Sickle Cell disease is a major genetic disorder in tropical Africa; its severity is often ameliorated by the presence of high levels of HbF, which is genetically determined. HbF was assessed in healthy Nigeria adults using the two minutes alkali denaturation method of Betke. The subjects studied included twenty-six males and twenty-four females all with HbA this was compared with twenty heterozygotes (HbAS). The mean HbF of the entire subject studied was 2.5 +/- 1.7% (range 0.4-12.8%). The mean value for Individuals with HbA genotype was 2.7 +/- 3.4% (range 0.4-12.8%). While the AS subject had

The sickle erythrocyte (RBC) is a pathologic RBC that contains multiple membrane abnormalities. Some of these abnormalities have been implicated in the pathophysiology of vasoocclusive crises characteristic of sickle cell disease; others have yet to be defined in terms of their clinical significance. Recent information has shown that sickle RBC adhere abnormally to cultured endothelial cells yet little is known about the ways in which sickle cells interact with model membranes of defined size and lipid composition. We investigated this phenomenon by interacting sickle RBC with artificial lipid

Identification of the molecular basis of the beta-thalassaemias and sickle cell disease (SCD) has made it clear that patients with the same beta-globin genotypes can have very variable patterns of clinical expression. Extensive biochemical and pathophysiological studies over the last 50 years have derived two major modifiers--innate ability to produce fetal haemoglobin and co-inheritance of alpha-thalassaemia, subsequently validated by family and population studies. However, these two modifiers do not explain the full clinical spectrum. Genetic studies have been successful in identifying modif

BACKGROUND: Malaria is the most common precipitating cause of crises in sickle cell disease in malaria-endemic countries. Health professionals often recommend life-long malaria chemoprophylaxis for people with sickle cell disease living in these areas. It is therefore important we have good evidence of benefit.
OBJECTIVES: To assess the effects of routine malaria chemoprophylaxis in people with sickle cell disease.
SEARCH STRATEGY: We searched the Cochrane Infectious Diseases Group Specialized Register (January 2006), Cochrane Cystic Fibrosis and Genetic Disorders Group Specialized Register

The combination of sickle cell disease crisis and thrombotic thrombocytopenic purpura has been described only a few times. Here we present the case of a patient with a hemolytic crisis due to sickle cell disease complicated by thrombotic thrombocytopenic purpura. We also review the cases previously reported and compare and contrast them, highlighting diagnostic challenges.

Sickle cell disease is a world-wide problem which has been noted to cause high tone sensorineural hearing loss. We report a case with sudden onset bilateral hearing loss which progressed to total sensorineural deafness. To our knowledge there has been no report of such a case occurring previously.

Hyphema may lead to a secondary glaucoma in some patients. Six different mechanisms by which anterior chamber blood may cause elevated intraocular pressure are described and the treatment of these different conditions is discussed. It should be noted that more than 1 mechanism may be present simultaneously and that other mechanisms unrelated to the blood may also contribute to the problem.

Malaria has had a major effect on the human genome, with many protective polymorphisms-such as the sickle-cell trait-having been selected to high frequencies in malaria-endemic regions1,2. The blood group variant Dantu provides 74% protection against all forms of severe malaria in homozygous individuals3-5, a similar degree of protection to that afforded by the sickle-cell trait and considerably greater than that offered by the best malaria vaccine. Until now, however, the protective mechanism has been unknown. Here we demonstrate the effect of Dantu on the ability of the merozoite form of the

Red blood cells (RBCs) contain large amounts of iron and operate in highly oxygenated tissues. As a result, these cells encounter a continuous oxidative stress. Protective mechanisms against oxidation include prevention of formation of reactive oxygen species (ROS), scavenging of various forms of ROS, and repair of oxidized cellular contents. In general, a partial defect in any of these systems can harm RBCs and promote senescence, but is without chronic hemolytic complaints. In this review we summarize the often rare inborn defects that interfere with the various protective mechanisms present

In a village population in N. Nigeria the Fulani form a heterogeneous group in comparison with the Hausa and Maguzawa people. It was demonst … rent pattern emerged characterised by the nutritional factor and a haemolytic factor related to the sickle cell trait. The frequency of the sickle cell trait, however, was similar in all tribes. The significance of the findings is discussed and it is suggested that the heterogeneity of Fulani is possibly due to their less complete adaptation to stable malaria.

The cytoplasmic resistivities and membrane breakdown potentials of normal (AA), sickle-cell-trait (AS), as sickle (SS) red blood cells have been measured by the biophysical methodology of resistive pulse spectroscopy over a range of osmolalities. At isotonicity, the average membrane breakdown potentials are virtually identical for the three types of cells occurring at about 1150 V/cm. Average isotonic cytoplasmic resistivities are somewhat higher for the SS cells (166.7 +/- 7.49 ohm-cm) compared to the AA (147.6 +/- 1.98 ohm-cm) or AS cells (148.7 +/- 1.79 ohm-cm). As medium osmolality is vari

The abnormal adherence of sickle erythrocytes to endothelial cells (EC) has been hypothesized to play a role in the initiation of vaso-occlusion in sickle cell anemia. We studied erythrocyte/endothelial cell interactions under controlled flow conditions for normal (AA), homozygous sickle cell (SS), sickle cell trait (AS), mechanically injured normal, and "high reticulocyte control" red blood cells (RBC). Human umbilical vein endothelial cells grown to confluence on glass slides formed the base of a parallel plate flow chamber into which RBC suspensions were perfused at a constant flow rate, pr…

UNLABELLED: Twenty-four percent of the Gabonese population has sickle cell trait, and 1-3% has sickle cell disease. Patients' management must follow well-defined protocols that take into account the debilitating effect of severe pain episodes.
OBJECTIVES: To evaluate the three-step analgesic ladder for control of pain for sickle cell disease pain crises.
METHODOLOGY: This is a prospective and descriptive study based on surveys filled between February 2000 and March 2001. Surveys were restricted to sickle cell disease patients having developed pain crises before and during their stay at the h…

BACKGROUND: The 2 most common hemoglobinopathies, sickle cell trait and alpha (+)-thalassemia, confer partial resistance to fatal forms of malaria, but the molecular basis for this protection is still not understood. Examination of the relationship between these traits and malaria transmission intensity may provide insights into the protection afforded.
METHODS: The distribution of the 2 traits was assessed among children resident in 13 villages in the Eastern Arc Mountains in Tanzania, where Plasmodium falciparum transmission intensity is closely correlated with altitude. Associations betwee…
