Fluctuating asymmetry and developmental instability in sagittal craniosynostosis.

OBJECTIVE: To determine whether premature sagittal craniosynostosis is associated with developmental instability in the skull by analyzing fluctuating asymmetry in skull shape. DESIGN: Cranial shape was quantified by collecting coordinate data from landmarks located on three-dimensional reconstructions of preoperative computed tomography (CT) images of 22 children with sagittal craniosynostosis and 22 age-matched controls. A fluctuating asymmetry application of Euclidean distance matrix analysis (EDMA) was used to quantify and compare asymmetry in cranial shape using these landmark data. RES

Reproducibility of three-dimensional cephalometric landmarks in cone-beam and low-dose computed tomography.

OBJECTIVES: The purpose of this study is to compare the reproducibility of three-dimensional cephalometric landmarks on three-dimensional computed tomography (3D-CT) surface rendering using clinical protocols based on low-dose (35-mAs) spiral CT and cone-beam CT (I-CAT). The absorbed dose levels for radiosensitive organs in the maxillofacial region during exposure in both 3D-CT protocols were also assessed. MATERIALS AND METHODS: The study population consisted of ten human dry skulls examined with low-dose CT and cone-beam CT. Two independent observers identified 24 cephalometric anatomic lan

Morphometric analysis of treatment effects of bone-anchored maxillary protraction in growing Class III patients.

The aim of the present morphometric investigation was to evaluate the effects of bone-anchored maxillary protraction (BAMP) in the treatment of growing patients with Class III malocclusion. The shape and size changes in the craniofacial configuration of a sample of 26 children with Class III malocclusions consecutively treated with the BAMP protocol were compared with a matched sample of 15 children with untreated Class III malocclusions. All subjects in the two groups were at a prepubertal stage of skeletal development at time of first observation. Average duration of treatment was 14 months.

Basal skull fractures are associated with mortality in pediatric severe traumatic brain injury.

BACKGROUND: Basal skull fractures (BSFs) are caused by blunt force trauma, occurring in the temporal, occipital, sphenoid, and/or ethmoid bones. In pediatric severe traumatic brain injury (sTBI), there is a paucity of data on BSFs. Our goal was to investigate the BSF prevalence, anatomy, and association with short-term outcomes in pediatric sTBI. METHODS: We retrospectively reviewed all severely injured (Injury Severity Score ≥12) pediatric patients (aged <18 years) admitted to our hospital after experiencing an sTBI (Glasgow Coma Scale score ≤8 and head Abbreviated Injury Scale score ≥4). Ne

A morphometric analysis of the craniofacial configuration in achondroplasia.

Human achondroplasia can be viewed as an experimental model for studying the effects of abnormal endochondral bone formation on the development of the skull as a whole. In this study, lateral cephalograms of 25 adult males and 26 adult females with achondroplasia were converted to a two-dimensional coordinate model of craniofacial morphology and analyzed using 66 linear, angular, and area variables. Lateral cephalograms of 951 normal adults were used for comparison. Two sample t-tests were used to compare achondroplastic cephalograms with normal cephalograms. Multivariate statistical analysis

Skeletal and soft-tissue incidental findings on cone-beam computed tomography images.

Cone-beam computed tomography provides orthodontists with 3-dimensional images of the craniofacial region and valuable information for diagnosis and treatment planning of craniofacial or dental anomalies. However, a narrow focus on the skeletal and dental contributions to malocclusion can cause failure to identify skeletal or soft-tissue pathologies of the craniofacial structures unrelated to the orthodontic concerns. Two cases are presented that demonstrate skeletal and soft-tissue anomalies identified as incidental findings on cone-beam computed tomography scans of asymptomatic orthodontics

Observations on the growth of temporalis muscle: A 3D CT imaging study.

This study investigates the hypothesis that there is, during childhood, a disproportionate age-related expansion of the origin of temporalis muscle compared to the growth of the underlying skull. Lateral projections of 50 randomly selected 3D reformatted computerized tomographic (CT) scans (yielding 100 temporalis muscles) of children aged >0.6 to 15 years scanned for conditions that did not affect the shape of their head or face were windowed to provide the optimum delineation of temporalis muscle against the underlying bone. Vertical and anterior-posterior measurements of the muscle made ind

Shape analysis of the craniofacial skeleton in children prenatally exposed to anticonvulsant medications using geometric morphometrics.

