Studies on ataxia of lumbar origin in cases of vertigo due to whiplash injury.

We draw the following conclusions from clinical observations on traumatized patients with vertigo and lumbar pain. 1) Hypertonicity of the lumbar soft supporting tissues, and especially the lumbar erector muscles (i.e., over-excitment of the lumbar proprioceptors) is a cause of vertigo due to whiplash injury. Equilibrium tests after fixing the waist with a corset are useful for detecting ataxia of lumbar origin. 2) Over-excitement of the lumbar porpiroceptors and dysfunction of the brain stem and the cerebellum are closely related in bringing about vertigo due to whiplash injury. The lumbar pr

Biphasic modulation of large-fiber excitability accompanied by dynamic nerve-intrinsic immune activation following CFA-induced peripheral inflammation in mice.

Peripheral inflammation is known to enhance pain sensitivity through nociceptive pathways, but its impact on large-diameter myelinated fibers, which mediate touch and proprioception, is less understood. Here, we examined the temporal dynamics of large fiber (Aα/β) excitability in a mouse model of Complete Freund's Adjuvant (CFA)-induced hind paw inflammation, integrating behavioral, electrophysiological, proteomic, and immune analyses. CFA induced a biphasic modulation of large fiber function, with early hypoexcitability on day 1 followed by hyperexcitability on day 7, temporally aligned with …

Clinical Reasoning: A 28-Year-Old Man with Subacute-Onset Ataxic Gait.

The diagnostic approach to subacute progressive polyneuropathy can be challenging, particularly when accompanied by systemic features. We pr … remities. Sensory testing demonstrated a stocking-glove distribution of hypoesthesia. Vibration and proprioception were decreased in all four limbs. Romberg sign was positive, and he had a broad-based ataxic gait with bilateral steppage. Nerve conduction studies demonstrated a demyelinating sensorimotor polyneuropathy. Initial laboratory and imaging studies show …

Hoover's sign is a sign of functional weakness: or is it more?

Hoover's sign has long been used to support a clinical diagnosis of functional weakness. However, it also occurs in people with pain, other … iscussed. This illustrates the importance of sensory processing (especially sensory attenuation and proprioception) and learning/habituation to stimuli, which is known to be altered in functional neurological disorder and other functional disorders.Reframing Hoover's sign by linking it to agency and extending it to other clinical scenarios could help clinicians, …

PIEZO Ion Channels in Cardiovascular Functions and Diseases.

The cardiovascular system provides blood supply throughout the body and as such is perpetually applying mechanical forces to cells and tissues. Thus, this system is primed with mechanosensory structures that respond and adapt to changes in mechanical stimuli. Since their discovery in 2010, PIEZO ion channels have dominated the field of mechanobiology. These have been proposed as the long-sought-after mechanosensitive excitatory channels involved in touch and proprioception in mammals. However, more and more pieces of evidence point to the importance of PIEZO channels in cardiovascular activiti…

Sports and Children with Hemophilia: Current Trends.

Hemophilia is a sex-linked recessive disorder characterized by a lack of blood factors necessary for clotting. This review aims to investigate the benefits of sports activities in children with hemophilia in terms of both physical and psychological wellness. Sports activity is necessary for children with hemophilia to preserve joints' range of motion, reduce joint bleeding, improve muscle mass and strength, enhance proprioception and prevent secondary chronic diseases. In the past, high-impact sports were usually forbidden in children with hemophilia because of their high bleeding risk. Recent…

Effects of cognitive load and prosthetic liner on volitional response times to vibrotactile feedback.

Artificial tactile feedback systems can improve prosthetic function for people with amputation by substituting for lost proprioception in the missing limb. However, limited data exists to guide the design and application of these systems for mobility and balance scenarios. The purpose of this study was to evaluate the performance of a noninvasive artificial sensory feedback (ASF) system on lower-limb function in the presence of a cognitive load and a liner interface. Reaction times (RTs) and accuracy of leg-movement responses to vibratory stimuli at the thigh were recorded for 12 nondisabled i…

Failure of knee osteotomy in a case of neuropathic arthropathy of the knee.

Neuropathic arthropathy (Charcot's joint) is a degenerative process that affects peripheral or vertebral joints as a consequence of a disturbance in proprioception and pain perception. The knee is one of the most frequently affected joints, but even when the diagnosis is made at an early stage there is no consensus on the best treatment options. An early diagnosis of neurosyphilis was made in a 55-year-old woman presenting a delayed union of an asymptomatic Schatzker type IV fracture of the proximal tibia. A medial opening wedge tibial osteotomy was performed to achieve fracture healing, to co…

Recent progress in physical therapy of the upper-limb rehabilitation after stroke: emphasis on thermal intervention.

Poor recovery of arm function after stroke can often have a negative impact on the patient and his/her family. These patients often need assistance from the society and may need to rely on government resources. Numerous therapeutic treatments are currently available for stroke rehabilitation. Traditional rehabilitation strategies (Bobath, Brunnstrom, proprioception neuromuscular facilitation, and motor relearning) have been used for many years. However, few of these interventions have been tested in clinical trials and are thus practiced on an empirical basis. Various evidence-based therapies …
