing trend of extracorporeal cardiopulmonary resuscitation (ECPR) use in patients with cardiac arrest (CA). Although ECPR have been found to reduce mortality in patients with CA compared with conventional cardiopulmonary resuscitation (CCPR), the mortality remains high. This study was designed to identify the potential mortality risk factors for ECPR patients for further optimization of patient management and treatment selection. METHODS: We conducted a prospective, multicentre study collecting 990 CA patients undergoing ECPR in 61 hospitals in

Left Ventricular Support for Unprotected Left Main Coronary Artery Interventions (The Dayton Heart and Vascular Impella Registry).

BACKGROUND: Coronary artery bypass grafting is the standard of care for patients with obstructive left main (LM) coronary disease. In poor surgical candidates, high-risk percutaneous coronary artery intervention (PCI) is an alternative. METHODS: We investigated a retrospective cohort of patients who underwent LM PCI from January 2010 to March 2014 (n = 89). Obstructive LM disease was defined as 50% angiographic obstruction of luminal flow, and the primary endpoint was inhospital mortality. Coronary revascularization and circulatory support strategies in patients with myocardial infarction, multi-vessel coronary artery disease, and cardiogenic shock: Insights from an international survey.

UNLABELLED: Cardiogenic shock (CS) complicating acute myocardial infarction (MI) is associated with high mortality. In the absence of data to support coronary revascularization beyond the infarct artery and selection of circulatory support devices or medications, clinical practice may vary substantially. METHODS: We distributed a survey to interventional cardiologists and cardiothoracic surgeons through relevant professional societies to determine contemporary coronary revascularization and circulatory support strategies for MI with CS and multi-vessel coronary artery disease (CAD). RESULTS:

Risk factors and outcome analysis after surgical management of ventricular septal rupture complicating acute myocardial infarction: a retrospective analysis.

BACKGROUND: Ventricular septal rupture (VSR) is an uncommon but well-recognized mechanical complication of acute myocardial infarction (AMI). The outcome of VSR remains poor even in the era of reperfusion therapy. We reviewed our experience with surgical repair of post-infarction VSR and analyzed outcomes in an attempt to identify prognostic factors. METHODS: From October 1995 to December 2013, data from 47 consecutive patients (mean age, 68 ± 9.5 years) with post-infarction VSR who underwent surgical repair at our institute were retrospectively reviewed. The preoperative conditions, morbidit

Effect of cardiogenic shock hospital volume on mortality in patients with cardiogenic shock.

BACKGROUND: Cardiogenic shock (CS) is associated with significant morbidity, and mortality rates approach 40% to 60%. Treatment for CS requires an aggressive, sophisticated, complex, goal-oriented, therapeutic regimen focused on early revascularization and adjunctive supportive therapies, suggesting that hospitals with greater CS volume may provide better care. The association between CS hospital volume and inpatient mortality for CS is unclear. METHODS AND RESULTS: We used the Nationwide Inpatient Sample to examine 533 179 weighted patient discharges from 2675 hospitals with CS from 2004 to
