Remimazolam combined with propofol for intraoperative wake-up testing in scoliosis correction surgery: a prospective, blind, randomized controlled trial.

PURPOSE: Remimazolam is a fast-acting benzodiazepine used for anesthesia and sedation, notable for its quick onset and reversible effects. I … uicker wake-up times, better wakefulness quality, and higher satisfaction, without affecting spinal neurophysiological monitoring. This suggests it as a promising sedative option for spinal deformity surgeries. CLINICAL TRIAL REGISTRATION: https://www.chictr.org.cn/bin/project/edit?pid=203900 as ChiCTR2300076802.

Anterior clivectomy: surgical technique and clinical applications.

OBJECT: Midline clival lesions, whether involving the clivus or simply situated anterior to the brainstem, present a technical challenge for … de clivectomy windows by drilling the clivus between anatomical landmarks. Bilateral intraoperative neurophysiological monitoring was used (somatosensory evoked potentials, brainstem auditory evoked responses, and cranial nerves VI-XII). RESULTS: Of the 43 patients, 26 were female and 17 were male, and they ranged in age from 3.5 to 76 years (mean 41.5 years). Thirty-eight pa …

Mapping the Digital Mind: A Meta-Analysis of EEG Biomarkers in Cognition, Emotion, and Mental Health.

Background: Electroencephalography (EEG) provides millisecond-resolution measurements of neural activity, offering a unique potential to identify biomarkers of cognition, emotion, and mental health. However, the proliferation of methodologically diverse studies necessitates systematic synthesis to establish the reliability and clinical utility of proposed EEG biomarkers. Methods: Following PRISMA 2020 guidelines, we systematically searched PubMed, PsycINFO, Web of Science, and Scopus for studies published 2015-2025 examining EEG correlates of cognitive control, learning, emotion regulation, an

Role of Multimodal Use of Surgery Adjuncts for Overall Survival of Patients with Supratentorial WHO Grade 4 Gliomas.

BACKGROUND: In recent years, numerous advanced techniques supporting surgery (TSS) have been utilized to maximize glioma resection. However, the impact of the multimodal use of TSS, such as awake surgery, intraoperative neurophysiological monitoring, neuronavigation, fluorescence-guided surgery, functional magnetic resonance imaging m(fMRI), and diffusion tensor tractography (DTT), on improving overall survival (OS) remains uncertain. METHODS: In retrospective analysis of 81 patients with supratentorial WHO grade 4 gliomas, we assessed the effect of age, sex, tumor location, number of TSS use…

Reversible Causes of Transitory Motor Evoked Potential Decrease During Posterior Spine Fusion in Rapidly Progressive Scoliosis Treatment: A Case Report.

INTRODUCTION: Idiopathic adolescent scoliosis (IAS) is commonly managed non-surgically; however, patients with a Cobb angle >45° before skeletal maturity often require posterior spinal fusion. Because this procedure carries a risk of neurological complications, intraoperative neurophysiological monitoring (IONM) is essential for early detection of spinal cord compromise. CASE REPORT: We present a 13-year-old girl with rapidly progressing scoliosis (Cobb angle 78°) who developed intraoperative changes in motor evoked potentials (MEPs) during posterior fusion from L4 to Th2. Total intravenous a…

Neurophysiological assessment of disease severity in Friedreich's Ataxia: a study of brainstem auditory and visual evoked potentials.

OBJECTIVE: To evaluate a multimodal assessment including clinical rating scales, brainstem auditory evoked potentials (BAEPs), pattern reversal visual evoked potentials (p-VEPs), and serum biomarkers as candidate measures of disease severity in longstanding Friedreich's ataxia (FRDA). METHODS: In a prospective, monocentric cohort, we conducted a cross-sectional baseline assessment of 29 genetically confirmed adult FRDA patients. Disease severity was rated with modified Friedreich Ataxia Rating Scale (mFARS). All underwent BAEPs and/or p-VEPs; available serum markers (NfL, NfH, GFAP, IL-6) wer

Advanced neurosurgical intervention of recurrent tethered cord syndrome with split cord malformation type 1: an illustrative case.

PURPOSE: Recurrent tethered cord syndrome (TCS) with split cord malformation (SCM) is a rare presentation with distinct surgical challenges … scarred neural tissue. Preoperative planning with multidisciplinary coordination and intraoperative neurophysiological monitoring is essential to optimize outcomes. This report contributes to the limited literature on recurrent SCM management in adults and reinforces the importance of complete initial intervention and longitudinal follow-up in complex spinal dysraphisms.

Pain alleviation in diabetic neuropathy using hybrid closed loop insulin pumps (PAINLESS) - A randomised crossover trial protocol.

INTRODUCTION: Painful diabetic neuropathy (PDN) is a common complication of diabetes that leads to significant co-morbidity. Emerging evidence suggests glycaemic variability may be linked to fluctuations in pain intensity. Hybrid closed loop (HCL) systems reduce glycaemic variability, raising the possibility that they may modify pain intensity. This study aims to evaluate the effects of HCL use on pain intensity in people with PDN. MATERIALS AND METHODS: PAINLESS is a two-centre, randomised cross-over study, recruiting 49 adults with type 1 diabetes and PDN. Each participant will complete two

A novel approach for the EEG-driven assessment of divided attention through mutual information theory: A case study at the wheel.

Monitoring how attention is distributed across concurrent task demands is fundamental in daily life contexts such as mobility, education and industrial control. However, existing neurophysiological measures, typically based on univariate EEG markers, are sensitive to generic cognitive load, visual complexity, or motor activity, and therefore lack specificity for attentional splitting. Here, we introduce the Attentional Split Index (ASI), a novel EEG-based metric that employ Mutual Information (MI) theory, designed to quantify the coordinated modulation of multiple neurometrics that emerges whe

Differentiation with electroencephalography microstate in temporal lobe epilepsy with and without cognitive decline.

OBJECTIVE: Electroencephalography (EEG) microstate analysis is widely used in the study of various neurological and psychiatric disorders. We aim to assess whether EEG microstates are altered in TLE patients with and without cognitive decline. METHODS: This study included a total of 47 temporal lobe epilepsy (TLE) patients with or without cognitive decline and 14 healthy controls (HCs). All participants underwent 64-channel EEG monitoring. Two-minute epochs of preprocessed eye closed awake EEG data were extracted for microstate analysis. Participants were divided into groups based on intellig

Temporal lobe epilepsy with isolated amygdala enlargement: anatomo-electro-clinical features and long-term outcome.

BACKGROUND: Temporal lobe epilepsy with isolated amygdala enlargement (TLE-AE) still lacks a definite characterization and controversies exist. METHODS: We conducted a retrospective study identifying brain MRI scans with isolated AE between 2015 and 2021. We collected clinical and paraclinical data of patients with TLE-AE and evaluated the outcome. RESULTS: Forty-one subjects were included (20 males; AE: right 13; left 24; bilateral 4). A strong correlation was found between AE and MRI T2-hyperintensity (right: p < 0.005; left: p < 0.003). There was no history of febrile seizures; 85,4% had
