Hydatid disease is caused by the parasite Echinococcus granulosus. The liver and the lungs are common sites. When a cystic lesion is seen on CT scan, diagnosis is made based on the patient having lived in an endemic area. Serologic tests are used for differential diagnosis. Medical treatment is centered on albendazole. Surgery is recommended either by open or endoscopic technique depending on the characteristics of the cysts and the patient. Complications of surgery are rare except for prolonged air leaks. Mortality occurs when the cyst is located in the central nervous system or occludes majo…

PURPOSE: To report symptoms and signs from an otherwise healthy man with photopsia and monofocal outer retinitis occurring 3 days after the onset of hand, foot, and mouth disease.
METHOD: Case report.
RESULTS: Ophthalmic examination revealed a single circumscribed area of pigment epithelial mottling in the left eye and a corresponding nonarcuate absolute scotoma. Fluorescein angiography demonstrated attenuation of the retinal pigment epithelium. Serologic tests at 1 month after the onset disclosed serum antibody to enterovirus.
CONCLUSION: The close association between the cutaneous eruptio…

A previously healthy 4-year-old boy presented with typical measles and demonstrated lesions confined to basal ganglia. The clinical symptoms were an abrupt onset, impaired consciousness and mutism, extrapyramidal signs and severe neurovegetative dysfunction. No modification of the cerebrospinal fluid was observed; laboratory tests were all normal with the exception of a positive serologic test for measles. Cranial magnetic resonance imaging showed abnormal signals in the striatum, affecting the putamen and the caudate nuclei bilaterally. Neurologic improvement occurred within 2 months, with re

Techniques for detecting hepatitis E virus nucleic acid by RT-PCR and digoxin probe spot hybridization tests were developed. The examination by RT-PCR test of 250 stored stool specimens from patients with acute hepatitis E gave a positive rate of 40.9% and was positive in one stool specimen collected 28 days after the patient had contracted the illness. Serologic examination yielded a positive rate of 66.7%, and also utilizing the DIG probe labelled with PCR product it was 66.6%. RT-PCR with the digoxin probe proved to be suitable for clinical diagnosis and basic research on hepatitis E.

The systemic conditions described in this paper are (1) sarcoidosis, (2) tuberculosis, (3) toxoplasmosis, and (4) histoplasmosis. The lung is the primary target organ of granulomatous involvement for each of these disorders. This and other similarities may confuse the diagnostic picture; however, appropriate understanding of clinical, epidemiologic, pathophysiologic, immunologic, radiologic, and serologic features assists in clearly establishing a diagnosis. The goals of this paper are to help the clinician develop a working knowledge of these four conditions, as well as an understanding of wh

Infectious mononucleosis is a relatively common disease. In this paper, three patients with neurological symptoms related to infectious mononucleosis are described. Patient no. 1 had myelitis, one of the most frequent neurological complications. Patient no. 2 had cerebellar symptoms, with ataxia and disturbance of gait. In this case magnetic resonance revealed vermis atrophy, a finding which, to the best of our knowledge, has not been reported before. Patient no. 3 had bilateral optic neuritis. Repeated serologic tests may be necessary to confirm the diagnosis. Infectious mononucleosis should …

A total of 156 patients with early syphilis were treated with tetracyclin and oletetrin. The drugs were administered 4 times daily (every 6 hrs) in a dose of 0.5 g (daily dose 2.0 g). Patients with primary seropositive syphilis were treated for 20 days, the dose per course being 40 g, those with secondary fresh syphilis for 25 days, dose per course 50 g, those with secondary recurring syphilis for 40 days, dose per course 80 g. The treatment was well tolerated by all the patients. Early negative results of specific serologic tests after treatment confirm the efficacy of the suggested method.

All patients referred for gastroscopy were investigated by Campylobacter pylori culture and antibody titration. An enzyme immunoassay was developed and found to have a negative predictive value of 0.997, which indicates that serologic tests could be used for screening of patients before gastroscopy. A close correlation was found between active and inactive duodenal and gastric ulcer and positive C. pylori culture. Small open treatment trials with the antibiotics doxycycline, tinidazole, amoxycillin, and norfloxacin were ineffective for the eradication of C. pylori. Treatment with the new anti-…

