In this study, we documented the prevalence of coronal axis malalignment in a series of 93 tibias (from 54 patients) lengthened with monolateral fixation. The average length obtained by distraction osteogenesis was 8.9 cm (range, 3.5-15.6) or 38% of the original bone length (range, 11-78%). Osteogenesis imperfecta is an hereditary defect in the synthesis of collagen fibres. The disease can cause cardiac defects, such as aortic and mitral incompetence. Surgical repair of these defects is complicated by tissue friability and bleeding problems. We describe a case where an incompetent aortic valve was successfully replaced by a mechanical prosthesis.

Osteogenesis imperfecta (OI) is an uncommon congenital disease of collagen synthesis. It has a wide range of phenotypic expressions, but it is classically characterized by varying degrees of bone fragility. The chance of a woman with osteogenesis imperfecta to succeed in a pregnancy with a healthy baby and without any harm to herself, is poor. A case of a 37-year old patient is presented, who consciously became pregnant twice, despite her severe illness. Both pregnancies finally resulted in Caesarean section, but without any complications to the patient. While the first child, a boy, is healthy, the second, a girl, is expected to suffer from phosphate diabetes since the father suffered from this illness.

Fourteen multilevel osteotomies with Rush pin fixation have been performed in 5 children with osteogenesis imperfecta. The age of patients at the operation ranged from 6.5 to 15.5 years. Plaster of Paris immobilization never exceeded 8 weeks. An axial correction, improved ambulating and equalization of the limbs have been achieved in all cases. Complications of the method included varus deformity of the proximal tibia (1 case), migration of the pin (1 case) and supracondylar fracture of the femur at the level of pin end (2 cases).

Osteogenesis imperfecta is an inherited disorder of collagen synthesis. It has a wide range of phenotypic expressions, but cardiovascular anomalies tend to be rare. When they do occur, they usually consist of aortic or mitral valve disease. We report an autopsy case of a 36-week gestation infant with coexisting osteogenesis imperfecta and Ebstein's anomaly. The simultaneous occurrence of two relatively rare entities may reflect a generalized expression of an underlying collagen synthesis defect.

Early prenatal diagnosis of osteogenesis imperfecta at 14 weeks' gestation by transvaginal sonography is reported. An anencephalic fetus was found at autopsy, although on sonographic examination complete brain tissue had been observed. A case of osteogenesis imperfecta with aortic regurgitation is described. The patient had a dilated aortic valve ring and an aneurysm of the Sinus of Valsalva. The patient manifested severe hemodynamic abnormalities and underwent aortic root reconstruction using a valved conduit. The operative problems and the pathological findings are discussed.

Intramedullary extendable Bailey-Dubow nails were used for treatment of lower extremity deformities resulting from osteogenesis imperfecta. Sixteen patients had 56 nails placed in 48 long bones, including 18 revisions. All patients were braced postoperatively. The average duration of follow-up was 4.8 years.
