Fixation of Ultrasmall Proximal Pole Scaphoid Fractures Using Bioabsorbable Osteochondral Fixation Nails.

Surgical treatment of ultrasmall proximal pole scaphoid fractures poses a number of challenges. The fragment may be too small to fix with conventional headless screw fixation owing to the high risk of fragmentation upon insertion. However, excision of the fragment risks compromising the scapholunate ligament complex and also exposes the radioscaphoid joint to potential degenerative changes. Under these circumstances, we describe the use of bioabsorbable osteochondral fixation nails that allows stable low-profile fixation while minimizing the risk of fracturing the proximal pole.

Medial Femoral Trochlea Osteochondral Flap: Applications for Scaphoid and Lunate Reconstruction.

Vascularized osteochondral flaps are a new technique described for the reconstruction of challenging articular defects of the carpus. The medial femoral trochlea osteochondral flap is supplied by the descending geniculate artery. This osteochondral flap has shown promise in the treatment of recalcitrant scaphoid proximal pole nonunions and advanced avascular necrosis of the lunate. The anatomy, surgical technique, and results are discussed, with clinical cases provided.

Efficacy of Magnetic Resonance Imaging and Clinical Tests in Diagnostics of Wrist Ligament Injuries: A Systematic Review.

PURPOSE: To investigate the diagnostic performance of magnetic resonance imaging (MRI) and clinical provocative tests on injuries to the triangular fibrocartilage complex (TFCC), the scapholunate (SL) ligament, and the lunotriquetral (LT) ligament. METHODS: An electronic literature search of articles published between January 1, 2000, and February 28, 2014, in PubMed, Embase, and the Cochrane Library was carried out in April 2014. Only studies of the diagnostic performance of MRI and clinical provocation tests using wrist arthroscopy as the gold standard were eligible for inclusion. The Prefe

Radiocarpal fusion.

Limited wrist fusion is a common and often effective method of treatment for many painful wrist conditions. When post-traumatic, inflammatory and noninflammatory arthritis affects only the articular surfaces of the proximal carpal joint, a limited radiocarpal fusion can be considered. Specific indications are painful arthritis following distal radial fractures, rheumatoid arthritis with ulnar shift of the carpus, scapholunate instability with radioscaphoid arthritis, and stage IV Kienbock's disease. It is necessary for the midcarpal joint surfaces to be essentially normal. Either a radioscapho

Magnetic resonance imaging detection of avascular necrosis in carpal bones despite apparent fracture union: a case series.

Assessment of fracture healing in carpal bones is commonly performed using computed tomography (CT). However, CT may not accurately reflect bone viability. Magnetic resonance imaging (MRI) is more sensitive in detecting early changes in bone marrow and vascularity. A diagnostic challenge arises when CT suggests fracture union while MRI demonstrates features of avascular necrosis (AVN). We conducted a retrospective case series of three patients with prior carpal bone fractures (one scaphoid and two lunate) who presented with persistent or new-onset wrist pain following apparent fracture healing

Comparative outcome analysis of internal screw fixation and Kirschner wire fixation in the treatment of scaphoid nonunion.

In cases with difficult scaphoid screw placement due to small, fragile bone fragments, or transplants and insufficient space, the internal Kirschner wire fixation is a fallback option; however, controversy remains regarding its stability, safety, and outcome. Between 2001 and 2011, 95 patients were treated in our center (n = 80 with cannulated compression screws and n = 15 with Kirschner wires), and retrospectively analyzed. The outcome measurements included the analysis of patient data, union rate and analysis of functional measures, and quality of life. Bony reconstructions were performed wi

Chronic transscaphoid, transcapitate perilunate fracture dislocation of the wrist: Fenton's syndrome.

The authors report about chronic Fenton's syndrome. This rare injury of the wrist is characterized by scapho-capitate fracture accompanied by 180° rotation of the head of capitate and associated perilunate dislocation. Two patients suffering from chronic Fenton's syndrome were treated with pyrocarbon capitate resurfacing prosthesis. Patients were evaluated according to the wrist range of motion, Mayo modified wrist and DASH scores. In conclusion, prosthetic surgery may achieve satisfactory results for this rare and diagnostically challenging syndrome.

Ultrasonic Anchor-Assisted Restoration of the Scapholunate Ligament Complex: A Cadaveric Study.

PURPOSE: Scapholunate interosseous ligament injuries pose a substantial challenge in hand surgery because of the risk of altered carpal biomechanics and progressive carpal collapse. Although numerous repair techniques exist, an optimal fixation method remains elusive. Advances in ultrasonic technology-transitioning from diagnostic to implant applications-offer potential benefits including enhanced pullout strength, ease of insertion, and reduced bony disruption. This study evaluated the efficacy of an ultrasonic fusion anchor in restoring scapholunate (SL) alignment in a cadaveric model. METH

Surgical outcomes in chronic perilunate dislocations: A systematic review.

