The use of a paediatric speech and language therapy outcome measurement scale.

This research aimed to evaluate the inter- and intra-rater reliability of a locally developed paediatric speech and language therapy (SLT) outcome measure, the NBOM (Northern Birmingham Outcome Measures), immediately after 15 speech and language therapists (SLTs) had taken part in training then after a period of time. It was found that SLTs who had received more than the basic orientation training achieved a higher level of reliability than those who had not but after 5 months training effects were fading. It seems that as well as requiring training and practice on the scales, SLTs also needed…

Why are there so few ethnic minority speech and language therapists?

This report summarises the outcome of a pilot study to investigate the recruitment of ethnic minority students to speech and language therapy, physiotherapy and dietetics courses. The study also sought to identify factors contributing to the low representation of these students on the named courses through interviews and questionnaires to careers advisors, school students and course admissions tutors. Recommendations for changes in practice are made.

An effective model for a speech and language therapy service in mainstream schools.

Health and Local Education Authority (LEA) personnel met in 1993 to consider how to meet the expanding caseload of children, needing speech and language therapy support in mainstream schools. The drive towards an inclusive policy for all children with special needs led to the joint funding of a peripatetic team of speech and language therapists (SLTs) and assistants. The preparatory training programmes for the team and school based staff focused on ensuring that differing perceptions, expectations and skills were acknowledged. Evaluation of the model has examined throughput, the opinion of con…

Deficits in auditory temporal and spectral resolution in language-impaired children.

Between 3 and 6 per cent of children who are otherwise unimpaired have extreme difficulties producing and understanding spoken language. This disorder is typically labelled specific language impairment. Children diagnosed with specific language impairment often have accompanying reading difficulties (dyslexia), but not all children with reading difficulties have specific language impairment. Some researchers claim that language impairment arises from failures specific to language or cognitive processing. Others hold that language impairment results from a more elemental problem that makes affe…

Anterior translocation of language in patients with left cerebral arteriovenous malformation.

We studied seven patients with left cerebral atriovenous malformation (AVM) with superselective arterial injection of anesthetics during angiography to determine whether there was translocation of some language functions to other regions in the ispilateral hemisphere. All patients were right handed. With a catheter inserted into each target vessel, patients underwent aphasia examination in an A-B-A design: (A) baseline, no anesthetic; (B) 1 minute after anesthetic injection; and (A) 12 minutes after injection (when its effects had dissipated). The results showed that six of seven patients had …

A grammatical specific language impairment in children: an autosomal dominant inheritance?

The aim of this study is to provide further characterization of a subgroup of so-called "Grammatical specific language-impaired (SLI)" children. The Grammatical SLI children have a persistent and disproportionate impairment in grammatical comprehension and expression of language. Previous research has indicated that their language impairment may be characterized by a domain-specific and modular language deficit. This study provides an initial investigation as to whether there is a genetic basis underlying their disorder as has been found for other forms of SLI and for SLI in general. The incid…

Theory and practice in speech-language pathology: a review of systemic principles.

Linear, cause-effect reasoning has dominated the training and clinical practice of speech-language pathology for many years. Recently, however, the roles of many speech-language clinicians and the contexts in which they provide their clinical services have been changing, and traditional ways of viewing clinical work no longer seem to apply. This article applies principles of systems theory to such changes and advocates an integration of these principles with those of traditional, linear views to achieve a better understanding of this evolution in the clinical practice of speech-language pathol…

On the benefits of direct teaching of spelling in children's language arts instruction.

Three studies involved 5 children, ages 6 to 8 years, who were experiencing difficulty in learning skills in reading. Each participated in several spelling tasks which led to improved reading skills, perhaps because stimulus were formed.

Morphological usage and awareness in children with and without specific language impairment.

In order to investigate the relationship between oral language usage and morphological awareness, 5- to 7-year-old children with specific language impairment (SLI) were compared to age-matched (AM) and language-matched (LM) comparison groups on a variety of measures requiring metalinguistic skill. These included sentence completion (involving real and nonsense words); comprehension of inflected non-words; response to morphological errors (including judgment, identification, and repair), and deliberate creation of grammatical violations. Overall, the SLI children performed significantly worse t…

The human language faculty: genetically determined? An approach to testable hypotheses.

