Perinatal outcome of fetuses with a birth weight greater than 4500 g: an analysis of 3356 cases.

OBJECTIVE: To assess the perinatal outcome in a series of macrosomic fetuses according to the intended mode of delivery, and to estimate the individual risk of shoulder dystocia and brachial plexus injury upon information available either prior the onset of labor or at delivery. STUDY DESIGN: Perinatal and postnatal information of 3356 women who delivered during a 10-year period a macrosomic fetus (>4500 g) in vertex presentation were analyzed. After the exclusion of cases with extraneous factors that may have affected the health of the neonate, patient and neonatal characteristics were compa

Surgical treatment of bipolar segmental clavicle fracture: A case report.

BACKGROUND: Shoulder injuries caused by trauma are common, including clavicle fractures. Even so, bipolar segmental fracture of the clavicle is extremely rare and seldom reported. Therefore, there is still a controversial issue about how to treat these bipolar segmental clavicle fractures. CASE SUMMARY: A 56-year-old security guard arrived at our emergency room after falling on his electric bicycle. There was no loss of consciousness or other pain. He had no numbness in his fingers. X-rays and 3D computed tomography revealed that the patient's right shoulder had a bipolar segmental clavicle f…

System Identification to Characterise Shoulder Joint Dynamics in Two Degrees of Freedom.

In this study, we present a new design of a shoulder perturbation robot that can characterise the dynamics of the shoulder in two degrees of freedom. It uses two linear electric motors to perturb the shoulder joint in internal/external rotation and abduction/adduction, and force and position sensors to measure the corresponding torque and angular displacement about the joint. System identification techniques are used to estimate the dynamics of the muscles around the joint. The advantage our apparatus offers over the existing ones is that it can efficiently transfer torque to the joint and mea

Management of cartilage defects in the shoulder.

Articular cartilage lesions of the glenohumeral joint are an especially difficult clinical problem to manage, particularly in the younger, more active patient. Left untreated, these lesions may progress in the long-term, leading to further pain and disability. While shoulder arthroplasty remains a viable option in older patients with glenohumeral arthritis, concerns over component longevity and loosening in younger patients make it less attractive in that age group. Arthroscopic joint debridement with loose body removal, often with capsular release, has been successful in select, more sedentar

Type II error in the shoulder and elbow literature.

Although the randomized controlled trial (RCT) has been recognized as the gold standard for comparing interventions, trials that fail to show a difference between 2 interventions may suffer from type II error. This study was conducted to determine if the published RCTs in the shoulder and elbow literature have sufficient power to minimize potential type II error and conclude accurately that no difference between interventions exists. We searched Medline for RCTs with negative results in Arthroscopy, the Journal of Bone and Joint Surgery (American), and the Journal of Shoulder and Elbow Surgery

Arthroscopic shoulder instability surgery. Complications.

Complications associated with arthroscopic shoulder stabilization are relatively common. Excluding recurrence, complications are rarely disabling. Current statistics undoubtedly underestimate the true incidence of complications. Many complications, including neurovascular injuries and articular damage, are preventable and can be minimized through familiarity with anatomy, proper surgical technique and instrumentation, and clinical experience. Nevertheless, despite these efforts, a few complications, including recurrent instability, persist. Despite careful patient selection and attention to la

Os acromiale in a baseball catcher.

Throwing injuries of the shoulder can result from an acute traumatic event or chronic overuse. Shoulder impingement has multiple etiologies; the most common being rotator cuff weakness/overuse and resultant glenohumeral instability. However, an uncommon cause of shoulder impingement syndrome is that of a nonfused os acromiale. There are three centers of ossification in the acromion which are usually completely fused by 18 yr of age. The most common site of nonunion is between the meso-acromion and meta-acromion. Os acromiale is reported at a rate of 14/1000 (1.4%) and is bilateral in approxima

Lateral Scapular Winging and Shoulder Pain Following Posterior Fossa Surgery: A Case Report.

The spinal accessory nerve (SAN) provides motor innervation to the sternocleidomastoid and trapezius muscles. Injury to the distal segment of this nerve can result in trapezius denervation, leading to lateral scapular winging (LSW), shoulder pain, and impaired arm elevation. While iatrogenic SAN injury most commonly occurs during cervical lymph node biopsies and neck dissections, it may also arise following posterior fossa neurosurgical procedures due to prolonged traction, compression, or mechanical stress during surgical positioning or, less commonly, accidental direct ligation. Recognizing

Breakdance injuries and overuse syndromes in amateurs and professionals.

