Advanced simulation and numerical evaluation of pharmaceutical solubility estimation under supercritical processing using artificial intelligence computations.

Poor aqueous solubility of medicines has been a subject of great interest for improvement of pharmaceutical industry. Computational analysis of solubility data is of great importance for evaluation of supercritical processing which can be used to enhance drugs solubility. A computational method for estimating the solubility of pharmaceuticals in supercritical carbon dioxide (SC-CO2) at varying processing conditions was developed using machine learning. Leveraging an innovative combination of experimental data and advanced machine learning techniques, this study investigates the prediction of b

A case of spontaneous acalculous gallbladder perforation difficult to diagnose on computed tomography.

Gallbladder perforation is a rare and life-threatening event. Accurately diagnosing gallbladder perforation without cholecystitis or gallstones is difficult. The patient was admitted with abdominal pain and was diagnosed with diffuse peritonitis the following day via computed tomography (CT). Laparoscopic findings revealed the accumulation of bile-like ascites and a thinned and soft gallbladder with slight leakage of bile juice from a pinhole perforation. Laparoscopic cholecystectomy and peritoneal lavage were then performed. In the present case, it was difficult to diagnose gallbladder perfor

A Mimicker of Malignancy: A Case Report on Elastofibroma Dorsi.

Elastofibroma dorsi is a soft tissue tumor rarely identified on imaging. It is most commonly located in the infrascapular region, deep to the serratus anterior and latissimus dorsi of older women. We present a case of an asymptomatic female patient with a history of left rotator cuff repair and chronic lymphocytic leukemia in remission who was found to have elastofibroma dorsi. As a benign tumoral entity that can look hypermetabolic on positron emission tomography-computed tomography scan (PET-CT), this case demonstrates the importance of an effective clinical diagnosis of elastofibroma dorsi

[18F]FDG PET/CT in Necrotizing Fasciitis: A Case Series Investigating the Clinical Utility in a Challenging and Rare Condition.

Necrotizing fasciitis is a deadly yet rare soft tissue and skin infection that is usually diagnosed clinically. At times, clinical signs may betray the underlying etiology and masquerade as cellulitis in the early course of the disease. We report four cases with clinical suspicion of necrotizing fasciitis, some after the failure of therapy for cellulitis who underwent 18-F fluorodeoxyglucose positron emission tomography-computed tomography (18-F FDG PET/CT) showing the extent of the disease, showing subclinical sites of involvement in patients with necrotizing fasciitis on baseline scan as wel

Imaging of Venolymphatic Malformation in a Child Extending from Inguinoscrotal Region to Thoracic Region: Case Report with Review of Literature.

Vascular malformations (VM) are structural malformations of vascular development causing soft-tissue abnormality with functional and esthetic impairment. They are named by their predominant vessel type as arterial, venous, lymphatic, or mixed types. VM extending from the inguinoscrotal to the thoracic region are extremely rare presentation. We present a rare case of veno-lymphatic malformation in the inguinoscrotal region, which is extending superiorly up to the right thorax in a 14-year-old male child who presented with a large swelling in the bilateral inguinoscrotal region and reddish-brown

A case of mediastinal mesenchymal tumor with pericytic neoplasm feature that responded to radiation therapy.

Here, we report a case of mediastinal mesenchymal tumor with a pericytic neoplasm feature that responded to radiation therapy. A 43-year-old man visited our hospital with a complaint of esophageal obstruction and chest pain. Chest computed tomography revealed a middle mediastinal tumor and a mesenchymal tumor was diagnosed with a pericytic neoplasm feature by video-assisted thoracoscopic biopsy. The definitive treatment for soft tissue tumor is surgical resection; however, the mediastinal tumor was unresectable because of esophageal and tracheal invasion. Radiation therapy was administered and

Computed tomographic and bronchoscopic diagnosis of Oslerus osleri infection in a dog.

A 16-month-old Sarplaninac Shepherd cross dog presented for a 1-month history of a productive cough that was unresponsive to an empirical 10-day course of cephalexin. Thoracic computed tomography (CT) showed multifocal, well-defined, smoothly marginated, soft tissue attenuating, minimally contrast enhancing nodular airway mural thickenings protruding into the airway lumen in the caudal trachea and principal bronchi. These nodules were also visualized on bronchoscopy, and cytology revealed parasitic larvae consistent with Oslerus osleri. The dog was treated with oral fenbendazole for 26 days. C

Hidden problems in a case of atrial fibrillation.

Atrial fibrillation (AF) is the most common cardiac arrhythmia. However, the relation between congenital heart defects and the predisposition to AF is not fully understood. A 65-year-old male was admitted into the emergency department due to progressive dyspnea, orthopnea, palpitations, and edema. Transthoracic echocardiogram showed bi-atrial enlargement and dysplasia of the mitral leaflets with severe mitral regurgitation. Also, a membrane was noted in the LA, dividing the chamber into two parts, suggestive of cor triatriatum sinister. Coronary computed tomography angiography demonstrated a s

BACKGROUND: Pott's puffy tumor (PPT) is a rare and potentially deadly complication of frontal sinusitis consisting of subperiosteal abscess and osteomyelitis of the frontal bone. CASE PRESENTATION: We report the case of a 9-year-old boy who presented with fever and soft tissue swelling of the forehead. Magnetic resonance imaging (MRI) depicted an abscess in the subcutaneous tissue frontally and an epidural empyema, while a cranial computed tomography (CT) scan revealed bone erosion as a sign of osteomyelitis. The patient was treated accordingly. CONCLUSIONS: This rare condition is essential

Cystic retroperitoneal dedifferentiated liposarcoma: A case report.