Legionnaires' disease can be diagnosed by culture, by direct detection of the bacterium or its products by using immunologic means, with a DNA probe, and by serologic means. Culture diagnosis is the most specific and sensitive test. Legionellae can be isolated from sputum samples by using selective techniques. Antibody detection is more suited for epidemiologic purposes than for use in individual cases. Immunofluorescent microscopy is a useful and rapid means of diagnosis. Alternative rapid and specific tests are urinary antigen detection and the use of a DNA probe. Culture must always be perf

We report 3 cases of rickettsial hepatitis observed in the Lyon area which were secondary to boutonneuse fever. The temperature was elevated and clinical signs were an often painful hepatomegaly and digestive disorders. These signs are only indicative when accompanied by atypical pneumonopathy or exantherma. Indeed, if the inflammatory syndrome is clear, hepatic function is little perturbed. Diagnosis is sometimes influenced by the epidemiological situation, and depends on serologic tests and histological evidence of granulomatous hepatitis (2 of the 3 cases here). The successful antibiotic tr…

A seriously ill patient with diffuse abdominal tenderness of unknown cause is described. The diagnosis proved to be fulminating necrotizing amebic colitis with perforation. This case report serves as a reminder that amebiasis may occur in patients who have not been outside Canada, that it may readily be confused with other types of inflammatory bowel disease, and that particular care should be taken in obtaining a history of exposure. Before inflammatory bowel disease is diagnosed not only should the usual diagnostic tests such as stool examination and mucosal biopsy be done, but also serologi

A 24-year-old man who had been in contact with a woman who had infectious syphilis developed a painless sore on his penis. His general practitioner prescribed tetracycline (250 mg four times a day for seven days). After approximately one month he developed another painless sore at the same site as the previous one. The diagnosis of syphilis was confirmed by dark-field microscopy and serologic tests. Reinfection was thought to be unlikely because he denied having sexual intercourse after the first visit to his general practitioner. Thus, the new lesion was thought to be a case of monorecidive s…

Rats experimentally infected with Trypanosoma brucei rhodesiense developed a syndrome characterized by anemia, splenomegaly, and glomerulonephritis. Serologic evaluation revealed that the syndrome was accompanied by the presence of 3 autoantibodies--cold-active hemagglutinin, immunoconglutinin, and antibody to fibrinogen/fibrin products. Fluorescein isothiocyanate conjugated antibody tests showed the presence of fixed complement and fibrinogen on both trypanosomes and erythrocytes. All infected rats died by the ninth day of the infection with 5 animals showing signs of pulmonary involvement an

Two patients with hereditary homozygous C2 deficiency are described. They showed many similar clinical features. Both had discoid-like skin rashes and serologic abnormalities consistent with SLE, but neither had evidence of involvement of any organ other than skin. Although biopsy of these lesions revealed expected changes by light microscopy, typical immunofluorescent findings were not present. The presence of the HL-A haplotype 10, W18 was seen in both families and was associated with the C2 deficiency gene. The 2 patients were mutually nonreactive in MLC tests and both were found to be homo

The diagnosis of American tegumentary leishmaniasis (ATL), caused primarily by Leishmania braziliensis in Latin America, remains difficult due to the lack of specific serological tools. Here, we report the chemical synthesis and immunological evaluation of five structurally defined galactofuranosyl-containing oligosaccharides that mimic type II glycoinositolphospholipids (GIPLs) found on the parasite surface. The synthetic glycotopes, equipped with a terminal aminopentyl linker, were immobilized onto microarrays and tested against human sera from ATL and Chagas disease (CD) patients, as well a

American tegumentary leishmaniasis (TL) caused by Leishmania braziliensis is characterized by a spectrum of clinical presentations, ranging from localized cutaneous ulcers (CL), mucosal (ML), or disseminated (DL) disease, to a subclinical (SC) asymptomatic form. Current diagnosis based on parasite culture and/or microscopy lacks sensitivity and specificity. Previous studies showed that patients with CL and ML have very high levels of Leishmania-specific anti-α-Gal antibodies. However, the native parasite α-Gal glycotope(s) is(are) still elusive, thus they have not yet been explored for a more

Toxoplasmosis represents one of the most common comorbidity factors in solid organ or hematopoietic stem cell transplant recipients as well as in other immunocompromised patients. In the past decades, availability and performance of molecular tools for the diagnosis or the exclusion of toxoplasmosis in these patients have greatly improved. However, if accurately used, serology remains a complementary and essential diagnostic tool for physicians and medical parasitologists for the prevention and management of toxoplasmosis in immunocompromised patients as well. It is required for determination