Perilunate dislocations and perilunate fracture-dislocations are marked by a wide range of high energy trauma to the wrist. Chronic perilunate injuries are defined by a treatment delay of at least 6 weeks. This systematic review aims to compare the various operative techniques and outcomes for chronic perilunate injuries. A systematic review of literature on chronic perilunate dislocations and perilunate fracture-dislocations were performed according to Preferred Reporting Items for Systematic Reviews and Meta-analysis statement guidelines using PubMed, Embase, Scopus, and Web of Science. Novel Treatment of Post-Traumatic Scaphoid Osteonecrosis in a Pediatric Patient Utilizing Temporary Scaphocapitate Fixation: A Case Report.

Avascular necrosis or osteonecrosis of the scaphoid following fracture is extremely rare in the pediatric population with no concrete treatment protocol established in the literature. In this case report, we present an 11-year-old boy with a scaphoid waist fracture nonunion complicated by proximal pole osteonecrosis. Due to the severe fragmentation of the scaphoid, fixation across the nonunion site was not feasible, and we instead used temporary scaphocapitate fixation. The patient ultimately had radiographically confirmed bony union and full wrist range of motion at 3 months post-op.

One-Year Outcomes of the Anatomical Front and Back Reconstruction for Scapholunate Dissociation.

PURPOSE: Anatomical front and back (ANAFAB) reconstruction addresses the critical volar and dorsal ligaments associated with scapholunate dissociation. We hypothesized that patients with symptomatic, chronic, late-stage scapholunate dissociation would demonstrate improvements in all radiographic parameters and patient-reported outcomes (PROMs) after ANAFAB reconstruction. METHODS: From 2018 to 2021, 21 ANAFAB reconstructions performed by a single surgeon were followed prospectively, with 20 patients having a minimum follow-up of 12 months. In total, 17 men and four women were included, with a

Clinically Significant Treatment Delay in Pediatric Scaphoid Fractures.

PURPOSE: Pediatric scaphoid fractures present to treatment in a delayed manner 8% to 29% of the time. The indications for cast immobilization in this population are not clear. The definition of a clinically important treatment delay is based only on anecdotal reports. Successful treatment with a cast may be more desirable than surgical intervention. However, it remains unclear what clinical and radiographic factors may predict success with casting. METHODS: A retrospective analysis of all scaphoid fractures treated at a single pediatric hospital was performed to identify fracture characterist

Two-, Three-, or Four-Corner Arthrodesis for Midcarpal Osteoarthritis: A Systematic Review and Meta-Analysis.

PURPOSE: Several limited midcarpal arthrodeses have been used in the treatment of midcarpal osteoarthritis as part of scapholunate advanced collapse and scaphoid nonunion advanced collapse. There is no consensus on whether two-carpal arthrodesis (2CA), three-carpal arthrodesis (3CA), bicolumnar arthrodesis, or four-carpal arthrodesis (FCA) results in the best outcomes. The objective of this study was to determine whether there is a difference in outcomes in patients undergoing FCA, 3CA, 2CA, or bicolumnar arthrodesis for midcarpal osteoarthritis. METHODS: A systematic review and meta-analysis

Scaphoid fracture displacement is not correlated with the fracture angle.

Classifications of scaphoid fractures associate the angle of the fracture with its stability. To examine this assumption, we measured acute scaphoid fracture angles and inclinations in relation to different scaphoid axes, using fracture displacement as an indicator of instability. We examined the effect of using different axes on the measurements of angles. CT scans of 133 scaphoid fractures were classified according to the location of the fractures. Using a three-dimensional computer model, we computed four scaphoid axes. For each fracture, we then measured the fracture angle and the directio

Factors associated with surgeon recommendation for additional cast immobilization of a CT-verified nondisplaced scaphoid waist fracture.

INTRODUCTION: Data from clinical trials suggest that CT-confirmed nondisplaced scaphoid waist fractures heal with less than the conventional 8-12 weeks of immobilization. Barriers to adopting shorter immobilization times in clinical practice may include a strong influence of fracture tenderness and radiographic appearance on decision-making. This study aimed to investigate (1) the degree to which surgeons use fracture tenderness and radiographic appearance of union, among other factors, to decide whether or not to recommend additional cast immobilization after 8 or 12 weeks of immobilization;

Detecting scaphoid fractures in wrist injury: a clinical decision rule.

INTRODUCTION: The aim of this study was to develop and validate an easy to use clinical decision rule, applicable in the ED that limits the number of unnecessary cast immobilizations and diagnostic follow-up in suspected scaphoid injury, without increasing the risk of missing fractures. METHODS: A prospective multicenter study was conducted that consisted of three components: (1) derivation of a clinical prediction model for detecting scaphoid fractures in adult patients following wrist trauma; (2) internal validation of the model; (3) design of a clinical decision rule. The predictors used w