Liposarcoma is the most common primary retroperitoneal sarcoma in adults. We report the case of an 86-year-old male who presented to the emergency department with frequent falls and unexplained weight loss that was found to have a cystic retroperitoneal dedifferentiated liposarcoma. Initial computed tomography revealed a large heterogeneous complex cystic hypoenhancing lesion in the left retroperitoneum. Subsequent magnetic resonance imaging demonstrates a multilocular cystic mass with microscopic lipid content, diffusion restriction, and enhancing nodular soft-tissue components. Histologic ex

Cranial Mediastinal Thyroglossal Duct Adenocarcinoma in a Dog.

A 7 yr old female spayed Canaan dog was presented for evaluation of a subclinical intrathoracic mass noted during meta-static staging for a digit and lingual mass. Thoracic ultrasound and computed tomography revealed a 6.0 × 5.5 cm mass within the cranial mediastinum. The mass was septated with mixed fluid and soft-tissue structures and no evidence of vascular invasion. Narrow excision of the mass via a sternal thoracotomy was performed. Histopathology of the mass was consistent with a thyroglossal duct cyst adenocarcinoma. This is the first reported case of a thyroglossal duct cyst adenocarci

Testicular and Spermatic Cord Metastases from Gastric Adenocarcinoma: An Unusual Case.

The metastasis of gastric cancer to the testis or spermatic cord is very rare; however, simultaneous testicular and spermatic cord metastases are even rarer. Here, we report an unusual case of a 54-year-old male patient who had undergone radical gastrectomy for gastric adenocarcinoma more than two years before. He complained of palpable painful masses in the right scrotum and inguinal area occurring for more than half a year. Enhanced computed tomography (CT) separately showed an enhanced soft tissue mass in the right testicular and spermatic cord areas. Therefore, a right radical orchiectomy

Emerging Roles of PET/MR in the Pediatric Hospital.

Add "improving" before "detection"? PET/MR is beneficial particularly in pediatric patients who undergo recurrent imaging, such as those with cancer or chronic inflammatory disease. PET/MR has advantages compared with PET/computed tomography, including decreased radiation exposure and superior characterization of soft tissue. Ongoing challenges include reducing examination duration and costs and detection of pulmonary lesions. Accepted clinical applications of PET/MR in pediatric patients are evaluation of epileptic foci and diagnosis, staging, and follow-up of solid tumors. PET/MR also may ha

Clinics in diagnostic imaging (194). SVC aneurysm.

A 68-year-old woman presented with weight loss of 7 kg over two years. Clinical examination revealed no specific abnormality and the patient was otherwise asymptomatic. Chest radiography performed to screen for malignancy revealed a soft tissue opacity projected over the mediastinum. Computed tomography of the thorax showed an enhancing anterior mediastinal mass with heterogeneous enhancement and contrast pooling in the arterial phase, followed by homogeneous enhancement in the venous phase, consistent with an aneurysm arising from the superior vena cava. This case highlights superior vena cav

G-CSF-producing Undifferentiated Pleomorphic Sarcoma Adjacent to the Ascending Colon and in the Right Iliopsoas Muscle: A Case Report and Review of the Literature.

Undifferentiated pleomorphic sarcoma (UPS) is a soft tissue sarcoma, occurring most commonly on the lower extremities. We herein report a rare case of primary UPS adjacent to the ascending colon and in the right iliopsoas muscle. Computed tomography of the abdomen revealed large masses, and the patient experienced a high-grade fever, leukocytosis, elevated serum C-reactive protein level, and hematopoietic activation on 18F-fluorodeoxyglucose-positron emission tomography. This inflammatory reaction was caused by granulocyte colony-stimulating factor secreted by tumor cells. Surgical resection w

Primary Undifferentiated Pleomorphic Sarcoma of the Colon Mesentery.

An undifferentiated pleomorphic sarcoma (UPS), also known as a malignant fibrous histiocytoma in the past, commonly involves the soft tissue of the extremities and the retroperitoneum. However, a primary UPS of the colon mesentery is very rare. A 69-year-old male patient visited our outpatient department for treatment of an enlarged, palpable mass in the right lower quadrant (RLQ). Computed tomography showed a 15-cm multilobulated, heterogeneous, enhanced mass in the RLQ, which we suspected originated from the colon. He underwent a right hemicolectomy, and the pathologic result was a colon mes

Severe tracheal stenosis after first administration of pembrolizumab rescued by Dumon Y-stent in a lung cancer patient.

A 70-year-old woman diagnosed with advanced, non-resectable programmed cell death ligand 1-positive-non-small-cell lung carcinoma was treated with pembrolizumab as first-line therapy. Soon after therapy initiation, she presented with severe dyspnea, and chest computed tomography revealed a soft tissue mass in the lower trachea of the right main bronchus. During bronchoscopy, she became severely hypoxic, and we performed endoscopic tumor ablation and Dumon Y-stent placement. We considered this severe deterioration caused by pseudoprogression, and suggest that it is necessary to perform bronchos