BACKGROUND: Serious injuries due to breakdancing have been presented only as singular case reports to date. So far, there have been no comprehensive studies about injuries in this sport. HYPOTHESIS: Professional breakdancing might lead to a higher incidence of injuries than amateur training. Wearing safety equipment is correlated with a decreased incidence of injuries and pain. STUDY DESIGN: Descriptive epidemiology study. METHODS: The retrospective study surveyed 40 breakdance professionals and 104 amateurs by questionnaire. RESULTS: There were 1665 injuries and 206 overuse syndromes foun

Propulsion kinetics of recumbent handcycling during high and moderate intensity exercise.

People with spinal cord injuries (PwSCI) are at high risk of developing cardiovascular disease (CVD). While regular exercise can reduce risk of CVD, PwSCI face various barriers to exercise, including high rates of upper limb injuries, especially in the shoulder. Handcycling high intensity interval training (HIIT), which consists of periods of high intensity exercise followed by rest, is a potential exercise solution, but the musculoskeletal safety of HIIT is still unknown. In this study, we characterized three-dimensional continuous applied forces at the handle during handcycling HIIT and mode

[A temporal head injury involving intracranial penetration by glass].

The authors report a rare case of intracranial glass injury due to a temporal head injury. This 72-year-old man slipped on a bathroom floor, impacting a glass door with his head and right shoulder. His right temporal scalp and right shoulder were cut by the broken glass. He visited our emergency unit four hours after sustaining the injury. Physical and neurological examinations showed no abnormalities except for two lacerated wounds on both the right temporal scalp (1.5 cm) and the right shoulder skin (10 cm). Foreign bodies were not palpable around the lacerated wounds. Skull X-ray and CT stu

Is macrosomia predictable, and are shoulder dystocia and birth trauma preventable?

OBJECTIVE: To assess our ability to detect macrosomic fetuses, and to examine the relationship between prenatal diagnosis of macrosomia and the incidence of shoulder dystocia and birth trauma. METHODS: We instituted a protocol for routine detection of macrosomic fetuses, defined as weight estimated to be at least 4500 g. Fetal weight was estimated by ultrasonography when there was clinical suspicion of macrosomia. We collected data on these pregnancies as well as on deliveries of macrosomic infants, shoulder dystocia, and birth trauma. RESULTS: During the 14-month study period, there were 44

[Rehabilitation patients after fractures of upper limb].

Rehabilitation after fractures of upper limb is very complicated and should be coordinated with orthopedic procedures. A choice of method and schema is depended on general health status of patient and used orthopedic treatment. This process should start immediately after surgeon's intervention. An aim of rehabilitation is improvement of range movement, straight of muscles and acceleration of callus formation. It is very important to restore proprioception in a region of trauma. An intensity and velocity of rehabilitation is the greatest after extra bone consolidation and the slowest after cons

Axillary artery transection and bilateral pulmonary embolism after anterior shoulder dislocation: case report.

INTRODUCTION: Anterior shoulder dislocation can be associated with vascular and neurological complications. However, axillary artery injury associated with shoulder dislocation is rare and extremely rare without bone fracture. An early diagnosis of these complications allows predicting long-term functional outcomes. METHODS: This article reports the case of a 66-year-old patient who presented an anterior shoulder dislocation after a ski fall without any neurological dysfunction or pulse deficit. RESULTS: The first reduction attempts were unsuccessful and during the new attempt, we observed a

The Retrograde Left Atrial Tumour Embolism In Thepatient With Metastatic Extremity Chondrosarcoma.

A 34 years old male, labourer by occupation was brought to our trauma centre in an inebriated state with a history of fall from a height of approximately 10 feet. He complained of pain in the right elbow. A complete examination of the affected upper limb revealed an ipsilateral anterior shoulder dislocation and a posterior elbow dislocation with no other concomitant injuries. Both the dislocations were reduced conservatively and the patient was successfully managed. The patient is absolutely fine now, 6 months post his dislocation and has resumed back to his work. This case has been presented