Children exposed prenatally to antiepileptic drugs may have a typical facies characterized by midfacial retrusion, a short nose, and anteverted nares. Our aim was to determine whether the shape of the maxilla was altered in its sagittal displacement, or whether the defect in the underlying articulation with the cranial base was responsible for the appearance of midface retrusion. Our hypothesis was that the sphenoid bone as well as the maxilla and other bones in the cranial base were affected by the anticonvulsant medication. The lateral cephalograms of 65 children exposed prenatally to monoth…

Synchronized "One-Step" Resection and Cranio-Orbital Reconstruction for Spheno-Orbital Lesions With Custom Made Implant.

BACKGROUND: In the last few years, skull base tumors involving the fronto-orbital region have been approached with complex, invasive, and time-consuming cranial reconstruction techniques. On the other hand, recent custom-made implants allow easier and faster procedures, with excellent aesthetic results. The authors propose an easy surgical-planned protocol with a synchronized "one-step" resection and reconstruction of these complex lesions, with a preformed poly-methyilmethacrylate (PMMA) cranioplasty. METHODS: Our technique consists of a 2-phases procedure. In the first one, the so-called "v

Endoscopic anatomy of the pterygopalatine fossa and the transpterygoid approach: development of a surgical instruction model.

INTRODUCTION: The pterygopalatine fossa (PPF) is a narrow space located between the posterior wall of the antrum and the pterygoid plates. Surgical access to the PPF is difficult because of its protected position and its complex neurovascular anatomy. Endonasal approaches using rod lens endoscopes, however, provide better visualization of this area and are associated with less morbidity than external approaches. Our aim was to develop a simple anatomical model using cadaveric specimens injected with intravascular colored silicone to demonstrate the endoscopic anatomy of the PPF. This model cou

Traumatic, spontaneous and postoperative CSF rhinorrhea.

CSF fistulas are a major complication of head injury but also occur spontaneously or symptomatically in connection with tumours of the skull base, empty sella syndrome, ethmoidal encephalomyelocele, intracranial hypertension or postoperatively in connection with operations on skull base tumours or ENT operations. Their main risk is the possibility of meningitis. The main clinical symptom is CSF leakage from the nose, but meningitis may be the first manifestation. Isotope cisternography and metrizamide CT cisternography are the most important methods for precise localization, sometimes also for

Application-Dependent Early-Phase Stability of Autologous Fibrin Glue in Endoscopic Transsphenoidal Surgery.

BACKGROUND: In endoscopic transsphenoidal surgery (eTSS), fibrin sealants are applied in a confined and frequently inclined operative field, where early-phase material behavior may influence sealant retention and procedural controllability before complete polymerization. Autologous fibrin glue is characterized by slower polymerization kinetics, which may influence its early-phase coagulation behavior immediately after application. However, the clinical relevance of this early-phase coagulation behavior and its association with application techniques remain unclear. METHODS: We retrospectively

Spheno-orbital meningiomas: predictors of recurrence and novel strategies for surgical management.

OBJECTIVE: Spheno-orbital meningiomas (SOMs) represent approximately 2%-9% of tumors affecting the sphenoid wings. The main challenge in treating these meningiomas is achieving gross-total resection (GTR) due to the hyperostosis, which is pathognomonic. This study explored potential correlations between the recurrence rate of SOM and the extent of resection (EOR) of both the bony tumor portion (BTP) and soft tumor portion (STP). Additionally, it analyzed the progression-free survival (PFS) of SOM patients in relation to the EOR, adjuvant treatments, and other recognized risk factors, including

Optimum Surgical Strategies for Medial Sphenoid Wing Meningiomas: A Comprehensive Evaluation of Resection Extent, Visual Outcomes, and Vascular Injury.

BACKGROUND/OBJECTIVE: Visual impairment affects 55%-80% of medial sphenoid wing meningiomas (mSWMs) patients, making optic nerve decompression a critical surgical goal. Complete resection often leads to better visual outcomes. However, involvement of critical neurovascular structures increases postoperative morbidity and mortality, with vascular injury reported in 18%-20% of cases. This study aims to evaluate the relationship between the extent of resection (EOR), visual outcomes, and the incidence of vascular injury, seeking to identify the optimal surgical approach for mSWMs. METHODS: We re