A kind of generation gap seems to divide researchers interested in the evolutionary status of the human language capacity. There is still opposition to the theory of a genetically determined universal or primal grammar. But although not well acquainted with the transformational-generative linguistic methodology that led to the hypothesis, an increasing number of linguists base their work with a "new grammar" on the acceptance of innateness of the human language faculty. It is proposed that dyslexia offers a hypothesis testable with the new advanced gene mapping, and that Creole Languages and S…

[Alexia without agraphia in the Arabic language. Neurolinguistic and and MRI study].

A 33 year-old woman developed an alexia without agraphia, a color anomia, a right hemianopia, an aphasic amnesia and a verbal amnesia. The brain MRI showed the lesions in the left splenium of corpus callosum, forceps major, optic radiations and anterieur temporal lobe. The fact that she measured writing comprehension and had complete recovery of reading impairment despite the persistence of anatomic lesions plead in favour of an active participation of the right hemisphere (RH) on reading; this capacity of the RH may be due to the linguistic particularities of arabic writing.

Advances in burn care management: role of the speech-language pathologist.

Because the inclusion of the speech-language pathologist in a burn management team is not widely practiced, we discuss our successes as members of a burn team. We also review speech-language evaluation and treatment strategies and present two patients with head and neck burns who gained from our intervention.

Case study of language and numerical disability: A sequential processing deficit?

We report a case study of Christine, an intelligent 30-year-old woman with a developmental learning disability. Psychometric evaluation and extensive interviews revealed several findings: most notably, evidence of anomia, auditory processing problems, difficulty acquiring reading and spelling skills, and an extremely poor sense of number. In addition, Christine showed all four of the Gerstmann symptoms (left-right confusion, finger agnosia, dysgraphia, and dyscalculia). We suggest that most, if not all, of these impairments may be explained in terms of a sequential processing deficit (e.g., Ba

The radiology of juvenile rheumatoid arthritis. A review of the English language literature.

The radiologic abnormalities seen in patients with juvenile rheumatoid arthritis (JRA) include disturbances of growth, various types of joint destruction, abnormalities of bone density, periostitis, and soft tissue abnormalities. We review the English language literature, which deals with the radiologic abnormalities in general, and at specific sites. We also review briefly radiologic abnormalities seen in other organ systems. The role of the other imaging modalities in the assessment of JRA is discussed.

[Curriculum development in speech-language therapy and audiology: basis and principles].

The aim of this article is to discuss some of the important issues that form the basis of curriculum development for education in speech-language therapy and audiology. The demands of the profession are such that a horizontal occupational structure is no longer adequate in the multicultural, multilingual RSA context. It is therefore necessary to investigate alternative educational options to accommodate current needs. These may include diploma training for technicians, certificate programmes for community rehabilitation workers and professionally directed masters courses. It is also apparent t…

Anatomical correlates of normal and impaired language in a set of dizygotic twins.

This report presents findings for a set of dizygotic twins at age 4 years 9 months. The male was diagnosed as specifically language impaired. For both children, the left-right perisylvian configuration was atypical. Only the male's configuration was symmetrical, a finding in line with autopsy data reported for subjects with "developmental dyslexia" who also may have had a form of language impairment. In addition, the male had an atypical (L greater than R) configuration of the cerebral hemispheres, a finding not seen in his twin, or in a series of volunteers without a history of developmental …

Involving relatives in aphasia therapy: an application of language enrichment therapy.

A programme of therapy for aphasia devised in Finland, Language Enrichment Therapy, was tested in an English version, particularly in respect of its co-operative use by speech therapists and the patients' relatives at home. Although a number of changes were recommended, the method was approved of by therapists and relatives. The majority of patients showed improvement, as assessed by the Western Aphasia Battery, over a period of less than three months with a regimen of one hour a week with the therapist supplemented by an average of five hours of help by a relative at home. These results shoul…
