BACKGROUND: Petroclival and foramen magnum meningiomas sometimes encase the vertebrobasilar arterial system. Magnetic resonance imaging can clearly reveal such encasement. The case presented here was of a meningioma involving the lower clivus and the foramen magnum, encasing a lateral segment of the posterior inferior cerebellar artery (PICA), despite the fact that no definitive diagnosis of the encasement of the PICA was made on preoperative radiological examination. End-to-end anastomosis of the PICA was necessary before excision of the tumor.
METHODS: A 55-year-old woman presented with com

Endoscopic endonasal surgery (EES) offers a direct ventral corridor to the skull base, potentially enabling maximal resection of extensive cholesteatomas (epidermoid cysts) in the posterior cranial fossa and parasellar region. However, outcomes and complications specific to this approach require detailed analysis. A retrospective review was conducted on 34 patients who underwent EES for cholesteatomas of the posterior fossa and parasellar region at a single center from 2011 to 2025 year. Extent of resection (near-total > 95%, subtotal 90-95%), complications, and neurological outcomes were anal

OBJECTIVE: To compare dosimetric differences between intensity-modulated proton therapy (IMPT) and photon-based radiotherapy (VMAT/IMRT) in the treatment of skull base tumors, focusing on target coverage and organ-at-risk (OAR) sparing.
METHODS: All patients with skull base tumors who underwent proton therapy between December 2021 and August 2022 were included in this study. For each case, both IMPT and VMAT/IMRT plans were generated using Eclipse TPS, with robust optimization applied for protons. Dosimetric metrics for target volumes and OARs-including the brainstem, spinal cord, visual path

Skull base osteoradionecrosis (sbORN) is a severe complication of radiotherapy (RT) in patients with nasopharyngeal carcinoma (NPC) that can severely affect quality of life (QOL) and may even be life-threatening. The etiology and pathogenesis of sbORN remain largely unknown or uncertain. Therefore, this study aimed to identify potential risk factors for sbORN by analyzing the laboratory assessments of patients to enable early clinical interventions. This retrospective case-control study reviewed NPC patients who were pathologically diagnosed with sbORN after primary radical radiotherapy. These

BACKGROUND: Endoscopic endonasal surgery (EES) requires wide sinonasal dissection to achieve adequate visualisation of anatomical landmarks. This can also result in nasal sequelae like crusting, nasal discharge, and anosmia.
AIM: To use the sinonasal outcome test (SNOT) 22 questionnaire to study the postoperative sinonasal morbidity and its recovery in patients who have undergone EES.
MATERIALS AND METHOD: Prospective study conducted from November 2017 to May 2018. SNOT-22 questionnaire was administered on patients operated between before EES and then re-administered during outpatient visits

The aim of this work was to determine reliable anatomical landmarks for locating and preserving the abducens nerves (6th cranial nerves) during trans-facial or trans-nasal endoscopic approaches of skull base tumors involving the clivus and the petrous apex. In order to describe this specific anatomy, we carefully dissected 10 cadaveric heads under optic magnification. Several measurements were taken between the two petro-sphénoidal foramina, from the bottom of the sella and the dorsum sellae. The close relationship between the nerves and the internal carotid artery were taken into account. We

BACKGROUND: Predicting the displacement of cranial nerves by tumors could make surgery safer and the outcome better. Recent advances in imaging and processing have overcome some of the limits associated with cranial nerve tractography, such as spatial resolution and fiber crossing. Among others, probabilistic algorithms yield to a more accurate depiction of cranial nerve trajectories.
OBJECTIVE: To report how cranial nerve probabilistic tractography can help the surgical strategy in a series of various skull base tumors.
METHODS: After distortion correction and region of interest seeding, a

BACKGROUND: Nerve transfers have been widely used to reanimate paralyzed facial muscles after irreversible proximal injuries to the facial nerve. The author has developed a technique involving masseteric nerve transfer combined with cross-facial nerve grafting for treating skull base surgery-induced facial paralysis. This paper aims to demonstrate that this procedure is effective and causes negligible donor site morbidity.
METHODS: Seven patients who developed facial paralysis after the removal of skull base tumors were treated with masseteric nerve transfer combined with cross-facial nerve g

BACKGROUND: Skull base reconstruction can be accomplished using various donor sites. Vascularized tissue, commonly the nasoseptal flap, is the most effective option for large defects or high flow leaks. In cases where the septum cannot be used, a mucoperiosteal flap from the nasal floor, pedicled from the sphenopalatine artery, is a viable option without reported outcomes. The aim of this work was to describe this flap and to report successful outcomes in a cohort of patients.
METHODS: Retrospective chart review of patients seen by the senior author from 2011 to 2013 requiring skull base reco

We report two patients with a carotid body paraganglioma that extended to the skull base, a position that is surgically inaccessible by means of a traditional lateral cervical approach. In both patients we were able to remove the lesion by performing a double mandibular osteotomy. Both patients underwent preoperative embolization to reduce the mass. In our experience, this approach has allowed a safe radical excision of exceptionally high lesions with only minor permanent nerve damage. In our opinion this advantage definitely outweighs the consequences of the increased invasiveness of this tec

From January 2000 to January 2009, 15 patients presented with large solid hemangioblastomas of the posterior fossa; eight of these patients were also diagnosed with von Hippel-Lindau (VHL) disease. Diagnostic imaging showed large vascular lesions. All 15 patients underwent surgery through a suboccipital ipsilateral, modified far-lateral, suboccipital midline or suboccipital supracerebellar approach. Preoperative embolization was attempted in seven patients. Complete removal of the tumor was performed in all patients. An overall neurological improvement was observed in 11 of the 15 patients, co

Skull base approaches play a fundamental role in modern neurosurgery by reducing surgical morbidity. Increasing experience has allowed surgeons to perform minimally invasive approaches without straying from the premises of skull base surgery. The eyelid approach has evolved from the orbitopterional osteotomy into a more effective and targeted approach to disease of the anterior cranial fossa. In this technique, after an incision is made on the supratarsal fold, the orbicularis oculi muscle is incised, and a myocutaneous flap composed of the elements of the anterior lamella is elevated. Complex tumors OF the glomus jugulare present a surgical challenge because of their difficult location, extreme vascularity, and involvement with multiple cranial nerves. Modern microneurosurgical and cranial base techniques have enabled safe total removal of these complicated tumors. We describe a one-stage transjugular posterior infratemporal fossa approach for radical resection of glomus jugulare tumors located around the jugular foramen, the lower clivus, and the high cervical region from an anterolateral direction. This approach is a combination of transmastoid, suprajugular, transjugular

BACKGROUND: Classic craniofacial resection and the subcranial approach are well-established techniques for the removal of tumors involving the anterior skull base. These techniques require frontal craniotomy to allow broad exposure of the anterior cranial fossa, a procedure that may be further complicated by local infection and osteomyelitis or because of a reduction in tissue perfusion and direct communication between the osteotomy and the contaminated nasoethmoidal cavity. The authors present a new method for wrapping of the frontal bone segment with a pericranial flap with the intention of

We describe our experience in treating 7 patients who underwent skull base reconstruction with free flap (6 latissimus dorsi, 1 rectus abdominis) between October 1996 and November 1998. Four patients underwent temporal bone resection with auricular resection, 2 patients underwent anterior and middle cranial fossa resection, 1 patient underwent frontotemporal resection. There have been no failures of the free flaps and one cerebrospinal fluid leak. We advocate free flap reconstruction after temporal bone resection with auricular resection, and after anterior or middle cranial fossa resection wh
